CM Conditioning Clinic- a health and wellness clinic are looking for a Licensed Athletic Trainer. This full-time position is primarily in a collegiate setting working with 10+ sports teams, leading strength and conditioning settings as well as providing traditional athletic training services.
Performs thorough, organized, and objective injury prevention, evaluation, and treatment in appropriate depth for patient prognosis and clinical presentation.
Identifies musculoskeletal problems from the evaluation and incorporates patient potential as well as major barriers to treatment.
Responsible for scheduling other LAT, ATCs for additional coverage for all athletic games, sessions, and sports medicine clinical hours.
Mentoring students who may want to pursue careers in Athletic training.
Qualifications
Required Education and Experience:
B.S. or Master's Degree in athletic training with appropriate Maine Licensure and Certifications as an ATC, LAT
CPR-AED Certification (or we can train)
Certificate or experience in Strength and Conditioning or a strong recommendation such as ACSM, CSCS.

We offer flexible schedules for therapists.
Qualifications
Experience in internal pelvic assessment and treatment (elective, active practice)
Strong Manual orthopedic skills

A current New York State Physical Therapy license

The PhysicalTherapist has daily duties that include responsibilities for assessing patient's physical, psychological, emotional, educational, and spiritual needs, as well as planning and providing physical therapy treatments for patients in accordance with written prescription or referral from a physician, or as appropriate for direct access patients. Other responsibilities include instruction and education of physical therapy and physical therapy assistant students. PhysicalTherapist also provides education to the community. Does related work as required.
Roles and Responsibilities:
Identifying patient's goals and needs. Provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with physicians or by prescription.
Administers manual therapy (joint and soft tissue mobilizations, deep tissue massage); instructing, encouraging, and assisting patients in performing physical activities, such as therapeutic exercises, ambulatory functional activities, daily-living activities, and in using assistive and supportive devices, such as crutches, walkers, and orthotics to help patients accomplish their treatment goals.
Administers physical therapy modalities such as mechanical traction, hot/cold packs, ultrasound, and electrical stimulation for pain control, neuromuscular re-education, and functional improvements;
Evaluates effects of physical therapy treatments and fit of orthotic devices by observing, noting, and evaluating patient's progress; recommending adjustments and modifications.
Completes discharge planning by consulting with the patient, family and care givers, and physicians.
Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient programs.
Documents patient care services and charges by charting in patient's electronic medical record.
Required Skills and Experience:
Education: successful completion of accredited physical therapy program; may be Bachelor's, Master's, or Doctorate level program
Must complete yearly mandatory safety in-services and/or trainings as well as yearly Healthstream e-Learning Courses
Licensure: Current NYS Physical Therapy license
Current CPR Certification

Essential Duties and Responsibilities
Evaluates the degree of function and disability of patients unless contraindicated by his/her professional judgment or a doctor's written order.

Performs physical therapy procedures upon the written order or referral of a licensed physician.
Records on patients charts all physical therapy evaluations, re-evaluations, progress notes, consultation and discharge summaries within the time frames established by policy and procedure. Completes documentation by the end of the day on which the visit was made.
Performs the Comprehensive Assessment (OASIS) at the appropriate time points, and performs aide orientations and biweekly supervisions.

Occupational Therapist - Per Diem - Acute Inpatient Rehabilitation

Position Overview:
The Occupational Therapist (OT) will provide evaluations, therapy interventions, Activity of Daily Living performance assessments, and education with patients in medically compromised situations within their full scope of practice. This will include participation in the development, implementation, monitoring, and evaluation of programs and services for patients, families, and the community. The OT will work collaboratively as a member of the interdisciplinary team and provide safe, efficient and effective care for their patients. The OT is responsible for making high-level clinical decisions, in a rapid and dynamic environment, for the care of their patients.
Roles and Responsibilities:
Clinical Decision-Making: Completes comprehensive chart review to determine OT appropriateness. Chooses appropriate exam & integrates findings into a comprehensive plan of care that is person-centered & goal centered. Uses evidenced based practice to guide decision making & therapy intensity. Assesses response to intervention & adjusts interventions/plan of care appropriately.
Communication: Selects appropriate communication style based on age, learning style, cognition, and culture. Educates patient & family/caregivers using compassionate communication. Clearly communicates patient's OT needs & barriers to plan with interdisciplinary team to facilitate throughput. Initiates and maintains professional communication with every team member at all times including adverse, challenging, and crucial conversations.
Safety: Uses medical record & patient presentation to identify relative & absolute contraindications to participation in OT. Surveys patient and environment for safe movement barriers. Maintains expertise in use of all safe patient handling equipment.
Patient Management: Progresses interventions to facilitate achievement of patient's goals. Reassesses progress towards goals & adjusts plan of care. Produces clear, defensible documentation that supports clinical decision making. Uses clinical judgement to determine when immediate communication beyond documentation is required for safe coordinated patient care.
Discharge Planning: Synthesizes patient's life context and current functional status to determine discharge destination, level of support, additional services, and follow-up needs. Determines appropriate equipment needs and provides caregiver training as appropriate.
Required Skills and Experience:
Education: successful completion of accredited Bachelor's, Master's, or Doctorate level occupational therapy program
Licensure: Current NYS Occupational Therapy License
Current CPR Certification
Must complete yearly mandatory safety trainings and e-learning courses

Location and Shift Information:
* Location: Onsite at Seneca View Skilled Nursing Facility - 220 Steuben St, Montour Falls, NY 14865
* Shift(s): Days/Evenings FLEX. Operational Hours are 7am to 6pm, Monday through Friday. Rotating Weekends and Holidays, with Call.
Pay Range Disclosure:
* $34.00 to $39.00 per diem
