Responsibilities: The Patient Care Assistant maintains the clinical records of the hospice patients accurately and in accordance with the approved format per policy. It is also the responsibility of the Patient Care Assistant to act as liaison between the other team members and to assist in the day to day management of the patients and families we care for through the hospice program. This position reports directly to the Director of Patient Care Services. At least 2 years of recent experience in an acute/sub-acute care setting. Assures both physician signatures obtained on Initial Plan of Care within 5 days of admission. Enters data into Mumms software as well as visits. Acts as a go-between for staff, volunteers and patients/families. Responds appropriately and professionally to all family/patient, professional and staff phone calls. Sets up transport for LOC changes. Completes and is responsible for faxing paperwork required per facility guidelines. Initiates paperwork for coordinating care plan in LTC facilities. Prepares binders, as needed, for LTC facility charts. Assists with scheduling and staffing as requested. Assures that each patient's clinical record is complete and that all required documentation is filed in a timely manner. Patient Care Assistant will notify the appropriate team members of patient discharge and close the file per approved format. Orders and monitors supplies and durable medical equipment. Prepares documentation for interdisciplinary team meeting as directed. Accepts field assignments as needed. Accepts on call assignments when requested and scheduled. Other assignments as required. Qualifications Licensed Practical Nurse must be currently licensed and in good standing with the State Board of Nursing in the state of licensure. The LPN shall have at least 2 years experience in nursing as an LPN and at least a minimum of one year experience in hospice or long term care. The role of the Oral Surgery Dental Assistant is critical in providing exceptional patient care resulting in the best clinical outcomes and meeting the needs of our patients. Role and Responsibilities The Oral Surgery Dental Assistant will help surgeons during surgery by performing duties such as tissue retraction, intravenous lines, and completing documentation. * Performs preoperative and postoperative duties to facilitate patient care. * Verifies the identity of patients and operative sites. * Discusses with surgeon the nature of the surgical procedure including operative consent and methods of operative exposure. * Monitors and maintains aseptic technique throughout procedures. * Maintains an unobstructed operative field using surgical retractors, sponges, and suctioning and irrigating equipment. * Determines availability of necessary equipment and supplies for operative procedures. * Obtains and inspects sterile and non-sterile surgical equipment, supplies and instruments. * Operates sterilization devices. * Monitors patient intraoperative status. * Obtains dental radiographs and dental imaging. * Completes patient clinical narratives. * Other related duties as assigned. Education and Experience * High School diploma or equivalent required. * BLS certification required or the ability to complete within 1 month of hire. * At Least one year of related experience preferred. * DAANCE certification preferred. * Licensed dental assistant preferred. * Radiology certification preferred. Skills and Abilities * Basic computer knowledge. * Ability to work well in a team and individually. * Good Judgment and critical thinking skills. * Passion for keeping people safe. * Strong attention to detail, vigilance, and meticulous care on the job. * Excellent manual dexterity. * Superior listening skills with the ability to quickly comprehend instructions in emergency situations. * Basic knowledge of medical terminology.
