Position Summary
Patient Care Associates (PCAs) provide personalized in-home assistance to individuals with developmental disabilities who require help with personal care and activities of daily living (e.g., feeding, bathing, dressing, grooming, toileting, and ambulation). PCAs also provide help with tasks such as preparing meals, doing light housekeeping, and laundry. PCAs provide transportation to client responsibilities such as work, community events, doctor's appointments, and grocery shopping. Work is performed in various settings depending on the needs of the care recipient and may include, but not limited to locations such as their home, place of work, out in the community, or at other nonresidential facility.

Responsibilities
Assist clients with completion of daily tasks, such as housekeeping, dishwashing, personal hygiene, cooking, eating, and dressing and other activities of daily life
Aide clients with physical demands such as ambulation, getting in and out of bed with or without the use of a lift, using the restroom, and boarding vehicles
Work with clients on developmental and experiential goals such as skill building, social interactions, community engagements, and recreational activities
Provide transportation to client engagements
Shop and meal plan with clients to encourage nutritious dietary habits
Provide emotional and psychological support by deploying active listening
Foster psychological safety by listening, building rapport, providing companionship, and following through with promises
Work to carry out the established care/service plan on a consistent basis with the objective of encouraging the client to realize his or her full potential
Actively engage with client during shifts with commitment to process and developmental improvement
Utilize Electronic Visit Verification to check in and out of each shift
Complete documentation to report activities, details, or noteworthy events of a shift
Maintain client safety while accomplishing daily living tasks and assisting the client in his or her personal and professional engagements
Effectively document Unusual Incidents or Major Unusual Incidents that may have occurred during a shift
Communicate all relevant client information to supervisors and the entire care team to foster an environment of compassion, safety, and teamwork
Train and guide the work of other PCAs, sharing tips for best practices as well as acting as a mentor to peers
Consistently evaluate delivered care to ensure focus on achieving independence and realization of full potential
Maintain CPR and First Aid Certifications and all compliance documentation, as well as completion of annual DODD Trainings

What You'll Do
Administer medications, as prescribed, and under the direction of a licensed nurse
Promptly report to the nurse, a resident's request/need for PRN medications.

May also be required to daily act in capacity of Rehab Tech to facilitate provision of appropriate levels of rehabilitative therapies.
Duties and Responsibilities

• Record all restorative and/or functional maintenance program documentation entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
• Completes daily, weekly and monthly documentation in accordance with restorative program and/or functional maintenance program guidelines with oversight by Clinical Supervisor (Rehab Director / Facility Restorative Nurse).
• Work closely with therapists / Facility Restorative Nurse to establish restorative nursing goals and interventions for the resident's care plan.
• Follow established safety precautions in the performance of all duties.
• Under the direction of the Rehab Director / Facility Restorative Nurse, optimize abilities and skills of patients in facilities:
• Transport and move patients as directed.
• Assist licensed personnel in patient care.
• Assist with maintaining cleanliness of therapy equipment and area.
• Maintain awareness of change in patient population, programs, and patient conditions and inform licensed personnel.
• Establish and maintain strong relationships with professional staff and
good public relations with patients and families.
• Maintain confidentiality of company, patient and client information.
• Adhere to the appropriate dress code for assigned facility and work
• environment.
• Adhere to the requirements of the patient's Bill of Rights.
Administrative Skills:
• Complete appropriate forms and records, as directed, make copies,
and obtain necessary signatures.
• Ensure that written and oral communications are directed to
appropriate personnel.
• Follow established protected health information (HIPPA) policies to
prevent privacy or security breaches. If breach identified, report to Broad River Rehab Management.
