Psychiatric Evaluator EmergencyDepartment RN/LMSW
Our team of professional caregivers will do a thorough evaluation to help determine what is causing your distress and how we can best help you. We will check carefully for any underlying medical illness or biochemical origins to your symptoms. We will then work with you, your family, and other health-care professionals to develop a course of treatment that is right for you.
Roles and Responsibilities:
Responsible for collection of comprehensive biopsychosocial data, assessment/determination of lethality/risk, pre-certification of medical care with insurance/managed care entities.
Provide liaison services between emergencydepartment providers and psychiatric providers in determining the disposition of clients in need of psychiatric evaluation
Position is responsible for determination of need for clinical consultation with psychiatrist and medical consultation with emergencydepartment staff to allow timely patient disposition.
If RN, receive physician orders for disposition (If SW transfer to RN for receipt of orders).
Shift: Day/evening flex
Required Skills and Experience:
Previous experience in mental health field, preferably in an acute care inpatient psychiatric setting;
Provide psychosocial assessment and interview of patients, clients, family members and/or persons deemed necessary to obtain the most complete case history in the EmergencyDepartment or ED Flex space;
Complete documentation of psychosocial evaluation to include a formulation of all data leading to a decision for disposition, legal status paperwork, insurance authorization, releases of information appropriate to data collection.
Physical Requirements:
Audio/visual acuity and physical dexterity to interact appropriately with patients, peers, visitors, staff, and outside customers/agencies;
Ability to do long periods of standing, walking, bending and stretching, and ability to change position and body motion quickly;
Ability to lift and assist in lifting, utilizing appropriate body mechanics, without weight restriction;
Ability to participate as a member of the team trained to physically subdue and restrain individuals in behavioral dyscontrol, of all sizes and strengths. This activity includes, but is not limited to, lifting, holding, straining and running.

The ED Technician is responsible for working closely with the nurse to provide excellent direct patient care supporting activities of assessment, testing, and diagnosis of patients presenting to the emergencydepartment, and helps prevent complications.

* Direct Patient Care- Toileting, ambulating, repositioning, monitoring and recording intake and output of fluids, recording patient weight, obtaining vital signs, and reporting abnormal findings or changes in the patient's conditions to the RN.
* Performing direct observation and safety monitoring of patients assessed to be a threat to others, themselves, or ongoing medical interventions
* Intentionally rounding on each patient every hour.
* Perform 12 lead EKGs, Venipuncture and Post Care, Specimen Labeling, Point of Care Testing, prep and transport of instruments to Sterile Processing
* Safely transporting patients by stretcher or wheelchair outside the department for testing or admission
* Mandatory Orientation Training completion required Minimum of one full week of Monday through Friday 8a-4p orientation prior to starting work on the unit.

What you'll do
The Registered Professional Nurse IV, Acute Care is accountable for the delivery of safe patient care utilizing the nursing process. The RN assesses, plans, delivers, coordinates, integrates, and evaluates nursing care related to patient's needs. The RN contributes to the support of patients through interdisciplinary teamwork. RN IV is accountable to assure that the care provided is consistent with applicable standards, regulations, statutes and state, federal law. RN IV serves as a resource for other staff and clinicians as required.
The RN IV provides leadership to department staff, serving as a resource and mentor. The RN IV contributes to the body of professional nursing practice through participation in research, committee work, and professional organizations, internal and external.
* Provides nursing care and services to patients throughout the life span and in various settings consistent with educational preparation.

* Directs and supervises patient care provided by LPNs and support personnel 100% of the time according to department guidelines, as measured by co-worker feedback and managerial observation.
* Demonstrates consideration of safety, effective utilization of equipment and materials, and cost in managing patient care, as measured by direct supervisory observation and co-worker feedback.
* Acts effectively as charge nurse, group leader or preceptor when assigned, as determined by co-worker or preceptee feedback and direct managerial observation.
* Writes a progress note that demonstrates an understanding of the patient's status and progress toward achievement of goals (80-90%).
* Effectively precepts new staff members or students as determined by preceptee/student feedback and direct managerial observation.
* Ability to function in charge nurse or leadership capacity as determined by practice arena.
* Advocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights.
* Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity, 100% of the time, as measured by no significant, substantiated patient or peer complaints received by the manager/supervisor.
* Assures that patients have informed consent signed for indicated procedures, 100% of the time, as measured by random supervisory chart review.
* Acts on behalf of patients in an ethical manner 100% of the time, as measured by co-worker and patient feedback and managerial observation.
* Adheres to ANA Code of Ethics for Professional Nurses.

* Completes required unit credentialing according to departmental policy by providing documented evidence of completion to supervisor.

What we need from you
Education:

* All RN's hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor's prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN's
licensed on or before 12/19/17 are/remain exempt from this requirement.

Overview
Under the general supervision of the Business Manager and the Director of Practice Operations, provides direct nursing care for assigned patients at Pediatric Subspecialty, Bay Parkway Faculty Practice. Collaborates with providers, patients and their families, as well as other members of the health care team to formulate a plan of care and perform the duties of a registerednurse.
Responsibilities
A. ASSESSMENT
* Assessment, intervention, evaluation and health education are delivered within a knowledge framework of the growth and development stage appropriate to the patient.
* Initial observation and assessment of the patient's needs for immediate nursing action via telephone or walk-in triage.
* Assesses and intervenes appropriately to meet patient & families developmental, psychosocial, educational and home care needs.
* Plans and implements appropriate teaching about disease plan of treatment, side effects, diagnostic tests and psychosocial issues.
B. DIRECT PATIENT CARE
* Gives comprehensive patient care inclusive of but not limited to: immunizations, P.P.D.'s, medications, various injections, height and weight, head circumference, venipuncture, and repeat PKU's, allergy readings and other additional requirements requested by the provider.
* Collaborates with all members of the interdisciplinary health care team.
* Utilizes patient care standards in clinical practice, incorporating regulatory requirements D.O.H. and JCAHO standards.
* Maintains departmental reports and records and collects statistical data for administrative and regulatory purposes i.e. Citywide Immunization Registry entries and follow-ups.
* Acts as liaison between the Pediatric Center D.O.H., Early Childhood Intervention Centers, and multiple school districts. (school forms, W.I.C. forms etc.)
* Develops educational material for patients and families in multicultural languages. Initiates and follows through with patient and family teaching appropriate to the level of understanding and language of the family/patient.
* Initiates referrals to appropriate health professionals, home care services and/or community services as needed.
* Seeks clarification of authorized prescribed orders, protocols or standards that are unclear or inappropriate for age/population.
* Interprets clinical data and laboratory findings and notifies the physician promptly if immediate changes in management may be indicated. Documents findings and dispositions in the electronic medical record.
* Assesses and coordinates patient pain management.
C. EMERGENCY MEASURES
* Recognizes impending respiratory or cardiac failure and institutes appropriate measures of initiating CPR and calling 911 in an outpatient setting.
D. SAFETY
* Checks to ensure that emergency equipment is on hand and in good working order.
* Recognizes and reports malfunction or failure of equipment used to Biomedical Engineering; takes appropriate action to replace such equipment.
E. PEDIATRIC NURSING SKILLS
* Competent nursing skills appropriate for age/population specific growth and developmental stages, and clinically carry out interventions within the developmental framework.
F. INDIRECT PATIENT CARE ACTIVITIES
* Applies principles of infection control and safety.
* Incorporates the family into patient care; is proficient in teaching parents regarding specific care aspects of their child.

Qualifications
Education:

* Participates in patient care rounds by presenting findings on patients.
* Applies principles of infection control and safety.
* Assists in the orientation of new personnel.
* Provides clinical supervision of LPNs, Nursing Attendants, Patient Care Technicians, Unit Care Assistants and other ancillary staff.
* Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards; promotes adherence to Patient Bill of Rights.

Qualifications
Graduation from accredited school of Nursing.
Current NYS licensure as RegisteredNurse. BSN preferred.
