Job Summary
Transplant APC (Physician Assistant/Nurse Practitioner) Provides inpatient/outpatient/OR based both primary and specialty medical services for the division of Transplantation and Advanced Hepatobiliary Surgery.
This position is responsible for providing both primary and specialty medical services. This position may be required to access and administer medications within their scope of practice and according to state law.
Responsibilities

Position Summary
Under the supervision of the Division Director of Intensive Behavioral Services, the Program/Medical Assistant organizes, coordinates, and monitors all nonclinical operations of the team along with providing receptionist activities including triaging calls, coordinating communication between the team and clients, deescalating situations with clients who have increasing symptoms, and providing assistance to clients, their families, and other agencies. This position will also provide back-up Medical Assistant duties such as lab draws, injections and sitting with clients after they have received their injection.
Essential Duties
Greet people and answer phone calls including performing the following:
Triage and coordinate communication between ACT team members, clients, other agencies, and hospitals.
Obtain answers to questions for clients, families, community resources, and agencies.
Relieve urgent situations or temporarily manage them until other staff are available.
De-escalate or attend to symptomatic behavior of clients who come to the office.
Decide when and how quickly to refer calls to other staff.
Provide general administrative and clerical support to clinical and medical staff (schedule appointments, add new clients to EHR, bus pass tracking, monthly and weekly note tracking, enter and track SOQs monthly, monthly Medicaid checks).
Maintain professional boundaries and client confidentiality at all times.
Meet ACT documentation requirements.
Attend work on a regular and predictable basis.
Work must be performed on-site due to the need to provide direct client care and team support.
Secondary Duties

Position Summary

* Track clients' medications and assist in completing patient assistance applications.
* Facilitate medication refills when needed and appropriate. Provide medication injections and draw labs when needed.
* Engage and build rapport with clients, assist with referrals to link and connect clients with services and resources including primary care, mental health treatment, addiction treatment, recovery support, employment specialists, housing specialists and other social supports.

* Successful completion of an Accredited Medical Assistant program and a High School diploma or GED equivalent.
* Knowledge of Microsoft Office Suite.
* Knowledge of multiple phone line systems.
* Ability to communicate effectively with staff and clients.
* Experience in an outpatient mental health setting preferred.
* Demonstrates an ability to use sound judgment within the scope of the position's responsibilities.
* Ability to maintain professionalism and adapt to change.
* Organized, detail-oriented, self-starter.
* Excellent verbal and written communication skills; attention to detail.
* Demonstrated skills in crisis intervention.
* Driving is required: must be at least 21 years of age, possess a current driver's license, have a good driving record, and be insurable on the agency liability policy.
* Must be able to pass Utah-DHS - Office of Licensing background screening and pre-employment drug screen.
Physical Demands
* Ability to lift and carry 25 lbs.
* Ability to walk up and down stairs.
* Ability to move around the facility, interacting with clients for extended periods.
* Work is performed inside and outside of an office environment; must have ability to climb stairs, enter client homes, as well as deliver services in shelter, on the street, etc.
