Primary Purpose: To provide direct care in a residential setting to participants with intellectual and developmental disabilities who live in their own homes or apartments. To assist with providing engagement in their community

Essential Functions:
Provides essential physical and emotional care to participants, including,
Providing education and modeling to participants in personal hygiene, activities of daily living, and behavioral conduct while acting as a role model to enhance participants' self-esteem.
Coordinating and leading social and recreational activities, assisting the individual with shopping for food and clothing, and ensuring the safe transportation of participants.
Provide transportation to individuals for medical appointments, errands, activities, etc.
Completing/Assisting the participant with budgeting, general housekeeping, and household management.
Work with the individual to increase knowledge and understanding of enabling technology and digital literacy to help increase independence.
Nutritional support: assisting the participant in weekly meal planning/preparation.
Health/Wellness: assist the individual in attending their medical appointments as needed and communicating the results of those appointments with the Program supervisor.
Provide effective, respectful, appropriate, and courteous verbal and written communications,
To and from specific teams related to the care of participants, including observations and reports of any physical or behavioral changes while adhering to all documentation and record-keeping requirements.
With family members, guardians, DDS, and external healthcare providers, including coordinating health and hygiene-related appointments.
· Demonstrates knowledge of emergency procedures and responds appropriately to emergency situations.
· Responsibly manages and records the use of everyone's money according to the policies established by the CCARC, Inc. Board of Directors.
Collaborating to resolve conflicts, obstacles, and challenges, seeking guidance from manager and team members, as needed.
Demonstrates adherence to universal precautions, OSHA standards and infection control policies and procedures always, as well as department, program, and shift procedures.
Maintain good daily attendance (days of absence, illness, tardiness.) · Attends and participates in mandatory in-services, staff meetings, and conferences as required.

Technology Requirements:
Computer literate and proficient in Microsoft Office Word and Excel, Outlook, and be comfortable

Ability to work with and learn various assistive and enabling technology individual's independence.
Experience with THERAP, Sandata EVV systems preferred.
Education and/or Work Experience Requirements:
High School Diploma / GED
Must have valid Driver's License. Individual Home Support employees are required to have a vehicle for
use and maintain adequate insurance. Mileage is reimbursed.
Must complete the Agency's Employee Training
Certification and training, including but not limited to: CPR, First Aid, PMT (Psychological/Physical

Train, supervise, and support individuals at their worksites, following the worksite's dress code, vaccination requirements, and policies.

Ensure all information is updated in Therap for individuals on caseload.
Maintain regular contact and communication with the manager/designee regarding individual and staff issues, concerns, and updates.
Serve as a mentor/role model to all individuals and reinforce best practices.
Implement various employment service delivery models following each program's guidelines and individuals' Contract Service Agreement.
Obtain and maintain Customized Employment Certification.
Attend program-based training, in-services, and Communities of Practice, as appropriate.
As appropriate, assist the Employment Department in outreach, networking, marketing, and referral events.
Other Related Responsibilities:
