Key Responsibilities

Completes daily tasks such as following up with patients, conducting NIV follow-ups, and making phone calls to patients, families, or caregivers as needed.
Conducts NIV Setups and home visits, and participates in on-call duties as required.
Supports the continuum of care by creating and overseeing care plans, communicating with physicians or other clinicians, and resolving patient care issues whenever possible.
Maintains up-to-date knowledge of equipment and services, may assist with quality control, equipment maintenance, and support daily Clinical Outcomes Team operations.
Maintains current understanding of insurance benefits, coverage, and patient costs to explain them adequately to patients and caregivers.
Ensures compliance with HIPAA, The Joint Commission, State Respiratory Care Board, State Pharmacy Board, and all other applicable rules and regulations.

Position Summary

RESPONSIBILITIES
The Clinician here for La Jornada will perform duties including, but not limited to:
* Perform all work in a manner consistent with the National Youth Advocate Program's mission, values, and philosophies.
* Collaborate with the cross-functional service team to provide trauma-informed mental health related services including conducting required assessments, referrals to mental health providers and assessments to identify human trafficking concerns.
* Provide child-facing services on-site.
* Maintain an active clinical caseload pursuant to the assigned program ratio and consistently achieve established service delivery thresholds.
* Conduct mental health assessments; and provide ongoing individual and group counseling services, as well as provide crisis intervention if needed.
* Facilitate family sessions as needed.
* Perform visits at foster caregiver's homes within pre-established days from the child's admission as needed.
* Continually monitor the mental well-being of clients and arrange for appropriate services as needed, including arranging for and facilitating external specialized services with community mental health providers.
* Complete Risk, UC Assessments, Human Trafficking, RST assessment, safety plans, post 18 ages out plans, and other assessments as required by the assigned program within expected timeframes.

* Participate in staffing with the cross-functional team and stakeholders.
* Provide orientation to children in care.

IHBT Clinician; LSW Compensation: $63,000 Intensive Home-Based Treatment (IHBT) Clinician works to make a meaningful difference in the lives of the children and families served. This position works closely with youth, family, foster parents, and community partners, providing counseling or therapy, advocacy, and support toward living safely and successfully in the community.

Responsibilities
The Intensive Home-Based Treatment (IHBT) Clinician will perform duties including but not limited to:
* Perform all work in a manner consistent with the National Youth Advocate Program's mission, values, and philosophies.
* Actively supports, represents, and extends the organization's mission, vision, and values.
* Provides appropriate individual, group, and family mental health services, case management, transportation, and other professional services, meeting or exceeding established outcomes thresholds for persons served.
* Travels daily to provide community-based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements
. • Completes all required documentation promptly.
* Completes all required training promptly.
* Consistently achieves established productivity thresholds.
* Coordinate and monitor services for youth and families in the community, including, but not limited to, mental health, medical, educational, psychological, vocational, and social services.
* Regularly reviews and modifies youth-based plans to ensure consistent progress and success on written plan goals.
* Establishes and maintains strong relationships with assigned foster parents and families of origin.
* Assists in family reunification and/or stabilization efforts with the youth's primary family or other permanent living arrangement consistent with the youth's case plan.
* Provides safety assessment, crisis response, and behavioral stabilization services as required to assure the safety and stability of the youth served.
* Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis.
* Actively participates in the weekly supervision process.
* Performs other duties as requested.
Minimum Qualifications
* Bachelor's degree in Social Work.
* Active and valid LSW licensure.
* Two years of work experience working in direct service with youth and families strongly preferred.
* A valid advanced professional license as required and acceptable by state or contract regulations.
* A willingness to work flexible and non-traditional hours in the service of foster caregivers, families of origin, and persons served.
* Capacity to be flexible and responsive to youth served, foster caregivers, system partners, and internal customers.
* Capacity to remain objective and professional in all areas of job function.
* Demonstrates tolerance and respect for the ideas and actions of others.
* Capacity to effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds.
* Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines, photocopiers, including word processing, spreadsheet, and database programs.
Driving and Vehicle Requirements
* Valid driver's license
* Reliable personal transportation
