As a Home Health Aide, you will be:
Responsible for providing direct patient care and assistance in accordance with the plan of care

Duties:

Provides for the special needs of the residents which may include caring for Alzheimer's Disease, developmental disabilities, residents in pain, and dying residents.
Measures, reports, and records weights and vital signs which include temperatures, pulse, respirations (TPRs), blood pressures, intake and output, and pain.
Provides personal hygiene to residents such as bathing, toileting incontinent residents offering bedpans/urinals, providing oral hygiene and grooming.
Provides for the individual nutritional needs of the residents, i.e., hydration, feeding, nourishment, and supplements.
Utilizes safety techniques in care of residents including proper body mechanics, locking wheelchairs and beds, cleaning spills, and correctly utilizing equipment.
Responds to emergencies according to their scope of practice, e.g., falls, seizures, and choking episodes.
Utilizes and adheres to infection control procedures and utilizes standard precautions.
Reports all complaints and grievances made by residents or families to the RN Resident Care Coordinator.
Knowledge of:

PERSONAL CARE ASSISTANT/CNA
Salary: $18 - $24 an hour, based on years of relevant experience.
The Personal Care Assistant is responsible for providing and assisting Participants with direct and indirect care needs, including functional and instrumental activities of daily living in the community. The Personal Care Assistant will perform care for Participants primarily within their residence and occasionally within the LIFE Pittsburgh Adult Day Health Centers. Will provide direct care services, as approved and identified on the Participants Plan of Care to include, but not limited to the following ADLs: bathing, dressing, toileting, eating, mobility, van assist and medication prompting. Will also provide direct care services, as approved and identified on the Participants Plan of Care to include, but not limited to the following IADLs: light housekeeping, meal preparation, laundry, grocery shopping and companionship. The Personal Care Assistant will provides approved basic nursing care services to include, but not limited to, Foley catheter care, applying assistive devices, and assisting with colostomy bags consistent with policies and procedures. And is also responsible for completing and submitting accurate and timely documentation including Flow Sheets, Participant Logs, Communication books, etc. within the appropriate timeframe.
LOCATION/TRAVEL REQUIREMENTS:
Positions are based out of an Adult Day Health Center, which are located in North Shore, Allegheny Center, Greentree, and McKees Rocks. Services areas vary based on location.
Travel to multiple Participants' residences throughout a shift is required.

What will I do as an employee with Redstone?

* JOB SUMMARY
* Enhances the effectiveness of the clinical team by collecting patient data, responding to resident/patient requests, providing personal care, and maintaining the clinical environment.
* ESSENTIAL FUNCTIONS
* PROVIDES PATIENT CARE
* By prioritizing, planning and completing patient care as assigned
* attending to personal care needs
* assisting with patient transfers and ambulation
* obtaining accurate vital signs and other clinical parameters and and specimens including: blood sugar readings, EKG's and drawn blood samples
* responding to patient requests
* requesting RN or LPN supervision with new or unfamiliar tasks.
* Reviews Policy and Procedures prior to performing tasks that are unfamiliar.
* ESTABLISHES A COMPASSIONATE ENVIRONMENT
* By providing emotional, psychological and spiritual support to patients and families and other associates.
* PROMOTES CUSTOMER SERVICE
* By greeting patients, visitors, and associates in person and on the telephone
* answering questions within the scope of practice
* relaying calls to patients
* giving directions
* and referring inquires to clinical staff and physicians.
* COMMUNICATES INFORMATION
* By collecting and documenting in the patient record
* providing clinical reports at regular intervals to fellow nursing assistants and nurses
* notifying RNs of significant findings or change in patient condition.

* By recording information in the chart as soon as possible after collection
* recording intake/output totals at the end of shift or specified times

* MAINTAINS PATIENT CONFIDENCE AND ORGANIZATIONAL CONFIDENTIALITY

* responding to patient requests using Tact and diplomacy.
* COLLECTS RESIDENT/PATIENT INFORMATION
* By operating patient care equipment, including blood glucose monitor, blood pressure cuffs, lifts and scales according to policy and procedure

* PROVIDES A SAFE AND CLEAN WORKING ENVIRONMENT

Position Overview
Position responsible for performing advanced indirect and selected direct patient care duties in support of the nursing process in a competent manner under the direction of an RN and for performing related clerical functions, must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles.
Essential Job Functions
Must be able to demonstrate knowledge and skills necessary to provide care to the patient served
Delivers appropriate advanced indirect and selected direct patient care at a competent level as directed by a physician or registered nurse within the scope of hospital-approved PCA skills inventory according to the needs/age of the patient/population served; collaborates with other members of the healthcare team to plan, expedite and promote quality patient care; employs critical thinking skills when identifying and responding to abnormal findings, reporting such to the nurse immediately; maintains simple drainage devices; takes patient vital signs, perform blood glucose checks, perform EKGs, monitor intake and output, and maintain related records; perform skin preps and prepares patient for procedures as directed
Answers call bells, prepares patients for meals, prepares meals and feeds patients; performs/assists with baths; assists patient in mobility; transports patients within the facility and performs assigned treatments, as directed; maintains equipment and patient rooms in orderly fashion; performs housekeeping duties for patient rooms, performs assigned data entry via computer; serves as Unit Clerk and/or monitors patients as assigned
Ensures safe care to patients adhering to policies/procedures/standards of care, within budgetary specifications, including time management, supply management, and accuracy of practice
Other Job Functions
Promotes individual professional growth/development by meeting requirements for mandatory all-staff meetings/continuing education/skills competency; supports department goals which contribute to the success of the organization; serves as preceptor/mentor/resource to less experienced staff
Basic Qualifications
High School Diploma or GED, required
Certified Nursing Assistant, preferred
Current BLS certification, required
2 years of work experience in an acute care hospital as a Nursing Assistant or currently enrolled in an accredited nursing program, required (this qualification is preferred for hires in Oklahoma)
Ability to understand written instructions and perform basic mathematical calculations; demonstrates skills for assigned specialty area/function or assigned procedures
Demonstrated ability to perform duties with increased responsibilities.
Ability to perform duties requiring prolonged standing, walking, stooping, bending, and lifting
Basic knowledge and understanding of PPE protocols and universal precautions
Position requires fluency in English; written and verbal communication
Pennsylvania Candidates: Act 33 (Child Abuse History Clearance), Act 73 (FBI Fingerprint Criminal History Clearance) completed within the last 5 years, or must be obtained prior to start date.
