Our multi-disciplined treatment team works with individuals ages 15-26 who experience mental illness, serious emotional difficulties and first episode psychosis. The Young Adult Community Case Manager provides care planning, resource identification, benefit coordination, service monitoring, and community-based counseling support to assist clients with improving symptoms and behaviors that otherwise interfere with age-appropriate development, functioning and wellness goals. This is a full time, 35 or 40 hour per week position and is considered a non-exempt, in accordance with the Fair Labor Standards Act (FLSA). Major Functional Areas and Description of Duties Provide individual assessment to create care plans that specify presenting problems, objectives, interventions, and desired outcomes. Assist clients with applying for and maintaining adequate medical coverage to support consistent access to whole health care and treatment. Provide outreach, advocacy, coordination, resource identification, linkage and monitoring of internal and external supports and services. Provide counseling and Functional Support Services (FSS) in the home, school or other community-based setting as deemed appropriate for the objectives, interventions and desired outcomes identified in the client's treatment plan. Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement. Encourage engagement in and provide Family Training and Counseling (FTC) services to educate and promote how to best support the client's resiliency and recovery toward treatment goals. Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community. Provide timely documentation on each service delivered. Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around family systems approach is provided to the client. Provide set case management contacts per month and set hours of functional support services per week (eligible for incentive pay upon exceeding productivity expectation). Serve on appropriate committees, both internal and external, as determined by supervisor. Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients. Execute all required services and documentation in accordance with agency policies and federal/state standards. Maintain professional memberships, licenses, accreditation, certifications, etc. Represent CLM in a professional and appropriate manner in all settings. Perform all other duties as assigned by supervisor. Physical Demands and Working Conditions Half of the work will be visiting clients in the community and also in their homes. The other half will be office based. The ability to actively engage with children and conduct interactive play within a therapeutic setting is required. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required. We are seeking a dynamic and experienced Medical Director to join our team! The Medical Director is responsible for the operation of psychiatry services at Concord Hospital, the Emergency Department, and the 5 West Behavioral Health Unit. This oversight includes the provision of clinical care, staff supervision, fiscal accountability, risk management, and participation in short- and long-range service planning to meet the needs of the community. This position serves in both an administrative and clinical capacity, providing direct services to patients in the Behavioral Health sectors of Concord Hospital. The Director of Clinical Operations for Behavioral Health also shares a dyad leadership role for the Psychiatric Emergency Services team with the Program Director of Psychiatric Emergency Services. Additionally, this position works closely with the Nurse Manager of Behavioral Health Services at Concord Hospital. Salary: $260,000 - $290,000 Compensation is dependent on experience and credentials Essential Duties and Responsibilities: Provides direct services to Behavioral Health patients of CH Shares responsibility for providing services at 5 West, Emergency Department and for providing inpatient consultations. Provides emergency psychiatric evaluations to patients and coordinates with Emergency Services clinicians and care managers for ongoing disposition planning and safety recommendations. Provides acute psychopharmacological management. Orders, reviews, and interprets appropriate laboratory studies and medical tests essential to diagnoses and follow-up care. Baseline expectation is 16 weeks of call per year, though this number may vary upwards or downwards depending on the needs of the unit as determined by the CMO. The director of Clinical Operations works alongside a colleague, usually an APRN, during weekend call. Submits clinical documentation in a timely manner consistent with Concord Hospital and Riverbend legal and QA requirements. At request of medical/surgical providers or CMO, the Director of Clinical Operations shall participate in assessment and treatment of patients requiring Behavioral Health consultation throughout the medical/surgical units within the hospital. Leadership responsibilities to the Psychiatry team at CH Develop a positive atmosphere for the sake of excellence of patient care and successful work of the patient care team. Facilitate bidirectional communication between professional staff and medical leadership and administration. Promote physician engagement through physician rounding and other activities. As an expert in community resources, Peer Specialist can link the appropriate mental health resources that best fit the presenting problems of the consumer as well as their level of motivation and stage of change. Sometimes this linkage will involve transporting the consumer as deemed clinically appropriate by the Mobile Crisis Team Leader or Emergency Services Director to the community resource. 5. Peer Specialist can keep a connection with the consumer in the community for a period after the crisis to ensure follow up with agreed upon referral sources. This not only includes follow up to determine link to resources but to continue to meet with and educate the consumer about additional mental health resources beyond the amelioration of the crisis. 6. Peer Specialist will actively participate with the Mobile Team Leader and team members to establish treatment goals, parameter and services which contribute to the team increasing positive consumer outcomes in the community. 7. Peer Specialist must be able to make an accurate needs assessment, and document these needs and goals onto a contact note in alignment with the current consumer crisis.
