Assists in monitoring physical and mental status of Veterans/Members by utilizing appropriate medical equipment and techniques to take vital signs (temperature, pulse, respirations); obtain urine, sputum or stool samples; and track input/output
Responsible for medical record and worksheet documentation on all assigned Veterans/Members. Keeps records and prepares simple reports.
Assists Veterans/Members with dining to include transporting to the dining room, informing them about menu choices available for the day, communicating dining staff about Veterans'/Members' choices and serves Veterans/Members their meals in courses, assisting them if necessary
Provides resident care and comfort by responding to patient requests (verbal or call light) for assistance timely in accordance with the Resident Bill of Rights
Maintains cleanliness of areas neat and sanitary by changing bedding, linens and other items
Reports to and assists licensed nurses in emergency situations rapidly and appropriately following established policies and procedures
Performs other related duties such as transporting Veterans/Members to appointments outside the facility and clerical duties (e.g., answering phones, filing, etc.) as needed

POSITION SUMMARY
This position will provide services in the Ambulatory Care Department as assigned. ESSENTIAL FUNCTIONS
* Assists with direct patient care procedures and related tasks; checks patients in, assists in obtaining patient histories, takes vital signs, prepares records and assists with medical examinations. Complies with the laboratory requirements (i.e., CLIA, Joint Commission) for quality control of waived testing for accuracy and record keeping.
* Obtains bacteriologic specimens such as clean-catch urine specimens and topical cultures. Assists with incision and drainage procedures.
* Performs less complex dressings and treatments without assistance. Assist with cleaning, irrigating, and dressing complex wounds.
* Prepare and administer medication as directed by the LIP. Administer injections - IM (intermuscular), SQ (Subcutaneous), and ID (Intradermal).
* Instruct patients in proper collection of urine samples. Prepares samples for analysis (e.g., labels specimen tubes). Routes requisition forms to consultative services, dietary, laboratory and radiology.
* Shares the responsibility for reporting observations by observing the patient's conditions and reactions, and by keeping the provider informed of changes in clinical condition.
* Schedules and monitors patient appointments; may coordinate routine office like activities and administrative functions with supplemental staff of the clinic. Shares the responsibility of maintenance of clinical records. Utilize established software to data enter immunizations, follow-up on laboratory and radiological exams, or other computer functions as required by mobile clinical services or outpatient department.
* Educates and advises patients on specified medical issues within established parameters. Practices safety, environmental, and/or infection control methods. Prepares any patient related documentation for the provider or patient.
* Maintains a courteous and professional manner in contact with patients, family, and other clients. Assists in answering telephones and supplying routine information per policy.
* Performs general non-direct patient care duties at the site-based clinics including cleaning, arranging, and restocking supply cabinets from supplies; cleaning and sorting supplies, instruments and equipment; calling attention to deficiencies in supplies and equipment; cleaning refrigerators and assisting with maintenance of temperature logs; and maintains a clean and safe patient environment.

Essential Job Functions, Duties, and Responsibilities
Perform simple procedures as an extension of therapy services under the direction and supervision of the nurse, e.g. range of motion exercises, assistance in ambulation or exercises.
Performs personal care activities including bathing, grooming, dressing, toileting, skin care/nail care, oral hygiene, shaving, dressing, and others as directed/requested.
Performs household services essential to health care at home including meal preparation/feeding, laundry and ironing, changing/laundering bed linens, light housekeeping and other services as offered.
Initiates and participates in leisure activities provided for residents. Encourages residents to socialize and participate in planned activities or programs and to develop friendships with other residents.
Observes and documents the health and emotional condition of each resident, promptly reporting all changes to the nurse.
Responds promptly and courteously to resident requests for assistance and emergency pull cords.
Promotes personal safety and a safe environment for residents by observing infection control practices, following facility guidelines, and reporting unsafe situation to the nurse.
Assist residents in preparing and transporting for appointments and social programs.
Assist with moving and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
Assist residents to walk with or without self-help devices as instructed.
Required Skills and Qualifications
Must possess the ability to make independent decisions, follow instructions, and accept constructive criticism. Must be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must be able to work with ill, disabled, elderly, and emotionally upset people within the facility. Must be able to speak, write and understand English in a manner that is sufficient for effective communication with supervisors, employees, residents, and families.
Education and Experience
Must be a licensed Certified Nursing Assistant or a licensed Medication Assistant in accordance with laws of this state.
Prefer 6 months or more of experience in a home care, assisted living, or long-term care facility
Supervisory Responsibility
This position does not have any direct reports or supervisory requirements.

Requirements:
QUALIFICATIONS
Essential
High School Diploma or equivalent
A high-level dedication to excellent patient care, office teamwork, detail and accuracy are required
Must have good oral and written communication skills, good computer skills, be flexible and dependable, and be able to multitask in a fast-paced medical office environment
A Medical Assistant diploma or certificate from an accredited facility
Desirable
