Standard shift: Day shift Night shift Weekly schedule: 3x12 (Full Time) DUTIES AND RESPONSIBILITIES PERFORMS VITAL SIGNS ACCURATELY Takes temperature, pulse, respiration, and blood pressure using appropriate single usepatient equipment. Measures and records fluid and food intake and output Records calorie count accurately Performs weights accurately PERFORMS TREATMENTS AND PROCEDURES APPROPRIATELY Collects specimens, gives sitz baths Assists in physical examinations, special treatments or postmortem care as directed. Transports patients to and from various treatment centers when necessary; assiststransporters in moving patients in and out of stretchers and wheelchairs III. CLEANS EQUIPMENT AND INSTRUMENTS APPROPRIATELY MAINTAINS CLEAN WORK ENVIRONMENT Maintains the cleanliness of various instruments and equipment such as bedpans,urinals, and wheelchairs. Ensures that such equipment is labeled and properly storedwhen not in use. Empties canisters as needed. Follows appropriate Infection Controlprotocol. Keeps the patient's room in clean and orderly condition. Makes beds, replaces soiledlinen, straightens bedside tables, discards trash, keeps window ledges free of clutter,removes unused equipment and returns to storage after it is properly cleaned. MAINTAINS UNIT STOCK LEVELS OF SUPPLIES AND EQUIPMENT ASDIRECTED Assists in restocking rooms with supplies and equipment as directed. Checks trays, charts and cabinets for adequacy of inventory; assists with cleaning when necessary. PERFORMS PATIENT CARE ACTIVITIES APPROPRIATELY. Performs services for the patient involving bathing using basinless bath products, oralhygiene, care of hair, and oral feeding. Answers patient lights and performs services in a timely manner that adds to thephysical well-being of the patient. C. Participates in hourly rounds. Reports any unusual circumstances or change in patient's condition to the LPN and/or Supervisor. Recognizes and communicates cardiopulmonary arrest and initiates basic life support. Observes patients for unusual symptoms or signs of illness and reports significantfindings to LPN/Supervisor. Practices good isolation techniques to maintain a safe environment and to control thespread of infection. Performs all other duties as required. PROVIDES A LEVEL OF COMPETENCY TO PATIENTS AGES 18 ANDOLDER. Adults: Involves them with care planning by meeting any needs they identify duringassessment. Identifies their normal patterns of living and strives to attain them. Geriatric: Strives to obtain understanding by speaking distinctly and moving slowly. Involve family as much as possible. Explains all procedures, provides for adequate rest periods, and listens to theirconcerns and fears. VII. UPHOLDS ALL SERVICE STANDARDS Safety- We do no harm and create an error-free environment. Follow environment of care plan Follow established clinical practices Routine assessment of the environment for high risks with immediate correction (lookfor dangerous things and fix them; identify high-risk issues and fix them) Pay attention to detail Communication- We engage our customers in exchanging clear, timely and accurateinformation. At the start of the visits scribes checks the “document scribed” checkbox on the intake template at the top center. Under the direction of the provider, transcribes and places patient orders, including laboratory tests, radiology tests, referrals, etc. Documents any procedures performed by the provider or nurses in the EHR. Transcribes any consultation or discussions with family and/or the provider. Completes the patient's chart by transcribing results of any labs, x-rays, or other evaluations. In doing so, the scribe continuously checks on the progress of this data in order to get the patient's workup complete so that the provider is able to make his/her decisions regarding that patient. Scribe lists all proper diagnoses as well as any follow up instructions and prescriptions, as dictated by the provider. Scribe does not directly assist with patient care. The scribe documents orders generated by the provider. Scribe assists the provider and support staff with paperwork, telephone calls, faxing, and chart abstraction and may facilitate communication among the care team. Scribe is able to process portal communications and either task to appropriate member of the care team or get information from the provider to respond to the patient in a timely way. The Medical Scribe does not perform the following: Medications: A scribe cannot prescribe medications. Scribes cannot stop, start, or make any manipulation to any medications in the "medication module”. Orders: A scribe cannot order any in-house medications that are injected, ingested, or inhaled into the body including nebulizer treatments. Document Sign Off: The provider should always review the record and attest to its correctness and completeness. At the end of the day or shift, the provider should review the scribed documentation to ensure all events were recorded accurately. The provider generates the document, creating the completed "Master IM." The scribe does not. Patient Care: The scribe is not directly involved with patient care; can't touch the patient; can't handle bodily fluids; can't act independently without direct clinical oversight; conduct other duties while acting as the medical scribe; however, he/she may be asked to give patient paperwork during visit or discuss discharge instructions with a patient. The scribe can't give orders to Registered Nurses. They interact with patients on a limited level. For exceptions to this section, please see the “Optional Trainable Skills” section below. General Documentation: A scribe cannot document any of his/her personal objective findings or be involved in any of the medical decision-making. Work schedule: Monday through Friday 8:00am to 5:00pm, evening shifts of 11:00am to 8:00pm and Saturday shifts with advanced notice. Responsibilities Scribe-Specific Tasks and Duties: * The primary role of the scribe is to assist the provider with documentation of each patient's clearly recording each patient's symptoms, history, physical exam, and documenting the diagnosis as well as treatment plans in the medical record during a clinic visit. * At the start of the visits scribes checks the "document scribed" checkbox on the intake template at the top center. * Accompanies the clinician into the patient examination area, or joins the appointment remotely by telephone, in order to transcribe a history and physical examination as given by the patient and provider. * Under the direction of the provider, transcribes and places patient orders, including laboratory tests, radiology tests, referrals, etc. * Documents any procedures performed by the provider or nurses in the EHR. * Transcribes any consultation or discussions with family and/or the provider. * Completes the patient's chart by transcribing results of any labs, x-rays, or other evaluations. In doing so, the scribe continuously checks on the progress of this data in order to get the patient's workup complete so that the provider is able to make his/her decisions regarding that patient. * Scribe lists all proper diagnoses as well as any follow up instructions and prescriptions, as dictated by the provider. * Scribe does not directly assist with patient care. The scribe documents orders generated by the provider. * Scribe assists the provider and support staff with paperwork, telephone calls, faxing, and chart abstraction and may facilitate communication among the care team. * Scribe is able to process portal communications and either task to appropriate member of the care team or get information from the provider to respond to the patient in a timely way. The Medical Scribe does not perform the following: * Medications: A scribe cannot prescribe medications. Scribes cannot stop, start, or make any manipulation to any medications in the "medication module". * Orders: A scribe cannot order any in-house medications that are injected, ingested, or inhaled into the body including nebulizer treatments. * Document Sign Off: The provider should always review the record and attest to its correctness and completeness. At the end of the day or shift, the provider should review the scribed documentation to ensure all events were recorded accurately. The provider generates the document, creating the completed "Master IM." The scribe does not. * Patient Care: The scribe is not directly involved with patient care; can't touch the patient; can't handle bodily fluids; can't act independently without direct clinical oversight; conduct other duties while acting as the medical scribe; however, he/she may be asked to give patient paperwork during visit or discuss discharge instructions with a patient. The scribe can't give orders to Registered Nurses. They interact with patients on a limited level. For exceptions to this section, please see the "Optional Trainable Skills" section below. * General Documentation: A scribe cannot document any of his/her personal objective findings or be involved in any of the medical decision-making. A scribe cannot act independently; instead, he/she must simply document the provider's dictation and or/activities during the visit. * Optional Trainable Skills: * With the permission of medical/operational leadership and participation in an in-house training/competency program, a Scribe can be trained to perform vitals, room patients. * Other duties as assigned Qualifications Education and Experience: * High School Diploma or Equivalent (GED) required. * Experience with EPIC Electronic Health Records preferred. POSITION SUMMARY The Medical Scribe assists the provider with medical documentation to increase the efficiency and productivity of the clinical visit. The Medical Scribe does not provide direct patient care and follows local, state, and federal guidelines in an interdependent role in assisting clinical and front office staff. DUTIES AND RESPONSIBILITIES To be performed in accordance with RFHN Policies and Procedures * Reviews charts with the provider prior to patient visits to identify preventive care to be addressed during the visit. * Documents the history and physical exam findings during the patient visit as communicated in the patient and provider encounter. * Documents all procedures performed by the provider or clinical staff in the EHR. * Uses templates and work-flow documentation tools effectively to ensure that encounters are completely documented as well as enabling the collection of data for statistical reports. * Transcribes the consultations and discussions between family members and the provider. * Enters the results of labs, x-rays, or other evaluations into the EHR. * Monitors the progress to completion of the patient's work-up data to support the provider's decisions regarding the treatment plan. * Documents confirmed diagnoses as well as follow-up instructions and pends orders for diagnostic imaging, labs, referrals, and prescriptions, as dictated by the clinician. * Facilitates the consistent and complete use of PHQ9 or PHQA, drug alcohol tobacco brief screen, and any other screenings assigned to medical scribes. * Facilitates medication reconciliation. * Distributes "Reach out and Read" books. * Completes documentation on the patient record by end of regular scheduled work hours. Assesses for barriers to obtaining services (transportation, access, etc.) and follows up on what resources the patient might need to obtain screening. * Adheres to the scope of the role of medical scribe when documenting the patient's response to the provider's questions: a. Collects patient HPI data only while the provider is in the room, except for screening tools. b. Documents patient history as obtained by the provider; history is not taken independently. c. Documents the review of systems independently and records patient responses to provider questions. * Completes documentation of the physical exam, assessment and plan with the direction of the provider. The medical scribe role responsibilities do not include or permit medical decision-making. * Seeks clarification on details of the physical exam as needed and verifies they are choosing the correct assessment as it relates to lab, diagnostic tests, referrals, procedures, and other orders. They will review the chart completely with the provider.
