JOB DESCRIPTION:
SUMMARY OF POSITION
Provide clinical services consistent with established guidelines for best practice interventions, including evaluation, individual, group, family, or couple's psychotherapy for adults, adolescents and children seen in one's service area.
ESSENTIAL DUTIES & RESPONSIBILITIES
Maintain necessary communications and reporting with referral sources and other involved parties as approved by client.
Maintain effective working relationships with the clinical team
Maintain timely/accurate clinical records in accordance with organization policies.
Maintain effective relationships with Community referral sources.
Participate in the development and implementation of quality clinical practices.
Complete all assessments, including ANSA/CANS assessments and reassessments on time.
Complete clinical documentation within 24 hours of the service provided.
Complete treatment plans and other clinical service authorizations in a timely manner.

Effectively communicate via written, verbal, in person and virtual methods.
Provide excellent customer service to individuals seeking our services.
Utilization and knowledge of evidenced based interventions.
QUALIFICATIONS
Education Level
Master's degree in social work, psychology, or related field is required.
Obtain and maintain CANS/ANSA Certification
Years of Experience
Three to five years' experience in behavioral health environment is preferred. Experience in evidence-based and best practices interventions is preferred.

JOB DESCRIPTION:
SUMMARY OF POSITION
Provides person-centered assistance to clients to help ensure appropriate continuum of care. Updates the care plan and monitors the effectiveness of client treatment. Coordinates treatment plan in support of increased self-sufficiency. Primary case holder for the client and is responsible for maintaining the chart to ensure the client has access to services provided by this program. Completes necessary documentation in a timely manner and support clients as part of their clinical team. Teaches, guides, instructs and supports persons with serious mental illness and/or substance use disorders to address barriers in any life domain, associated with their recovery.
ESSENTIAL DUTIES & RESPONSIBILITIES
Facilitates client's use of family, social and community resources, acting as an advocate when necessary and maintaining necessary communication with other involved parties.
Assesses and updates person-centered plan with the client and collaterals and monitors progress of the plan. Ensures client's clinical record is maintained and up to date at all times. Clinical documentation is accurate, meets organizations standards in composition and content, and is adequate for communication about client's treatment and progress. Main point of contact for managed care team
Assists in census management, admissions, and discharges. Communicates with staff and clients to help overall flow of facility.
Completes authorizations paperwork for clients including ASAMs, Care Plans, CANS/ANSA's and all documentation/assessments necessary for treatment specific to the program.
Meets with clients individually to explain insurance status and needs.
Serves as the liaison between the client and the client's Representative Payee
Uses structured, standardized interventions and resources for teaching to assist clients in overcoming barriers to recovery.
Provides individual and/or group interventions in the home, community or office setting.
Works within various community systems to collaborate on behalf of, and with, the client. Includes the criminal justice system, the child welfare system, hospital/medical systems, educational, etc.
Submits reports to referral sources on client's participation and progress in treatment, where authorized
Understands how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model.
Performs all required job duties and responsibilities in accordance with CCBHC Standards.

SUMMARY OF POSITION
Assists clients with daily living activities and tracks, monitors and documents behaviors and interactions. Plans therapeutic recreation activities for groups, steers department philosophy toward purposeful interventions and therapies, works with other clinical staff on special projects and duties. Leads individual and group activities with clients.

Observes and documents client compliance with medication regimes as dictated in the treatment plan.

SUMMARY OF POSITION
The Community Health Worker (CHW) is responsible for improving the health and well-being of individuals and families within the community by bridging gaps between healthcare providers and community members.
