Come talk to us about joining the Centerstone team! JOB DESCRIPTION: SUMMARY OF POSITION Provides nursing care to clients in crisis in the Walk-In Center and Crisis Stabilization Unit. Provides medication monitoring, adjusts medications within limits prescribed by authorized prescriber. Provides information, education, and support to individuals in crisis that promote wellness and recovery. This is a night position (7p-7a) ESSENTIAL DUTIES & RESPONSIBILITIES Performs nursing assessments on individuals being admitted to the WIC/CSU. Monitors vitals, performs lab testing, and evaluates medical appropriateness for admission and continued stay. Obtain and enters orders into EMR from authorized prescriber. Administers and gives medications to patients, monitors the dosage, side effects, use and documents patients' reactions. Provides medication training to the guest or guest's family/guardian to administer the guest's medication, to monitor proper levels of dosage and watch for side effects. Develops and maintains the Treatment Plan of Care for guests. Oversees the administering of nursing needs of guests. Performs as a liaison role between guest and the authorized prescriber, pharmacist, family, and/or collateral support systems. Coordinates guest- staff assignment of care and provides shift change report. Observes guests on a day-to-day basis and provides documentation in the EMR. Provides group or individual education as needed. Participates and/or leads staff meetings and training as scheduled or necessary. KNOWLEDGE, SKILLS & ABILITIES Effectively communicate via written, verbal, in person and virtual methods. Performs basic electronic medical record computer skills. Detail oriented with effective problem solving skills Possesses leadership skills Knowledge base in psychotropic medications. Skill in crisis intervention. Ability to de-escalate patient/clients using evidence based de-escalation training (e.g. Theapeutic Options) to include safety holds as necessary. QUALIFICATIONS Education Level Minimum of Associates Nursing Degree from an accredited school or a Waiver approved LPN by State of TN. RN Preferred. Years of Experience One year of behavioral health nursing experience preferred Certification/Licensure Licensure required. Valid driver's license required. Administers and monitors medications as ordered. Monitors medication effectiveness, side effects and compliance. May perform assessments for any side effects/EPS. Monitors clinic medication supply and maintains necessary documentation. Provides phone triage and addresses medication issues with patients when necessary. Provides proper education and guidance as necessary. Assists patients with accessing medications including obtaining prior authorizations and accessing prescription assistance programs. Provides healthcare education to patients and/or families or staff as assigned. Provides CLIA Waived laboratory testing as ordered. Maintains CLIA waived laboratory environment including Quality Control testing and documentation. Monitors received laboratory values and reports abnormal values as required. Phlebotomy may be required. Provides employee health services as directed, including annual influenza vaccination clinics and staff TB skin testing. Works in collaboration with other members of the treatment team and in collaboration with external health care providers as necessary. Nurses may provide direct physical supervision to Certified Medical Assistants and delegate tasks in accordance with the Tennessee Nurse Practice Act and other applicable regulations.     Job Details Plateau Mental Health Center - Cookeville, TN High School $26.00 HourlyDescription Volunteer Behavioral Health is now seeking for LPN Part Time for Night Shift in the Cookeville, Tennessee area. EXEMPLARY DUTIES / RESPONSIBILITIES: · Implement policies and procedures of the Crisis Stabilization Unit. · Assist Psychiatric Technicians when needed in providing for the daily living activities of the patients. · Implement procedures for providing the medical needs ordered for each patient while in CSU. · Screen calls and questions pertaining to medication. · Arrange educational sessions for patients pertaining to medication. · Document all patients having had a handout sheet and education session pertaining to their medications. Also, patients' family when indicated. · Monitor lab work within the guidelines pertaining to the specific medication. · Help maintain and support patients receiving medications by individual evaluation and/or group. · Complete Admissions Assessments and create Treatment Plans. · Phone or dispense prescriptions (signed by MD/NP) in consultation with the Psychiatrist/Nurse Practitioner. · Participate in staffing. · Other duties as assigned. Qualifications Licensed as an LPN in the State of Tennessee and experience working in the mental health field.
