Primary function:
The Home Health Assistant (HHA) is a member of the Community Health Service team working under the supervision of a registered nurse and/or therapist responsible for the client. The caregiver provides services to clients as established in the client's Plan of Care. (The Home Health Assistant's/Homemaker's duties may be adjusted at the discretion of the Professional nurse and/or therapist based on an evaluation of the patient's needs.) The HHA reports to the Client Service Supervisor.
Principal duties and responsibilities:
Follows Plan of Care developed by RN/Therapist.
Adheres to infection control policies and HIPPA guidelines.
Follows responsibilities outlined in the Plan of Care such as: personal care, light housekeeping, meal preparation, accompany patient to physician appointments or clinics.
Assists with simple exercise programs, application of prosthesis and braces, ambulation, and transfers as outlined.
Provides stimulation and encouragement towards independence and achieving the goals established by the professional staff and the patient's stated goal.
Other patient care skills as indicated on additional skills checklist.
Education and experience requirements:
Completion of CNA or Home Health Aide training program of 75 hours or greater.
If there has been a 24 month or greater gap in work as an aide, then there must be proof of a training competency evaluation program or attendance and completion of a renewal course.

This position will be responsible for administering up to 3 test days a week (an average of 8 hours/ test day) at various locations within a 30-mile radius of the employee's zip code. Examinations are performed mainly Monday through Friday with some Saturday and occasional Sunday examinations.
Exams are administered within a 30-45 mile radius of the employee's zip code and Prometric reimburses for mileage beyond 30 mile radius.
QUALIFICATION REQUIREMENTS:
EDUCATION
Registered Nurses with active and unrestricted license in the state of operation
BA/BS Degree preferred
EXPERIENCE
One (1) year of experience in long term care or experience caring for the chronically ill of any age

SKILLS

The goal of this unit is to provide high quality, cost effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set.
Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPap respirators, Oxygen delivery methods, PCA & epidural pumps, wound vacutainers, ventilators and specialty beds. Position Overview:
The Certified Nursing Assistant is responsible for providing personal medical care and assistance with daily living activities that improve quality of life by allowing our patients to remain in their home (or facility) while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions:
Perform, assist, evaluate and document medical care, such as:
* Basic first aid
* Taking weight and vital signs
* Catheter and bowel care, including I&O monitoring
* Range of motion exercises, exercises as assigned by a Physical Therapist, Occupational Therapist, or Speech Language Pathologist
* Ambulation, transfer and positioning with wheelchairs and other non-life support medical equipment and devices
* Prepare meals according to dietary and nutritional needs
* Care for patient's special conditions and needs within scope of authorized tasks
Help patient maintain good personal hygiene, such as:
Assistance with bathing, toileting, grooming, shaving, skin and nail care, oral hygiene, dressing and eating
Thorough completion and coordination of all required documentation, such as:
Maintaining and completing the necessary patient/family and Company documentation records upon conclusion of each visit
Observe and report significant changes in patient's condition to the (RN) Nursing Supervisor
Provide hands-on personal care and performance of simple procedures as an extension of therapy or nursing services.
Provide services that are ordered by the physician in the plan of care and that the aide is permitted to perform under state law.
Requirements:

Ongoing Education:

Observes, reports and documents symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, dietary and participation in activities.
Responds to security system and resident call bells promptly.
Notifies supervisor and/or Health Care Coordinator if a resident has increased care needs.
Assists with continence management and disposes of all continence products properly to ensure sanitation of resident suite and community restrooms.
Serves and collects food trays and assists as needed; reports refusal of meals or loss of appetite.
Maintains cleanliness of resident's room and work areas.
