THE POSITION:
The Decision Support Manager performs and oversees the development of modeling, reporting, research, and analysis on internal financial and operational data, along with external market analysis using various tools for the purpose of providing operational insights and strategic planning. Provides project management and leadership in the implementation of key departmental initiatives. Responsible for effective maintenance of future Decision Support systems and data through regular audit driven oversight and documentation of processes. To be effective in this role the Manager, Decision Support will be highly motivated and possess advanced critical thinking skills, highly developed analytical ability, and be an effective communicator with all levels of the organization.

DUTIES AND RESPONSIBILITIES:
Develop annual budget, review and analyze monthly budget variance and trends, identify opportunities to realign departments to budget
Obtain, validate and analyze data for resource, expenditure, capital, strategic planning requests, and present analysis results
Maintain position control, review cost center labor metrics, and assist cost centers with maintaining resources within metrics

Oversee IBNR, RDT, Part D, and regulatory report preparation and submissions.
Maintaining an understanding of PACE Hierarchical Condition Category (HCC) Coding as well as paying careful attention to the PACE HCC reimbursement model.
Responsible for overseeing the Encounter data analyst's submission as well as auditing all monthly and ad-hoc outbound encounters/HCC reporting. Also responsible for analyzing along with the Encounter Data Analyst the inbound reports.
Establish and maintain strong relationships with Encounter Clearing Houses, external auditors/vendors, and internal stakeholders to fix, prevent, and to obtain extra support in resolving encounter and HCC coding errors.
QUALIFICATIONS:

The Custodian/Security Guard is responsible for on-call janitorial and maintenance services of the clinic, i.e., the building, offices and clinical spaces as well as overseeing traffic through the main entrance of facility, providing information and assistance to patients, and maintaining the safety of employees, members and the facility. The Custodian/Security Guard will open, close, and secure the building, daily as scheduled. DUTIES AND RESPONSIBILITIES Custodian
Work with supervisor on setting priorities regarding cleaning, maintenance duties, and special projects.
Proactively greet and assist all persons who enter the clinic in a friendly, helpful and professional manner, and with the highest degree of customer service. Provide assistance to patients and clients, when needed, by directing them to the appropriate department and/or staff person, helping to ensure they arrive at their destination in a timely manner. Provide support in maintaining the safety and security of staff and members.
Mop floors and disinfect bathrooms daily. Ensure all paper products and soap containers are stocked. Wipe down walls, toilets, sinks, bathroom stalls and doors with disinfectant at least daily.
Empty all trash cans, recycle and compost receptacles daily. Wipe outside of trash can and clean weekly with disinfectant. Supporting the clinic's recycling and composting program.
Clean glass windows weekly.
Empty infectious waste stored in biohazard waste containers, then place in the infectious waste container in biohazard closet for pick-up weekly and as needed. Assist biohazard disposal vendor with access to biohazard waste pick-up on a weekly basis.
Sweep and mop building floors and vacuum carpets daily and as needed.
Take out and bring in garbage containers from the street to inside the building as scheduled with waste management vendor.
Clean and wipe walls, door knobs and doors with disinfectants weekly and as needed. Document activity by signing log sheets as needed.
Move furniture as needed. Set up tables/chairs for meetings/special events, as needed.
Assist in ordering and monitoring of janitorial supplies. Maintain safety data sheet (SDS) binder.
Make sure all visitors, patients and staff, have vacated premises prior to closing the building.
Assist in emergency preparedness, including serving a role in the evacuation of the clinic when necessary.
Responsible for reporting and addressing any building damage, repair or maintenance issues throughout the clinic. Notify the Facility Supervisor of any building and/or safety issues that arise in a timely manner. Notify the Facility Supervisor, as well as the Executive Director, immediately of any urgent, potentially harmful or life-threatening issues or problems.
Monitor and direct all repair and service workers while providing services within the clinic. Act as delivery contact person for all custodial and building maintenance supplies.

Assist in de-escalating upset people and if necessary, escort them out of the premises. Document all incidents on appropriate forms (Unusual Occurrence) and submit to supervisor.
Assist members by opening doors for them, providing information on clinic services, and other general customer service duties.
Escort members and staff to their personal car as necessary.
Ensure the parking garage has no violators periodically throughout the day. Report any violations to the Facility Supervisor and the Executive Director. Deliver parking violations to violators. Make sure all visitors, patients and staff, have vacated premises prior to closing the building.
Secure the main entrance to the clinic. Responsible for locking and unlocking main doors, as well as arming and disarming the site alarm.
Assist in emergency preparedness, including serving a role in the evacuation of the clinic when necessary. During walkthroughs check site safety equipment including fire extinguishers, striker chairs and other emergency equipment to ensure they are in working order.
Work in tandem with emergency personnel to address any and all emergency situations. Clear pathways, maintain space, open doors, direct them to site, and provide assistance as needed.
Work in tandem with the Safety Committee to stay up to date on safety and emergency preparedness principles. Assist NAHC staff and members during an emergency. Maintain order, direct staff and members to safety location and ensure building is evacuated.
Responsible for reporting and addressing any building damage, repair or maintenance issues throughout the clinic. Report any safety concerns immediately to your supervisor and take action to prevent.
Ensure that clinic bulletin boards and flyers are up to date at all times.

The Dental Assistant (DA) is a key member of the Native American Health Center's four-handed dental process, and will increase dentists' efficiency by providing needed support. The DA will be travelling to all locations as scheduled for fill in coverage.
A lab coat will be provided by the Dental Department. DUTIES AND RESPONSIBILITIES
Perform all duties with excellence and in accordance with the Dental Practice Act of California.
Provide support to dentists. This includes following all instructions from the assigned dentist at chair-side.
Greet all patients in a courteous, professional, and timely manner.
Ability to work in the General and Pediatric Dental Clinics.
Explain plaque control to all patients in a manner that ensures their understanding.
In collaboration with the dentists, provide oral hygiene instructions to patients and parents/caregivers.
Apply topical anesthetic as requested by the assigned dentist.
Check patients; vital signs (i.e., blood pressure, temperature, etc.) as necessary and/or requested by the assigned dentist.
Prepare operatory for patient reception, according to departmental policy. Ensure operatory is as aseptic as possible for patient welfare.
Ensure pre-set trays are well maintained, and always ready to be used for procedures.
Proactively review chart notes for the planned treatment, in order to set up trays and operatory appropriately.
Ensure all materials and instruments needed for providing treatment are ready and available for use.
Prepare anesthesia for transfer to the assigned dentist.
Retract oral tissues in order to increase visual access into the oral cavity.
Remove excess moisture from the mouth by using hi-vac systems and/or air.
Properly transfer instruments to dentists while maintaining extremely high attention to detail and adhering to sterilization standards.
Prepare and properly deliver any required restorative materials to dentists. Obtain Informed Consent prior to dental treatment, and deliver post-operative instructions with guidance from assigned dentist.
Assist clinical staff in optimizing patient flow. This will include restocking items, seating patients, ‘floating', and supporting other dental team members with patient care.
Keep work area well organized, stocked, and clean at all times.
Keep uninvolved people out of the operatories at all times.
Take impressions for diagnostic and opposing study models.
Pour impressions with proper stone, package them for transport to dental laboratory, and forward to the receptionist.
Operate digital dental radiographic equipment for the purpose of oral radiography.
Proactively inform the Dental Lead RDA of any supplies that need to be replenished, so a consistent level of supplies is maintained at all times.
Assist the assigned dentist with referrals, prescription phone-ins, and other processes involved in providing high quality dental care.
DA, as deemed necessary and requested by Lead RDA or supervisors, will perform Sterilization duties in both the Dental and Pediatric Dental Clinics by performing sterilization and maintenance procedure according to departmental policy.

Safely operates company vehicles to transport participants to and from their homes, CEI sites, off-site medical appointments, and/or activity outings.
Provides physical assistance and monitoring to participants from the pickup or drop-off location to the vehicle and as they get on and off the van, lifting as necessary. May occasionally be instructed to go into participants' homes.
Reports any special situation in participants' homes, or while on the road, to the Road Supervisor including changes in participants' behavior or appearance that may indicate an alteration in medical condition.
Provides other transportation as directed, including medication or supply delivery.
Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

POSITION SUMMARY/SCOPE:
Family Medicine Physicians at Santa Cruz Community Health (SCCH) provide comprehensive family medicine services, including preventive care, chronic disease management, pediatric care, reproductive health services, mental health and substance use treatment, in-office procedures, and same-day/acute care services, to a Santa Cruz County patient population rich in socioeconomic, gender, racial, and ethnic diversity. SCCH is committed to shaping a medical staff that reflects its patient population; bilingual English/Spanish is strongly preferred.
SCCH physicians work alongside other medical providers, including nurse practitioners, physician assistants, registered nurses, medical assistants, mental health providers, chiropractors, Chinese medicine providers, and case managers in an integrated Care Team model. Physicians are clinical leaders of their Care Teams and act as supervising physicians for advanced practice providers. Physicians meet regularly with their supervising Site Medical Director and the larger medical department to further their clinical knowledge and skill, discuss patient cases, review performance dashboards, and improve quality and clinical processes.
The new Morehouse School of Medicine Dominical Hospital Family Medicine Residency Program started in Santa Cruz in July 2024, and SCCH serves as the residents' continuity clinic, procedure clinic, and osteopathic manipulative treatment (OMT) clinic. SCCH family physicians serve in the role of clinical faculty for the residency program as they teach and precept residents in continuity clinic 1-4 shifts per week. As clinical faculty, they meet monthly with the precepting team to participate in shared learning and support, can seek a faculty appointment with Morehouse School of Medicine, may mentor resident physicians with longitudinal Quality Improvement Projects, and may teach didactics. The remainder of their time is spent seeing their own patients as a primary care provider.
SCCH family medicine physicians may also teach students, and they make themselves available to respond to clinical questions from other care team providers including Advanced Care Providers (APCs) and Registered Nurses (RNs). As supervising physicians on their care teams, SCCH family physicians provide consultation visits to other care team providers' patients when requested and assume care of APCs' patients or transfer of care to APCs depending on patient complexity, provider scope of practice, level of education, and experience. They also champion efforts to improve clinical quality measures of the care team's panel, as directed by the Quality Improvement (QI team) and participate in other care team providers' clinical supervision and development including annual performance evaluation and quarterly peer review.
Most medical providers conduct some patient care remotely, usually from home, via telephone or video. 80% of an SCCH physician's time is spent providing direct patient care, while 20% of their time is allotted for clinical “administrative” time. All medical visits are 20 minutes in length, and physicians are expected to see nine patients per 4-hour shift unless precepting resident physicians. SCCH uses OCHIN Epic Electronic Health Record system for visit documentation, e-prescribing, and data collection for reporting on clinical quality.

Clinical Responsibilities
1. Provide primary care to members, including histories, exams, lab tests, prescriptions and referrals within prescribed up to date guidelines and protocols.
