A. / Transportation PRofessional Job Description Department Assisted Living (ALC) & Skilled Nursing (DSH) Reports to Assisted Living Administrator and DSH Administrator Position Purpose The ALC & DSH CNA/Transportation Professional supports Villagers during medical appointments by advocating for their needs, assisting with the appointment process, and empowering them to make informed decisions about their care. This role ensures Villagers receive the necessary support while respecting their autonomy in healthcare choices. Required Qualifications Certified Nursing Assistant (CNA) in good standing, meeting all relevant Georgia and federal requirements High school diploma or GED Proficiency in English (read, write, follow instructions) Valid Class C Georgia Driver's License, proof of insurance, and acceptable driving record Successful completion of background check, drug screen, and TB screening CPR/First Aid certification Physical capacity to lift up to 25 lbs independently and assist up to 75 lbs with help Ability to assist Villagers into and out of vehicles safely Key Responsibilities Transportation & Medical Appointment Support (Primary Focus) Provide safe, timely, and compassionate non-emergency medical transportation (NEMT) for Villagers to off-campus doctor visits, therapies, labs, hospital outpatient appointments, pharmacy runs, etc. Use accessible practices (ramps, lifts, securement) as needed, ensuring that each Villager's mobility needs are addressed and accommodated Offer emotional support, reassurance, and assistance to Villagers before, during, and after appointments to reduce stress and promote positive experiences Advocate on behalf of Villagers-help them communicate needs or concerns to medical staff, assist with check-ins, accompany them as needed Maintain detailed trip logs, mileage records, and required documentation of each outing Monitor vehicle condition: ensure cleanliness, appropriate fuel levels, safety checks, and report any mechanical or safety concerns Plan routes to optimize time and safety (considering traffic, Villager comfort breaks, accessibility) Coordinate pick-up and drop-off times with the care team to minimize waiting and overlap Follow all vehicle, traffic, and safety regulations; secure mobility devices, seat belts, restraints appropriately Support Role in Care (Secondary, When Off-Transport Duty) When not transporting, assist as needed with personal care tasks such as grooming, toileting, dressing, positioning-always in a supportive role and in a way that honors Villager choice Be available to help fill in gaps in care, offer respite support, or respond to situations during non-transport periods Communication & Documentation Observe Villagers' physical, emotional, or behavioral changes during transport or in supportive moments; report concerns promptly to nursing or administration Document transportation interactions, appointments attended, outcomes, challenges, and any follow-up needs clearly and promptly Communicate effectively with the care team (ALC and DSH) so everyone knows appointment outcomes, delays, or Villager feedback Safety, Infection Control & Environment Adhere strictly to Universal Precautions, infection control protocols, and safety procedures, especially when assisting Villagers in vehicles Use proper body mechanics when assisting with transfers into/out of vehicles Maintain hygienic and orderly Villager spaces and vehicles; report repair or cleaning needs Ensure vehicles are safe and properly clean; monitor maintenance, schedule repairs as needed Villager Rights, Dignity & Relationship Building Treat every Villager with kindness, respect, and patience; remain attuned to their emotions and preferences Protect Villager privacy and choices; never rush or force Promote Villager autonomy-listen to preferences about timing, seating, reinforcement, and comfort Build trust and a reassuring presence, especially during transport when Villagers may feel anxious Other Duties Assist with admissions, transfers, and discharges as directed (transport or supportive role) Perform additional tasks when needed, always keeping Villager safety, dignity, and comfort in mind Physical & Operational Requirements Frequent use of hands, arms, walking, bending, reaching Ability to sit, stand, walk, climb, kneel, balance, and reach in tight vehicle spaces Must be able to lift 25 lbs independently and assist with up to 75 lbs Must have good vision, hearing, depth perception, and ability to drive Must be able to perform CPR / First Aid if needed Must safely assist Villagers into/out of vehicles, using mobility aids, lifts, or ramps Ability to safely operate and monitor transportation vehicle systems Full time 8:00am - 4:30pm Monday - Friday

Assist residents with activities of daily living such as grooming, dressing, and continence program as needed Provide companionship and emotional support to enhance residents' quality of life. Help coordinate activities that promote social engagement among residents. Observe and report any changes in residents' conditions to the healthcare team. Support residents with mobility needs, including lifting or transferring as necessary. Requirements Experience in caregiving or personal assistance is preferred but not required Strong interpersonal skills with a compassionate approach to senior care. Ability to observe and monitor resident conditions effectively. Knowledge of dementia care practices is a plus. Basic understanding of vital sign monitoring is beneficial but not required.
