As a hybrid Kidney/Pancreas Transplant Pharmacist at Cedars-Sinai Medical Center in Los Angeles, California will maintain the quality and support of highly immunocompromised patients requiring high level of specialized services. The Kidney/Pancreas Transplant Pharmacist will help manage nephrology consults, pre-transplant infusions for highly sensitized patients, post-transplant infusion patients , and cover in-patient services including managing non-formulary requests. The Pharmacist will be responsible for entering therapy plans, conduct chart reviews, compose letters of medical necessity as well as participate in peer-to-peer reviews for denied insurance claims. The Pharmacist will also participate with in-patient rounding which would include discharge teachings, medication reconciliation, ordering and coordinating last minute medication changes prior to discharge.

Educational Requirements:

The Ambulatory Care Clinic Pharmacist at Cedars-Sinai Medical Center in Los Angeles, California is an Advanced Practice Pharmacist, an integral member of Cedars-Sinai's ambulatory and specialty pharmacy programs, embedded within specialty clinics to optimize complex, specialty or infusion therapy management, and enhance patient access. Collaborating with providers and interdisciplinary teams, a pharmacist in this role improves clinical outcomes and operational efficiency. Additionally, a pharmacist in this role provides comprehensive medication management, therapy monitoring, side effect management, and patient education for high-cost or complex specialty or infusion therapies. In alignment with the most recent clinical guidelines, a pharmacist in this role initiates, adjusts, and discontinues therapies, orders and interprets laboratory results, and authorizes medication refills. A pharmacist in this role also monitors and reports on adherence, therapeutic outcomes, and patient and provider satisfaction. Moreover, a pharmacist in this role provides patient education on specialty medications and acts as a clinical resource to all physicians and support staff. A pharmacist in this role contributes to creating or updating protocols, competency assessments and to continuous quality improvement initiatives.
Educational Requirements:
Graduate Degree - Graduate of an accredited School of Pharmacy - Minimum.
Completion of ASHP accredited residency program -- Preferred
Licenses and Certifications:
California Pharmacist license - required
Advanced Practice Pharmacist License -- required
Board certification in relevant practice area -- preferred

Previous experience in specialty or infusion therapy management in an ambulatory care setting preferred.

Documentation: Demonstrates the ability to accurately and promptly document all clinical interventions, assessments, and therapy changes in the patient's Electronic Health Record (EHR).
Project Management: Initiate, plan, design, implement, and monitor project(s) Ability to manage projects, establish timeliness, analyze data relevant to areas of responsibility. Ability to function independently, ability to plan, organize, direct, monitor and evaluate assigned work.
Budgetary & Financial: Possesses fiscal management skills and proven track record to prepare, monitor and control budgetary expenses and generate revenue.
Regulatory: Ability to interpret and apply knowledge of State, Federal and Agency standards to comply with regulations.
Technical: Ability to learn and adapt to the use of software, technology, and automated information systems in daily practice.
Physical Demands:
Frequent sitting; walking, bending; reaching, occasional pushing, pulling, lifting of materials up to 50 pounds.
Responsiveness to auditory alarms and communication devices including, telephones, pagers, fire alarms, and mechanical failure alarms.
Sufficient mobility to access equipment, patients, and other customers.

What you'll de doing in this role:
The Pharmacist is licensed by the California State Board of Pharmacy. Under the general supervision of the Director of Pharmacy and/or the Senior Pharmacist(s), the Pharmacist is responsible for the daily organization and supervision of all drug distribution activities in the hospital, including involvement in clinical endeavors. Reviews and validates medication orders for safe medication use. Acts as a drug information resource to patients, medical staff, nursing staff, and ancillary department personnel. Compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing related duties.

Current Pharmacist licensure in the State of California (required)
One (1) year of Pharmacy experience (if possible institutional) or the completion of a residency in institutional pharmacy practice or a Pharm D degree (required).
Ability to communicate effectively in English, both verbally and in writing (required).
Basic computer knowledge (preferred).
Automated pharmacy systems: electronic order entry, barcoding, automated medication dispensing point of care medication administration, barcoding, electronic documentation (preferred).
Aseptic technique and use of the IV Laminar Flow Hood (preferred).
