SUMMARY: Under close supervision, the AHD Certified Nursing Assistant (CNA) performs a variety of CNA tasks relating to the care of patients and residents in a hospital, skilled nursing facility or clinic. When assigned as a sitter, responsible for but not limited to protecting the patient(s)/resident(s) from harm and assisting in meeting their emotional and physical needs through one-to-one continuous observation. Provide on-going observation and basic patient care. When assigned to an outpatient setting, prepares examination room, ensuring that it is stocked with the necessary supplies, equipment and instruments for the type of clinic to be held; weighs, takes and documents patients' vital signs; assists physicians or mid-level practitioners during the examinations as directed; labels and delivers specimens to the lab; assists in the maintenance of a clean and well supplied clinical area; stocks shelves and monitors expiration of medical supplies. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services. Role description This recruitment is for multiple positions at all DPH facilities, including Laguna Honda Hospital and Rehabilitation Center and Zuckerberg San Francisco General Hospital and Trauma Center. Under direct supervision of a licensed nurse, the Certified Nursing Assistant (CNA) performs a variety of routine duties involving care of medically or mentally ill acute or sub-acute patients; performs direct patient care; observes and reports changes in the behavior and physical status of patients; provides a clean and safe environment; and performs related duties. Documents care according to department standards. * Maintains constant and continuous communication with medical providers and staff. * As directed by licensed staff; reviews and implements treatment plans, and provides one-to-one care when essential to treatment plans. * Observes, records and reports patient medical, social and psychiatric behavior and physical conditions. * Participates in multidisciplinary team meetings and treatment planning by reporting observations and concerns and by asking questions. * Attends and participates in patient groups, case conferences, staff meetings and in-service training programs. * As assigned, completes procedures and specimen collection including phlebotomy. * Adheres to health, safety, and infection control policy and resource utilization. * Manages work time effectively, efficiently and productively.
