Schedule: Monday - Friday 8 am to 4:30 pm Location: 309 SW 4th Ave. Portland, OR 97204 Salary: $53,914 - $75K SUMMARY: The Community Manager's primary role is to ensure that the properties meet all ownership goals and objectives, to enforce Central City Concern's policies and procedures, and to adhere to Fair Housing and Landlord Tenant Laws at all times. A Community Manager is responsible for supervision of other property staff and vendors to provide outstanding customer service to the residents, maintain excellent curb appeal, provide well-timed and proactive property maintenance, safeguard the good financial health and stability of the property, confirm timely compliance with all government and property program requirements, and complete all required paperwork and reports on time ESSENTIAL DUTIES AND RESPONSIBILITIES: * Represent CCC in a positive and professional manner at all times. * Act as the company's primary coordinator to assure that the company's efforts fully meet and exceed property management obligations. * Manage all property operations in compliance with all local, state, and federal laws (Fair Housing, Landlord Tenant Laws, OSHA, etc.). * Under the direction of the Portfolio Operations Manager, assist in the development of operating income/expense budgets and capital budgets that reflect the owner's objectives for operating the property, cash flow requirements and leasing strategy. * Analyze and report monthly financial statements including operating variances from budget, cash management and strategies for collection of receivables in a timely manner. * Administer budgets, review and analyze operations, and recommend corrective actions needed to maximize efficiency, owner and resident satisfaction, and profitability to Portfolio Manager. * Ensure that the property operates in compliance with the affordable program requirements applicable to the property. * Complete resident income certifications and recertification in a timely and accurate manner to maintain compliance with program funding and regulatory agency requirements. * Coordinate collection and documentation of all revenues following lease obligations of tenants and CCC's policies on accounts receivable. * Follow confidentiality guidelines for all resident, property, owner, and CCC information at all times. * Direct the day-to-day activities of other property staff. Develop, hire, discharge, evaluate performance, coach and discipline staff with direction from Portfolio Operations Manager and Corporate HR Department. * Accurately record time worked; adhere to time keeping guidelines including approving site staff time records on a daily basis. * Administer leasing efforts, including reviewing tenant billings, analyzing lease clauses, and preparing vacancy reports, enforcing tenant compliance, and performing landlord obligations. * Identify leasing prospects and occasionally respond to routine leasing inquiries. * Collect delinquent accounts according to legal guidelines and communicate with accounting department to ensure accuracy of rent roll and accounts receivable. * Work with Portfolio Manager to identify, engage and supervise property vendors. * Facilitate regular meetings between Resident Services, community partners, and building staff * Establish and maintain collaborative working relationships between departments and with coworkers. Prepare and lead all site staff meetings, including monthly safety meetings, and document meeting minutes to submit to the corporate office. * Constantly strive for improvements in work process and results to better meet resident needs, ownership goals and objectives, and company standards and expectations. * Function as part of a team providing supportive housing and report to various job sites as directed by supervisor. * Ability to work as a team member and to establish priorities; ability to display a non-judgmental attitude, and willingness to learn new skills. * Adhere to all state and federal privacy and security regulations applicable to the program, and to CCC policies and agreements regarding confidentiality, privacy, and security. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. * Perform other duties as assigned by the Portfolio Operations Manager. SKILLS AND ABILITIES: * Ability to independently take a leadership role, plan, and direct the work of others. Ability to use good judgment in completing and delegating the work required to meet overall objectives within specified time and budget. * Sufficient organizational skills to develop and maintain files and process reports. * Ability to express thoughts, perceptions, and ideas clearly and concisely, orally and in writing. * Ability to effectively communicate and interact with a wide variety of people including the general public, merchants, housing authorities, law enforcement personnel, building tenants, program participants and co-workers. * Ability to consistently multi-task and perform in a complex and challenging environment and to meet deadlines with minimal supervision. * Practical experience with Fair Housing regulations, Low-Income Housing Tax Credit programs and various subsidy programs. * Ability to identify and analyze problems and opportunities; establish priorities, and efficiently allocate resources. * Ability to maintain accurate records and efficiently complete required paperwork * Excellent computer skills in word processing, spreadsheet and database applications. MINIMUM QUALIFICATIONS: * High school diploma or equivalent * Three years of experience in property management or housing to include: a. One year of lead or supervisory experience OR b. One year of experience assisting a Community or Property Manager OR c. One year of a combination of the above * Working knowledge of landlord tenant law * Knowledge of financial & project management concepts preferred * Strong mathematical skills and basic understanding of property budgets and financial accounting * Experience in community development and working in multi-cultural organizations and communities is a significant plus * Demonstrated knowledge of mental health diagnosis and chemical dependency desirable. * Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Workplace Ace of 1988. Our residents often are able to stay with their children, who might otherwise be placed in foster care. The OC Patient Navigator fills in as a critical member of a multi-disciplinary team providing intensive case management for patients enrolled in the program, working to support patients through the experience of their RCP stay. This support includes welcoming and orienting new participants, engaging with them on a daily basis, and coordinating resource support needs as well as linkage to medical services to manage their acute needs and ongoing care. The Recuperative Care Program (RCP) is an innovative service that provides immediate access to emergency housing, primary care, and intensive case management support for individuals who have medical support needs while experiencing homelessness. The service partners with Portland area hospitals and clinics, in concert with CCC's patient-centered medical home, to help clients/patients receive the right level of care and support for their medical and social needs. The OC Patient Navigator fills in as a critical member of a multi-disciplinary team providing intensive case management for patients enrolled in the program, working to support patients through the experience of their RCP stay. This support includes welcoming and orienting new participants, engaging with them on a daily basis, and coordinating resource support needs as well as linkage to medical services to manage their acute needs and ongoing care. The OC Patient Navigator is skilled in crisis management and provides timely referral to appropriate substance abuse, physical health, and mental health treatment, support in accessing benefits and entitlements, and support in identifying appropriate and affordable housing as part of a comprehensive approach to ending homelessness and achieving self-sufficiency. The OC Patient Navigator works to provide coverage for the primary point person for managing a caseload of program participants, working to meet the case specific medical, behavioral health, and housing needs of the individuals they serve. The population served includes many people who have been chronically experiencing homelessness, often with co-occurring substance use and mental health support needs in addition to the acute and/or chronic medical conditions for which they are receiving support. ESSENTIAL DUTIES AND RESPONSIBILITIES Fill in for regular Recuperative Care staff as needed based on program capacity, staffing, and task needs. Work under the medical direction of the Recuperative Care Program to coordinate acute and ongoing medical care for current Recuperative Care patients in conjunction with Recuperative Care staff. Serve as primary point of contact for internal and external care coordination and resource linkage for patients on caseload. Assist patients with obtaining prescribed medications and supporting patient in plans to self-administer medications as prescribed. Perform welfare checks and charting on patients enrolled in the Recuperative Care Program. Review hospital charts and internal medical records and obtain releases from patients to gather collateral information. Create patient charts and update electronic medical records in a timely manner with patient interactions, appointments and encounters. Actively participate in inter-disciplinary case collaboration, including regular caseload reviews with RN Complex Care Manager. Within interdisciplinary collaboration, develop, implement and maintain a case plan to as needed for patients upon admission to the program. Support patients in coordinating with their primary care providers, specialty medical providers, and other health service providers when health concerns are raised or observed. Identify, create, and implement behavioral interventions when appropriate, in conjunction with interdisciplinary team and Integrative Care Manager Coordinate medical, behavioral health, services available to patients as care plan develops and as directed through medical support structure. Effectively de-escalate heightened situations with patients experiencing trauma, exacerbated mental health symptoms, and behavioral complexity. Work to identify housing and/or placement options relevant for patients and work to support patients in overcoming barriers to accessing housing or placement. Collaborate with housing specialists and/or other resources as available to support patients in achieving housing goals. Follow Policies and Procedures of Recuperative Care Exercise Trauma Informed Care and Harm Reduction principles in providing care for RCP participants. Respond to emergency medical situations as needed, complete pre-hospital care reports and incident reports when necessary. Provide and/or arrange for transportation for patients to and from program related appointments, interviews, and hospitals/clinics. Screen patient referrals via phone and/or at partner hospitals upon identification of patients referred for Recuperative Care. Work with other Recuperative Care and Central City Concern staff to support patients in coordinating their health care with their primary care provider and/or specialty healthcare services. Attend clinical meetings as structured and provide current information to team regarding caseload. Participate in case reviews with Recuperative Care team (including RN Complex Care Manager, Case Managers, Logistics Specialist, Housing Specialist, Program Assistant and clinical, logistics and operations volunteers) to ensure quality care for patients in Recuperative Care, including helping to identify and address psychological, social and medical needs. Participate in compliance, quality assurance and quality improvement activities as directed. Transport clients when needed (Only if approved CCC driver). Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. Attend all mandatory CCC trainings in a timely manner. Perform other duties as assigned. This position will work as a member of a multi-disciplinary team at the Blackburn Center to deliver comprehensive, person-centered, integrated services to the Portland Eastside community. The Blackburn Medical Assistant (MA) plays a critical role in patient care. MAs work to support providers and patients throughout the patient visit and are also integral members of the patient care team.
