PURPOSE AND SCOPE:
A Certified Registered Nurse Anesthetist (CRNA) in an advanced practice registered nurse (APRN) who practices both autonomously and in collaboration with a variety of health providers on the interprofessional team to deliver high-quality, holistic, evidence-based anesthesia and pain care services. The CRNA cares for patients at all acuity levels across the lifespan in a variety of settings for procedures including, but not limited to, surgical, obstetrical, diagnostic, therapeutic, and pain management.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Perform and document the pre anesthesia assessment and evaluation of the patient.
Review, request, and order (as permitted by state law), laboratory/diagnostic studies.
Obtain informed consent for anesthesia and related services.
Select, obtain, order, and administer preanesthetic medication or treatment.
Develop and implement an anesthesia care plan.
Select, obtain, prepare, and use equipment, invasive and non-invasive monitors, supplies, and drugs for the administration of sedation, anesthesia, and pain management services; perform safety checks as needed.
Select, order, obtain, and administer analgesic and anesthetic agents, adjuvant drugs, accessory drugs, fluids, necessary to provide anesthesia services.
Document a time- based record of events during peri-anesthetic care
Perform all aspects of airway management, including fiberoptic intubation.
Facilitate emergence and recovery from anesthesia by evaluating patient response, and selecting, ordering, and administering medications, fluids, and ventilator support to maintain patient stability during transfer.
Initiate and administer post-anesthesia pharmacological or fluid support of the cardiovascular system.
Initiate and administer respiratory support to ensure adequate ventilation and oxygenation in the post anesthesia period.
Evaluate the patient's response to anesthesia and surgery; take appropriate corrective action post anesthesia.
Provide post anesthesia follow-up evaluation care
Discharge the patient from the post anesthesia care unit
Implement acute and chronic pain management modalities
Manage emergency situations by assessing, stabilizing, and determining disposition of patients, including providing airway management, administration of emergency
Place and manage regional anesthetic techniques, as delineated by his or her clinical privileges or granted special privileges.
Insert and manage peripheral, arterial, pulmonary catheter, or central venous lines and catheters.
Provide patient and caregiver education.
Consult with respiratory care providers and Implement respiratory and ventilator care when appropriate.
Supports patient safety when performing his or her job functions and through participation in facility, department, or unit patient safety initiatives.
Takes action to correct observed risks to patient safety.
Reports adverse events and near misses to appropriate management authority.
Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.
Using an interdisciplinary approach, including consultation or referral to another clinician, as appropriate.
Perform advanced and interventional pain management using drugs, regional anesthetic techniques, or other accepted pain relief modalities with or without fluoroscopic guidance.
Utilize ultrasound, fluoroscopy, as appropriate.
Participates in center quality meetings, medical executive meetings and other management meetings upon request from the Medical Director.
Review and comply with the Code of Business Conduct and all applicable company policies and procedures, local state and federal laws and regulations

The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/ materials and radiation. May be required to wear protective gear in procedure rooms for extended periods of time. Majority of the time spent on feet, may require lifting, and positioning of patients. Position Requires ability to lift objects heavier than 50 pounds.
Travels between locations primarily during the business day, although some out-of-the-area and overnight travel may be expected.
This position may require approximately 10- 15% travel.

EDUCATION AND REQUIRED CREDENTIALS:
Graduate of a nurse anesthesia educational program accredited by the Council on Accreditation of Nurse Anesthesia Educational Programs or its predecessor.
Current Appropriate State Licensure and complies with any applicable state statutory and regulatory requirements concerning CRNAs.
BLS and ACLS certification
Is currently certified or recertified by the National Board of Certification and Recertification for Nurse Anesthetists.
Insurable by an appropriate medical malpractice insurer for the limits required by this institution or state.
EXPERIENCE AND SKILLS:
8 - 12 years' related experience or an equivalent combination of education and experience
5 years CRNA experience preferred
Ability to perform clinically as a nurse anesthetist in the facility setting on a regular basis.
Ability to operate, use, and interpret data from a wide variety of equipment to perform anesthesia care as well as necessary perioperative or operating room care to support the safety and wellbeing of the patient while they are under the direct care of the CRNA.
Effective verbal and written communication skills are required to effectively collaborate with various clinical staff and other disciplines.
Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served.
Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.
Knowledge of infection prevention and control techniques.
Ability to maintain confidential records.
Ability to implement and adhere to facility policies and procedures.

Primary Responsibilities:
Conduct in-home assessments on health plan members, including past medical history, review of symptoms, physical examination, medication review and depression screening

The Nurse Practitioner, Bridging Care Provider (NP) acts as part of the clinical operations team and is responsible for providing direct patient care in ChenMed medical centers virtually and in the home depending on the nature of the assignment. Under the guidance of the Regional Chief Clinical Officer, the NP will train new PCPs and provide bridging care to patients. The responsibilities include but are not limited to: geriatric assessment, medical history, physical exam, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to ChenMed standards for quality, service, productivity and teamwork. It also includes participation in clinical rounds and conferences plus in-depth documentation through written progress notes and summaries.
The Nurse Practitioner must demonstrate the ability to function both independently and in collaboration with other health care professionals. Consults with the manager, physician, and medical director to ensure compliance with guidelines. This position may require participation in risk and quality management programs, clinical meetings and other meetings.
The Nurse Practitioner will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
Functions independently as a certified nurse practitioner for a patient population in collaboration and consultation with a licensed physician. Practices in accordance with a written or electronic practice agreement.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
In collaboration with the Regional Chief Clinical Officer (RCCO), train new PCPs in a market on using our technology/ EMR
In collaboration with the Regional Chief Clinical Officer (RCCO), provide bridging care in centers that are understaffed with providers or need clinical coverage by seeing patients in clinic or virtually, addressing workflow items and returning patient phone calls
Independently assesses acute and non-acute clinical problems. Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan in response to the data obtained. This also includes assisting in the development of the nursing plan of care in addition to providing appropriate patient/ family/significant other counseling and education.
Plans patient care based on in-depth knowledge of the specific patient population and/ or protocol, anticipating and identifying physiological and/ or psychological problems commonly encountered including the consideration of the patient's cultural background, level of understanding, personality and support systems. Serves as a patient advocate.
Patient management includes the following:
Communicating with patients virtually or in a clinic
Ordering and interpreting appropriate laboratory and diagnostic studies.
Ordering appropriate medication and treatments.
Referring patients for consultation when indicated
Documenting in-depth progress notes and summaries. Performing invasive procedures independently upon the completion of documented competency.
Participating in patient care rounds and conferences, communicating patient management strategies to members of the patient care team; collaborating with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care needs.
Recognizing situations that require the immediate attention of a physician and initiating life-saving procedures when necessary.
Using advanced communication skills to solve complex situations and improve processes and patient service.
Other Responsibilities may include:
Collaborating with other multidisciplinary team members to analyze and evaluate current systems of health care delivery to identify and implement new practice patterns as appropriate.
Participating in outside activities that enhance personal and professional growth and development.
Prescribing medication to patients based on regulations by the state of practice.
Other duties as assigned and modified at the manager's discretion.
EDUCATION AND EXPERIENCE CRITERIA:
ARNP or similar advanced degree in Nursing required
Current Nurse Practitioner Certification in the State of practice required
Board certification by AANP or ANCC is preferred but may be required for certain States
Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice
A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates
Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment

Once through orientation, independently manage patients with multiple, complex conditions through a balance of tele-assist home visits (video visits with field care coordinators in the home) and telephone/video interventions.
Complete thorough initial visits with patients, diving deep into medical, social and behavioral context.
Complete ongoing follow up visits with patients, focusing on geriatrics and chronic condition management.
Complete post-discharge visits on recently admitted patients, including comprehensive medication reconciliation, Specialist coordination, and chronic condition management
Complete engagement and outreach visits on patients who have not been seen by their primary care provider this year or who have chronic condition opportunities
Work directly with the patient and through the multidisciplinary care team to impact ongoing care with particular emphasis on preventable hospitalizations and ensuring care aligned with patient goals.
Collaborate and coordinate with the patient's primary high risk care team and primary care team and specialists. Participate actively in team huddles to review care plans and assess patient goals of care and care plans.
With support of the team, respond to patient and patient's family telephone calls.
Participate in the market after-hours call pool.
What you’ll need
Completed an accredited NP program with a national certification (ANCC, AANP) in Family, Geriatrics, or Adult
5+ years of complex home based care, transitions of care, geriatric primary care, or palliative care experience as a Nurse Practitioner
Bilingual in Spanish and English highly preferred
Currently licensed in Washington and able/ willing to obtain licensure in the state of Arizona and additional States as requested
Flexibility to work in Pacific Time Zone and other time zones as needed

* Manages the care of women and their newborns and gives direct client services including: antepartum, intrapartum, postpartum, newborn, family planning, and gynecological care, according to the standards of the American College of Nurse-Midwives, the American Association of Birth Centers, and the policies, procedures and clinical guidelines of Community of Hope at Family Health and Birth Center and Washington Hospital Center.
* Consults with the physician, according to the established risk criteria, regarding any client who develops symptoms of medical or obstetric complications.
* Co-manages with the physician the care of women who develop complications.
* Refers to medical management any woman who develops complications which, in the judgment of either or both the physician and CNM, are best managed by a physician.
* Plans and provides continuing health supervision and education to all clients under her/his care.
* Consults with and refers to other members of the health care team and community resources as indicated.
* Maintains complete, accurate, legible, and appropriate records.
* Is compliant with all federal regulations, HIPAA, OSHA.
* Is current with ongoing training and CMEs as required for licensing.
* Completes and locks all progress notes within 3 business days after patient visit.
* Documents all patient encounters, including face to face, telephone, and electronic communications, in the medical record in a timely fashion.
* Uses all mandatory, structured fields as required in eCW.
* Uses the patient alert system to prevent 'missed opportunities' for care.
* Signs off on all laboratory results, diagnostic imaging results, and referral reports in a timely fashion; sharing this information with patients as appropriate.
* Secures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential.
* Maintains effective and efficient working relations' with the birth center staff, consulting staff, other health disciplines, families and public.
* Attends required hospital and birth center meetings.
* Participates in public relations programs that publicize midwifery, advanced nursing practice and/or birth center care.
* Assists with providing key clinical information for fundraising purposes as needed.
* Attends Health Services' staff meetings and other all-staff meetings.
* Represents Community of Hope at health related community functions as requested.
Requirements
Minimum Qualifications:
* Current license as a Nurse-Midwife in in the District of Columbia required.
* National Certification as Certified Nurse Midwife required.
* Experience with both hospital and out-of-hospital births; experience with 40 or more births preferred; No new grads at this time.
* Current adult CPR certification (ACLS or BLS for the Healthcare Provider or equivalent) required.
* Current neonatal resuscitation certification (NALS or NRP or equivalent) required.
* Participation in appropriate continuous medical education;
* Bilingual in Spanish and/or Amharic preferred.
* Commitment to working with the underserved.
* Ability to work in a non-profit environment.
