The primary functions of an LPN in assisting with assessment, planning, implementing and evaluating the care of all patients, regardless of age, during his/her shift. Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with Facility Policy.
Taking the patient's vital signs such as respiration, temperature, blood pressure, and heart rate and reporting changes in the patient's /residents conditions
Practices ethically and professionally within their scope; advocates for patient
Maintains clinical competence specific to their care area including organization required education within stated timeframe
Coordinates care with interdisciplinary team
Documents appropriately and thoroughly in EMR/Point Click Care
Keeps other nursing personnel and responsible RN informed regarding changes in condition of patients/ residents and / or other pertinent observations
Participates in unit councils and staff meetings.

The LicensedPracticalNurse supports the physicians and/or mid-level providers in the practice with clinical activities and direct patient care. The clinical assistant may also assist in appointment scheduling and other related activities for optimal patient service.
Roles and Responsibilities:
Perform clinical duties in accordance with the scope of practice per LPN license and in accordance with NYS laws and regulations.
Take and record patient's vital signs, prepare exam room and sterilize equipment and supplies per protocols.
Provide call management for physicians/providers with regard to clinical questions, prescription refills, test results and other patient calls per practice protocols.
Prepare patient's record for visit, as directed by physician/provider.
Participates in scheduled team meetings to discuss /clarify roles, responsibility, processes, and patient issues.
Required Skills and Experience:
Graduation from an accredited LPN program
Current NYS licensure as a LicensedPracticalNurse(LPN)

Previous Primary Care medical office experience

The LPN on Maternal Child Health is responsible for direct patient care to all patients within the Maternal Child Health department. The position will work collaboratively with all department members to implement and evaluate the patient's physical, psychological emotional, educational and spiritual needs under the direction of a Registered Professional Nurse. Strong interpersonal and excellent communication skills are required as well as being able to interact with people from a variety of cultures and socioeconomic backgrounds.
Roles and Responsibilities:
Dispenses medication within scope of practice and accurately documents in patients EMR. Plans for care of patients, demonstrating the ability to effectively prioritize both expected and unanticipated needs. Involves patients and families in establishing care goals
Obtaining data related to patient care and recording accurately in the patients EMR.
Reports abnormal findings or changes in mental and emotional conditions of the patient to the RN. Examples include changes in vital signs, abnormal weight gain or loss, inability to locate a fetal heart rate, increase in pain, etc.
Responds promptly to call bells and patient needs.
Know what surgical procedure is being performed, Follows AORN Guidelines/Hospital P&P for surgical hand scrub, anticipate surgeon's needs, make sure a "time out" has been called before handing scalpel to the surgeon, and maintain a sterile field.
Appropriately stocks all areas of the department, including the OR and does bi-weekly audits of the OR.
Knows how to appropriately perform and document a hearing screen.
Appropriately sets up, cleans (including equipment) and stocks after a delivery and C-section.
Full time night position with weekend and holiday rotation requirements.

Will obtain BLS during introductory period, ACLS, PALS, and NRP within 6 months of hire.
