The Junior Facilities Stager is a highly organized and motivated individual. This position is responsible for assisting the smooth and successful staging of new facilities and the upkeep and maintenance at existing facilities that provide services to clients with autism spectrum disorder. Required Qualifications, Skills, and Knowledge:
High School Diploma/GED required
Minimum 2 years' experience in general building troubleshooting, maintenance, and repairs
Project Management, Construction Management, or Trade Education encouraged
2+ years of related work experience and technical training.
2-3 years of experience in construction or new site openings, preferred.
2-4 years of experience in facility maintenance, preferred.
Working knowledge of laptop/desktop PC

Perform up to 20 visits per week, ensuring compliance with required timelines (Rehab: every 30 days; PDN: every 60 days).
Prepare and process all documentation associated with scheduled visits.
Review recent shift notes to understand client status and evaluate documentation quality.
Conduct root-cause analysis related to client hospitalizations.
Maintain client care plans, including creating, updating, sending, and tracking POCs and addendums.
Provide clinical education to clients, families/guardians, and field staff as needed.
Communication & Support
Build relationships with field staff to offer support, coaching, and mentorship.
Communicate updates on client status promptly and effectively.
Collaborate with Clinical Leadership, Operations, and Healthcare teams for escalated care needs or staff support.
Provide a positive and professional experience for all clients and staff.
Compliance & On-Call Duties
Perform Telehealth On-Call responsibilities when required.
Ensure compliance with Centria's Code of Conduct, state and federal regulations, and payer/funding source requirements.
Report any violations of company policies or the Code of Conduct.
Financial & Managerial Components
Act as an individual contributor with some financial oversight responsibilities.
Provide mentorship to designated clinical team members to support skill development and performance.
Qualifications
Education & Licensure
Current Registered Nurse (RN) license in good standing (required)
Bachelor of Science in Nursing (BSN) preferred (one version listed required; optional to list as required or preferred based on your organizational preference)
Work Experience
Minimum 1+ year of nursing experience
Knowledge and/or experience in home care strongly preferred

Our client is a very friendly paraplegic woman that has a peg tube. Due to her being wheelchair-bound and having limited use of her right hand, she requires assistance with the following:
Range of motion exercises
Breathing treatments
Getting dressed
ADLs (Activities of Daily Living)
Transportation to doctor's appointments and other accident related appointments (aide must be able to transport)
She currently receives weekly speech therapy, and physical and occupational therapy
🕒 Shift times: 7:00 am - 3:00 pm or 3:00 pm - 11:00 pm🏡 About the Role
Centria caregivers provide essential in-home support to help individuals remain safe, comfortable, and independent. You'll work one-on-one with clients in a private home setting, supporting their unique care needs.
✅ Responsibilities
Assist with mobility and transfers to prevent falls
Provide basic personal care, including hygiene, toileting, and dressing
Prepare light snacks or drinks upon request
Offer companionship and emotional reassurance throughout the shift
Assist in positioning and turning to prevent discomfort
Document and report changes in client condition promptly
Respond to emergencies following proper procedures
Perform duties as outlined in the patient's plan of care
Monitor client's physical and mental condition, intake/output, and exercise
Maintain a safe, clean, and healthy care environment
Follow professional standards, care policies, and local/state/federal requirements
📝 Qualifications
CNA, HHA, or equivalent experience in home care preferred
Experience working with fall-risk and mobility-impaired clients strongly desired
Must be comfortable assisting with personal care tasks
Reliable, patient, and attentive to individual needs
Comfortable working in a private home setting
Ability to work independently and follow a care plan
Strong oral and written communication skills
Ability to follow verbal and written instructions
Comfortable using tablets/computers for documentation
Strong attention to detail and ability to follow all care protocols
Ability to address minor medical concerns as they arise
