PERFORMANCE RESPONSIBILITIES AND STANDARDS
Under the supervision of the Program House Manager, the CNA/HCA
Provides support to program participants that meets their needs, provides opportunities for community integration, including swimming, and creates a nurturing, safe, and healthful living environment.
Must be familiar with the Residential Service Guidelines issued by the Developmental Disabilities Administration.
Provides each participant with an array of instruction and support services, as described in the participants Individual Instruction and Support Plan (IISP).
Ensures that the daily needs of the participants are met, including personal hygiene, food preparation, house cleaning, shopping, and monitoring of health needs, including supervision of medication.
Assists participants with regularly accessing the community to take part in a variety of outings of the participants choosing. This may require transporting participants or arranging for transportation.
Ensures that participants are always treated with dignity and consideration. Promotes and safeguards each participants ability to make decisions and to freely exercise his or her rights.
Interacts with participants, provides informal counseling and problem-solving with participants.
Attends staff meetings as required by the House Manager or the Site Supervisor and/or Administrator.
Attends training offered/required by the agency, DDA, and other parties.
Takes part in the development and implementation of the IISP for each participant with whom they work.
Will maintain CNA at employees own expense initially and annually.
Must enroll and complete Nurse Delegation study course and take the test within 30 days of employment.
Provides necessary support and supervision of participants.
Maintains professional working relationships with other services providers, the participants parents/relatives/guardians and other agency staff. Responsible for timely completion of all documentation, including, but not limited to, the medication log, shift notes, progress notes, data sheets, incident reports, and client financial records.
Performs other duties as assigned by the House Manager or the Site Supervisor and/or the Administrator.
Must have a valid drivers license and car and must be willing to transport clients in their own vehicle.

Completion of a state-approved certified nursing program and/or successful passing of a competency evaluation program which meets state regulations.
Meets state specific certification requirements and possesses unrestricted certification as a CNA.
At least twelve months of work experience as a CNA in a healthcare setting.
Current CPR as required by client facility; other health and screening tests as required by specific facilities and/or regulatory agencies.
ENVIRONMENTAL WORKING CONDITIONS:

Home Care Aides with their certified nursing assistants (CNA) license provide care and personal services either in-home (client) or on campus (resident). These services often include companionship, personal care, housekeeping, and meal support according to a written Plan of Care. Home Care Aides approach all clients and residents with compassion and a service commitment to fulfilling their needs as they age in home.
