Position Qualifications
Meets the training requirements of the State.
Effective August 14, 1990, a person who has successfully completed a state established or other training program that meets the requirements of S 484.36(a) and a competency evalu ation program, or state licensure program that meets the requirements of S 484.36(b), or a competency evaluation program or state licensure program that meets the requirements of S 484.36(b).
Shows proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau, preferred.
At least 18 years of age.
Ability to read and follow written instructions and document care given.
Understands hospice philosophy, comfortable providing specialized care to the terminally ill.
Satisfactory references from previous employers.
Is self-directing with the ability to work with little direct supervision. Secure with issues of death/dying. Provides a calm manner when in a patient's home.
Has empathy for the needs of the ill, injured, frail and the impaired.
Possess and maintains current CPR certification.
Demonstrates tact, patience and good personal hygiene.
Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

Report any medication found in the patient's room and takes found medication to the charge nurse as directed.
Keep supply room and work areas clean at all times. Washes hands after caring for each patient.
Follow infection control procedures, as established by this facility, when isolation techniques are necessary.
Report all accident/incident, regardless of how minor, and all hazardous conditions and equipment to the charge nurse immediately. Attend and participates in -service educational programs, on-the-job training programs, etc., as directed.
Ensure that patient's rights to fair and equitable treatment, self determination, individuality, privacy, property and civil rights, including the right to wage a complaint, are well established and maintained at all times.

Position Overview
The Certified Nursing Assistant (CNA) provides assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) for patients residing in skilled nursing or RCFE communities and at home. As a representative of Care Indeed, the CNA is expected to demonstrate professionalism, flexibility, and respect, acting as a brand ambassador in all interactions. The CNA must be willing and able to care for a diverse range of clients during assigned shifts.
Essential Duties And Responsibilities
DUTIES
Provide personal care and support to clients in accordance with the service plan developed by the agency, instead of using the plan of care
Support Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) while promoting client independence to the extent tolerated
Maintain professional conduct and appearance as a representative of Care Indeed in skilled nursing facilities or RCFE communities
Accept assignments as scheduled during designated shifts
Participate in regular communication with the Nurse Staffing Specialist regarding scheduling, patient needs, and performance
Maintain patient confidentiality and uphold ethical standards in all care interactions (HICC)
Demonstrate flexibility and responsiveness to agency and patient needs
RESPONSIBILITIES
Note: Meal prep, grocery shopping, and light housekeeping duties apply only to home-based assignments; not applicable for CNAs placed in facilities where floor staffing is required.
Assist with:
Personal hygiene (bathing, grooming, oral care)
Dressing and undressing
Toileting and continence care
Skin care and repositioning
Bed mobility and ambulation assistance
Feeding and nutritional support
Meal preparation and grocery shopping as assigned
Light housekeeping (laundry, linen changes, cleaning patient areas)
Encourage client engagement and promote independence
Provide emotional and psychological support to clients and their families
Observe and report changes in the client's physical or mental condition
Maintain accurate, timely documentation of services provided
Follow safety practices, including universal precautions and infection control protocols
Coordinate with the manager or supervisor involved in the client's care
Attend required in-service trainings and maintain active CPR and other certifications

Job Conditions
The ability to drive and the ability to access clients' homes, which may not be routinely wheelchair accessible, are required.
Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
On occasion, may be required to bend, stoop, reach, and move client weight up to 250 pounds; lift and/or carry up to 50 pounds.
Must be able to communicate clearly, both verbally and in writing, in English.
Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull.

High school diploma or equivalent
Must have completed a minimum of 75 hours of training, which includes an introduction to personal care services
If a Nursing Assistant, certified by the state (CNA), the individual will meet the qualifications for a Home Health Aide, as his or her hours of training will exceed the minimum required.
Must understand and respect clients, including ethics and confidentiality of care
Must have current CPR certification. Online certification is not accepted.
Must have a criminal background check
Must be free from health problems that may be injurious to the patient, self, and co-workers, and must present appropriate evidence to substantiate this
