Our client is a 72-year-old woman with limited mobility and a Traumatic Brain Injury (TBI) resulting from a motor vehicle accident (MVA). She requires comprehensive assistance to safely and comfortably manage her daily routine.
🌟 Key Responsibilities & Requirements:
ADL Support: Provide full assistance with Activities of Daily Living (ADLs), including bathing, dressing, hygiene, and positioning.
Transfers: Safely assist with transfers between her bed, wheelchair, and commode. Experience with safe transfer techniques is essential.
G-Tube Care: Provide competent and professional care related to her G-Tube (gastrostomy tube).
Companionship & Observation: Offer supportive companionship and diligently observe for any changes in condition throughout the night.
**Special Consideration:
To enhance our client's comfort and ability to communicate effectively, we are strongly seeking candidates who are proficient in Arabic (spoken).

The RN Field Staff Supervisor (Visiting RN) is responsible for completing in-home assessments, medication reconciliation, supply inventory, and ongoing medical status updates for Rehab Home Care (RH) and Private Duty Nursing (PDN) clients. This role ensures clinical compliance with accrediting agencies, state regulations, and funding source requirements while delivering exceptional support to field staff and clients.
This position plays a key role in maintaining patient safety, clinical quality, and strong communication between operations, clinical leadership, and field staff.

Duties & Responsibilities
The following essential functions reflect the core requirements of the RN Field Staff Supervisor. Duties may evolve based on client needs, regulatory requirements, or organizational priorities.
Client Visits & Clinical Oversight
Monitor, schedule, and complete client visits (primarily face-to-face; telehealth as allowed).
Perform up to 20 visits per week, ensuring compliance with required timelines (Rehab: every 30 days; PDN: every 60 days).
Prepare and process all documentation associated with scheduled visits.
Review recent shift notes to understand client status and evaluate documentation quality.
Conduct root-cause analysis related to client hospitalizations.
Maintain client care plans, including creating, updating, sending, and tracking POCs and addendums.
Provide clinical education to clients, families/guardians, and field staff as needed.
Communication & Support
Build relationships with field staff to offer support, coaching, and mentorship.
Communicate updates on client status promptly and effectively.
Collaborate with Clinical Leadership, Operations, and Healthcare teams for escalated care needs or staff support.
Provide a positive and professional experience for all clients and staff.
Compliance & On-Call Duties
Perform Telehealth On-Call responsibilities when required.
Ensure compliance with Centria's Code of Conduct, state and federal regulations, and payer/funding source requirements.
Report any violations of company policies or the Code of Conduct.
Financial & Managerial Components
Act as an individual contributor with some financial oversight responsibilities.
Provide mentorship to designated clinical team members to support skill development and performance.
Qualifications
Education & Licensure
Current Registered Nurse (RN) license in good standing (required)
Bachelor of Science in Nursing (BSN) preferred (one version listed required; optional to list as required or preferred based on your organizational preference)
Work Experience
Minimum 1+ year of nursing experience
Knowledge and/or experience in home care strongly preferred
Knowledge of SCI, TBI, respiratory failure, and pediatric populations a plus
Technology Requirements
Basic computer proficiency
Working knowledge of a laptop/desktop PC
Proficiency in Microsoft Office Suite and Google Workspace (G Suite)
