JOB DESCRIPTION:
SUMMARY OF POSITION
Provides psychiatric services consistent with established guidelines for best practice interventions with adults, children or adolescents. Systematically assesses patient's health status, arrives at appropriate diagnosis(es), and implements care plans for effective clinical outcomes.
ESSENTIAL DUTIES & RESPONSIBILITIES
Clinical Supervisory Responsibilities:
Maintains collaborative practice or Delegation agreements with Advance Practice Practitioners (Nurse Practitioners/Physician Assistants)
Quality Improvement:
Perform comprehensive psychiatric evaluations and provide medication management.
Considers thorough range of treatment/preventive options and arrives at a recommended course of action that is age appropriate, culturally appropriate, realistic, and attainable.
Participate in treatment team meetings for medical supervision, utilizing advanced knowledge of psychopathology and psychopharmacology to assist clinical team members in diagnosis, case formulation, and choice of treatment interventions.
Participate in on-call rotation for daytime and nighttime emergency service coverage (with additional compensation).
Supervise collaborative practice agreements with nurse practitioners.
Appropriately manages patient care and health status following initiation of treatment/preventive plan, and monitors and adjusts as warranted.
Communicates effectively with patients and family so that they understand their diagnosis, treatment plans and need for follow-up care.
Educates and encourages patients to become an active participant in their own health care related behavior and provides patient education materials of use in encouraging wellness.
Seeks consultation and advice by providing adequate information to consulting physicians and communicates request effectively or responds to such requests from the referral source in a courteous, professional, and timely manner.
Documents and completes patient charts to facilitate ongoing quality of care in a timely manner (within one business day of the service provided).
Performs appropriate medical procedures within scope of practice as defined by the Centerstone Health Services Credentialing and Privileging processes.
Delegates directly and indirectly patient care activities to appropriate personnel. Responds appropriately to emergent situations.
Maintains and improves skill level through participation in continuing medical education.
Actively participates in clinical quality improvement initiatives. Takes actions to assure continuity of care for patients.

The Steven A. Cohen Military Family Clinic offers a variety of services to post 9/11 Veterans, Active-Duty Service Members, Reservists, National Guard and their family members. The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Essential Duties & Responsibilities:
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community, and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports, and include their input in the person-centered planning process.
Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Monitor and document the status of the consumer's progress.
Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
Qualifications
Education & Experience:
Minimum of a Bachelor's Degree in human services or related field is required.
Two years' experience working with youth or families.
Current Family Centered Treatment Certification is preferred.

The Intensive Outpatient IOP Facilitator will perform assessments, and facilitate IOP group sessions, as well as individual, family and weekly programs as assigned using industry recognized therapeutic modalities and interventions and complete all required paperwork per timelines established to ensure successful billing of services.
Perform intake screening and assessments unless otherwise assigned to a central intake staff.
Perform and/or monitor all duties associated with program administrative functions including but not limited to psychosocial assessments and admission documentation, contacts with all referral sources and family, urine drug screens, and ensure that treatment has been authorized with the payer source both initial and concurrent.
Provide psychoeducational programming to include didactic, audio visual, and other formats of teaching that are appropriate for this level of care.
Perform the clinical group process portion of the program using industry recognized therapeutic modalities and interventions.
Perform individual and family sessions as needed.
Perform the weekly family education and weekly programs.
Maintain the expected census in each IOP group sessions.
Present appropriate cases at the weekly treatment team and participate in clinical supervision as scheduled and/or necessary.
Develop a system for tracking current census, scheduling and performance of assessments of potential admissions.
Maintain current CPR certification, yearly TB testing, and annual TB Screening Certification as required by the state.
Assist the Program Director (PD) and/or IOP Coordinator as needed with business development and marketing functions.
If professionally licensed:
As a licensed clinician working for Health Connect America your NPI number will be used to bill for services performed by those being supervised by you.
As a licensed clinician you will be responsible for ensuring that your credentials remain current, you comply with all training requirements, and you are adequately supervising your staff.

JOB SUMMARY: To perform a combination of behavioral health clinical tasks including and not limited to intake assessment on referrals for mental health services and/or provide counsel with emphasis on prevention and recovery as assigned on weekly rotation schedule by Program Manager. This position will help individuals deal with complex and comorbid diagnosis which may include: serious persistent mental health illness, addictions and substance abuse; family, parenting, marital problems and challenges with social determinants of health; suicide; stress management; problems with self-esteem; and issues associated with aging, physical, mental and emotional health.
RESPONSIBILITIES:
Perform face-to-face intake for consumers who have been referred to MHC and assess for care management, medication management or therapy eligibility.
Complete all intake evaluation paperwork: initial intake, and grief when needed and any other necessary paperwork.
Determine an individual's appropriateness for care management, medication management or therapy and assign to all that are appropriate.
Disposition consumer to appropriate Agency for other mental health services when ineligible for care management.
Identify individuals that are in need of MCRT services rather than a routine intake. If appropriate, facilitate crisis action.
Assist in resolving any grievance received from a consumer regarding referral.
Fill in for Admissions Coordinator as indicated by supervisor.
Utilize Intake Committee for all consumers who meet criteria who have a history of violence, sexual crimes or serious criminal activity. Write completed intake and email to Intake Committee for review. Contact consumer with decision.
Utilize evidence based therapeutic framework, counsel consumers and patients, individually and group sessions, to assist in overcoming dependencies, adjusting to life and making changes.
Guide consumers in the development of skills and strategies for dealing with their problems using evidence-based modalities.
Encourage consumers to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships.
Develop and implement treatment plans based on clinical experience and knowledge.
Modify treatment activities and approaches as needed in order to comply with changes in consumer's status.
Learn about new developments in their field by reading professional literature, attending courses and seminars, establishing and maintaining contact with other social service agencies.
Counsel family members to assist them in understanding, dealing with and supporting consumers.
Evaluate the effectiveness of counseling programs and consumers' progress in resolving identified problems and moving towards defined objectives.
Refer patients, consumers or family members to community resources or to specialists as necessary.

What you will do

The CPRS Peer Leader is a key position within our mental health peer support program, responsible for coordinating, supervising, and leading a team of Certified Peer Recovery Specialists (CPRS). Participant Support: Oversee the provision of peer support services to participants, ensuring they receive the help and support they need. This could involve leading group sessions, providing one-on-one support, and liaising with other mental health services.

REQUIREMENTS:

JOB DESCRIPTION:
SUMMARY OF POSITION
Centerstone is seeking therapist with experience treating individuals with mental health and substance use disorders. This is an opportunity to work with a multi-disciplinary team. No weekend or on call work required.
ESSENTIAL DUTIES & RESPONSIBILITIES
Develop therapeutic rapport with clients - Meet with clients as scheduled to aid in development of therapeutic relationship.
Diagnostic Assessment - Formulate a provisional diagnostic assessment based on information presented, identify need for risk assessment (to include substance use assessments).
Concurrent Documentation - Completes documentation within 24 hours.
Provide assessment services, individual (and possibly family) therapy to those identified with substance use issues.
Treatment Planning - Developing, writing, implementing, evaluating and revising overall treatment goals/objectives and plans per ongoing assessment of needs and input from the individual based on their identified strengths.
Therapeutic Interventions - Use of empirically based modalities to engage and manage their mental health or substance abuse diagnosis.
Progress notes completed within current agency guidelines.
Responds to supervisory request for urgent care situations and coverage for vacant caseloads. This includes telephones and face to face intervention.
Keeps administrative supervisor informed of all potential issues that could impact the program or the organization.
KNOWLEDGE, SKILLS & ABILITIES
Effectively communicate via written, verbal, in person and virtual methods.
Proficient in Microsoft Office products
Coordination, ability to organize, and plan are critical for this role
Responds to needs of consumers in a timely and professional manner.

QUALIFICATIONS
Education Requirements
A Master's degree is required in one of the following fields: Clinical Psychology, Mental Health (MH) Counseling, Social Work, Marriage and Family Therapy, and/or a specialization in MH Counseling
License-eligible in respective discipline preferred.
OR
Applicants with a Master's degree in a related field (outside of the disciplines listed above) must meet the following criteria:
Completion of at least 24 graduate-level semester hours in psychology, social work, counseling, or other behavioral health-related coursework.
Relevant coursework and work experience in the following areas:
Clinical treatment and treatment planning
Appraisal and assessment of mental disorders
Psychopathology
Use of the DSM for diagnostic purposes within a clinical internship or practicum in a behavioral health setting
Experience Requirements
A minimum of one (1) year of experience providing treatment to individuals with mental health disorders is required, and substance use disorders experience preferred.
Internship experience may be applied toward the minimum experience requirement for recent graduates.
Certification/Licensure
Must possess a valid driver's license and meet eligibility requirements to be covered under the organization's vehicle insurance policy.

Job Title: Behavioral Health Clinician (LPC, LMFT, or LCSW)

JOB SUMMARY: To perform a combination of behavioral health clinical tasks including and not limited to intake assessment on referrals for mental health services and/or provide counsel with emphasis on prevention and recovery as assigned on weekly rotation schedule by Program Manager. Work with individuals and groups to promote optimum mental health. This position will help individuals deal with complex and comorbid diagnosis which may include: serious persistent mental health illness, addictions and substance abuse; family, parenting, marital problems and challenges with social determinants of health; suicide; stress management; problems with self-esteem; and issues associated with aging, physical, mental and emotional health.
RESPONSIBILITIES:
Perform face-to-face intake for consumers who have been referred to MHC and assess for care management, medication management or therapy eligibility.
Complete all intake evaluation paperwork: initial intake, and grief when needed and any other necessary paperwork.
Determine an individual's appropriateness for care management, medication management or therapy and assign to all that are appropriate.
Disposition consumer to appropriate Agency for other mental health services when ineligible for care management.
Identify individuals that are in need of MCRT services rather than a routine intake. If appropriate, facilitate crisis action.
Assist in resolving any grievance received from a consumer regarding referral.
Fill in for Admissions Coordinator as indicated by supervisor.
Utilize Intake Committee for all consumers who meet criteria who have a history of violence, sexual crimes or serious criminal activity. Write completed intake and email to Intake Committee for review. Contact consumer with decision.
Utilize evidence based therapeutic framework, counsel consumers and patients, individually and group sessions, to assist in overcoming dependencies, adjusting to life and making changes.
Guide consumers in the development of skills and strategies for dealing with their problems using evidence-based modalities.
Encourage consumers to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships.
Develop and implement treatment plans based on clinical experience and knowledge.
Modify treatment activities and approaches as needed in order to comply with changes in consumer's status.
Learn about new developments in their field by reading professional literature, attending courses and seminars, establishing and maintaining contact with other social service agencies.
Counsel family members to assist them in understanding, dealing with and supporting consumers.
Evaluate the effectiveness of counseling programs and consumers' progress in resolving identified problems and moving towards defined objectives.
Refer patients, consumers or family members to community resources or to specialists as necessary.
Meet with families, clinic providers, care managers and other interested parties in order to exchange necessary information during the treatment process.
Collaborate with other staff members, primary care practice partners including pediatricians, doctors, nurse practitioners, office staff, and referral sources as assigned to perform clinical assessments and develop treatment plans.
