About the Unit:
High Intensity Care Unit (HICU) - (Vent/Pulmonary Care/Acute Rehab): Comprised of 20 semiprivate rooms and can accommodate up to 40 patients. A team approach is taken to provide care for patients who may require attention for an acute brain injury, need respiratory assistance or special monitoring or require rehabilitation. Patients are monitored for cardiac conditions and serious complex conditions.
HICU staff members are outgoing and positive and must utilize strong organizational and leadership skills to assess patient status, implement clinical nursing interventions to modify symptoms, and develop and implement patient care plans to enhance and improve outcomes.

JOB SUMMARY:

The selected individual will preformcomplex professional and advanced human support work in the care and treatment of patients in a public health program, planning, organizing and supervising the daily operation of medication management as a part of a team that includes the prescriber, pharmacist, other nursing staff, counselors, case managers and peer recovery specialists. Essential Functions
Assists the prescriber in conducting physical and mental assessments of patients; monitors and documents patient symptoms.

Provides education to patients about the impact of opioid addiction on physical health, and monitor related medical conditions such as TB, HepC , HIV/AIDS and STDs, working with case management and counseling staff to refer patients to medical care resources.
In consultation with the pharmacist, provides education to patients about potential drug interactions.
Obtains physician orders; orders medications; collaborates with pharmacy; transcribes orders. Assists with acquiring pre-authorizations of medication refills per pharmacy/insurance request.
Administers medications both intramuscularly and orally.
Completes medically-related assessments including taking vital signs.
Draws blood and performs urine drug screens; labels specimens via computer printout; administers and interprets TB tests; processes gel top blood tubes via centrifuge; assists, monitors and tracks lab results.
Assesses patient service needs, capabilities and appropriateness for services; presents options and services based upon a needs assessment.
Maintains thorough documentation of daily activities including nursing, treatment services, crisis intervention, case management, reimbursement, and productivity statistics.
Attends and participates in staff meetings as required.
Responds to medical emergency situations, including the administration of naloxone to reverse opioid overdose, and calls emergency medical services for consumers and staff when warranted.
Maintains inventory of medical supplies, including coordinating with Pharmacy to order take-home bottles and other stock supplies with the approval of the Program Manager.

Staff Related:
Participate in staff meetings as scheduled.
Assist in short term and long term patient care plan meetings.
Acquire information and knowledge in current practice related to dialysis principals and technique by participating in scheduled in-service classes.

A Day in the Life of an LPN:
Administers prescribed medications and treatments in accordance with approved nursing techniques.
Provides accurate and descriptive records of medical and nursing care of the residents.
Assigns daily resident care duties to Nursing Assistants and Hospitality Aides, monitors and evaluates the delivery of care.
Accurately transcribes and carries out all physicians' orders and assists with physician visits.
Participates in resident care planning.
Evaluates and is alert to the physical and psychological needs of the residents and responds in accordance with proper nursing techniques and protocol.
Assists with admissions and discharges of residents.
Assists in the management of Nursing Assistants and Hospitality Aides.
Assists with direct resident care as time and responsibilities permit.
Assists with carrying out physician orders.
Treats all information about residents, their condition, and family as well as personnel matters as confidential information.
Complies with established Corporate and Departmental policies and procedures and maintains established standards and practices.
Performs other functions as directed by the supervisor.
Our Requirements:
Current and valid LPN license in the state you will be providing care. If you are currently a student in an LPN program, please see the Graduate Practical Nurse (GPN) position.
Judgment capabilities, initiative, and dependability.
Ability to read, write, and understand English well.
Ability to stand, walk, stoop, twist, and turn frequently throughout the course of an eight-hour shift.
Ability to lift 30 pounds in assisting co-workers with resident lifts, transfers, etc. several times throughout the shift.
Standing and walking distances constantly throughout the campus is required during the shift while evaluating residents and supporting line staff and (at applicable facilities) responding to emergency needs of the Retirement Community.
Ability to push a med cart in dispensing medications.
Ability to react quickly and decisively in emergencies and in unexpected behavior of residents.
Ability to understand and follow oral and written instructions.
