On-call crisis therapists will work 12 hour shifts providing as-needed telephone, virtual, and face-to-face emergency and triage services to adults, adolescents, and children. Consults with other community caregivers to strengthen and coordinate social intervention programming.
Charts the client's progress in the clinical record and completes all paperwork necessary by MHS policy for professional, administrative, financial, and evaluative purposes of monitoring clinical care and of assuring efficient and effective agency operations.
Participates in quality improvement activities by reviewing the clinical charts, reviewing the criteria for admission/continued stay/discharge, performing patient care audits, and investigating adverse occurrences in treatment.
Screens for pain and encourage clients to follow up with a health care provider.
Reads relevant professional literature, attends conferences, seminars, workshops, and in-service training as required to remain current on best treatment protocols and techniques.
Is responsible for delivery of number of units of service budgeted in order to assure the fiscal viability of MHS.
Follows all universal precautions for safety, infection control and follows all state, federal and joint commission health and safety standards.
Treats clients and family with dignity and respect at all times and holds all patient information in the strictest confidence.
Adhere to professional standards, policies and procedures, federal, state and local requirements and Joint Commission standards, including National Patient Safety Goals. Presents a positive image of MHSCC to other community agencies, caregivers and citizens.
Completes all MHS required education and training, including initial agency orientation, mandatory trainings and education, and upkeep of all required certifications and licensure as required by state, federal and regulatory requirements.

Education/Experience
Bachelor's degree in social work, counseling, or related field (Required)
Master's Degree social work, counseling, or related field or Ph.D./Psy.D (preferred)
Experience with mental/behavioral health and /or SUD/dual diagnosis (preferred)

Shift: Part Time and PRN
Responsibilities
Provide physical therapy services to patients according to a plan of care. This may include assessing and evaluating therapeutic/rehabilitative/functional status and participation in developing the total plan of care as well as providing direct physical therapy treatment.
Assess patients for muscle strength, mobility, gait, ROM and transfer capabilities.
Instruct patients, families and caregivers in the use and care of therapeutic appliances.
Report patient's reaction to treatment or changes in care to patient's physician as needed and appropriate.
Initiate physical therapy programs and instruct other personnel, patient and/or family/caregivers in their therapy program, as well as the goals of therapy.
Supervise the Physical Therapist Assistant (PTA) as necessary.
Participate in Otterbein Universal Home Health Quality Assessment/Performance Improvement program.
Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care.
Prepare and submit clinical and progress notes in a timely manner and in accordance with Otterbein Universal Home Health, state and federal guidelines and regulations.
Education and Experience
Education: Bachelor's degree or master's in physical therapy.
Experience: Minimum 2 years of physical therapist experience with community / home health preferred.
Licensure, Registration, and/or Certification: Current license as a Physical Therapist from the State of Ohio or made application for same. Must have a valid Ohio driver's license and access to reliable transportation. Current CPR certification required.
Complete a minimum of twelve (12) hours of in-services per year.
