POSITION SUMMARY
The Resident Assistant is responsible for providing daily care to community residents. The Resident Assistant functions as the primary liaison between residents and community staff. ESSENTIAL FUNCTIONS
The Resident Assistant reports to the Community RN, Executive Director and Asst. Executive Director. The Resident Assistant is responsible for carrying out the primary duties as outlined by the Executive Director. These essential functions include, but are not limited to:
§ Demonstrate positive interpersonal relations in dealing with employees, residents, residents' families, visitors, and coworkers in a professional and confidential manner.

DUTIES
The Resident Assistant is expected to perform position-related duties that include, but are not limited to:
Dietary
§ Encourage residents to safely walk appropriate distances to meal times to preserve their ability to ambulate.
§ Remind and escort residents to appropriate meals times when needed.
§ Transfer residents from wheelchair/walker to dining room tables and ensure proper storage of assisted devices during mealtimes so the dining room is safe from fall hazards.
§ Observe the food being served and give input according to resident's needs.
§ Visit with residents while they are eating and help clear dishes.
§ When advised by Administration to do so, provide residents with feeding assistance.
Residents
§ Assist residents with activities of daily living (ADL's) ADL's include, but are not limited to, grooming, care and selection of clothing, cleanliness of clothing, shaving, bathing, oral hygiene, shampooing and curling hair, eating and sleeping, planning daily activities, and spending quality time with individual residents.
§ Assist residents that may be incontinent of bowel and/or bladder, including making sure residents are clean and dressed in clean clothing.
§ Assist residents to other daily meals, activities, and in preparing for family outings or appointments.
§ Supervise residents in medication self-administration according to the Medication policy. Coordinate with the nurse on side effects or concerns.
§ Assist with maintenance of resident's records to include charting medications, recording daily intake, etc.
§ Record vital signs, weights and measurements, and other information as assigned.
§ Facilitate staff, resident, and family communication to resolve conflict and create a positive home environment.
§ Ambulate and transfer residents, utilizing appropriate assistive devises and body mechanics.
§ Participate in resident care planning by identifying the needs of residents in accordance with the medical assessment, working with other professionals as necessary.
§ Observe and report immediately any unsafe or unsanitary conditions in resident apartments.
§ Assist with admissions by greeting and helping with the orientation process to the facility.
§ Do resident checks as outlined by the Administrator based on resident's individualized service plan.
§ Communicate any concerns regarding resident health or behavior to the Executive Director or Asst. Executive Director so that it may be considered for documentation in the Significant Change Log.
Regulatory and Legal
§ Comply with community's policies and procedures and recommend changes as necessary.
§ Bring to the Administration's attention to any deficiencies that may arise.
§ Ensure compliance to policies relating to blood-borne pathogens, hazard communication, and infection control.
§ Ensure compliance with respect to risk management; the dying and the aging process; fire safety; disaster preparedness; residents' rights; grievance procedures; and medication administration.
Other
§ Develop skills and education through other higher educational and/or continuing education opportunities. Annual continuing education requirements shall be determined by State regulations or as required by the Executive Director.
§ Complete full orientation upon hire.
§ Attend staff meetings as required.
§ Participate in patient care planning conferences.
§ Set a good example to all employees of punctuality, dependability, productivity, use of time (no cell phone/texting, social media, or computer use). Display genuine resident care, positive and happy attitude, work ethic, communication skills, confidentiality, and trust.

Position Summary
Under the supervision of the Division Director of Intensive Behavioral Services, the Program/Medical Assistant supports medical and medication operations in collaboration with the medical provider, nurse, and treatment team. This role coordinates initial primary care appointments upon admission and partners with internal programs and external providers for ongoing care coordination. The Program/Medical Assistant manages triage calls and communication with clients and community providers to support smooth transitions from the program, and transports clients to appointments when needed. Establish primary care appointments for newly admitted clients and provide updates on medical needs and progress during treatment team meetings.

Pick up newly prescribed medications as needed, accurately document medication changes in the shift log, and promptly communicate any discrepancies, concerns, or client-related issues to the medical provider.
Coordinate, track, and maintain follow-up medical appointments on the client appointment calendar with internal and external providers, ensuring clients are informed and prepared.
Communicate with VOA programs, clinics, hospitals, medical/dental providers, and pharmacies to coordinate medications, records, referrals, and continuity of care.
Triage medical situations with medical and treatment team raised by clients and staff
Maintain proper patient medication storage.
Maintain facility emergency kits with adequate medical supplies.
Audit medication logs, storage areas to ensure only current client medications are being stored and report to RN/Program Manager any issues.
Work with staff manager bi-weekly to dispose of medications left behind or that need to be discarded according to VOA policy.
Record keeping. Ensure all paperwork related to client medications is completed properly, managed confidentially, and stored in the appropriate location.
Maintain a spreadsheet of clients connected with Librium or other MAT.
Ensure daily NARC counts are completed and reported at each shift change. Report discrepancies to Program Manager/RN.
Ensure an incident report on all emergency or critical incidents is complete by the end of shift.
Attend work as scheduled and arrive at the designated time.
Work must be performed on-site due to the need to provide direct client care.

Performs & documents laboratory quality control to include actions taken for out-of-range or unexpected values
Complies with VFC eligibility guidelines
Adheres to Universal Precautions and Biohazard waste guidelines
Assures rooms are stocked with medical supplies for each visit. Turns in weekly inventory to center manager
Understands & Adheres to Standing Orders
Uses USIIS in efforts to improve Immunization rates
Understands and follows ACIP Immunization guidelines
Uses two patient identifiers during any patient interaction
Accurately completes MA visit Encounters with all pertinent billing information entered correctly
Documentation of patient visits meets MTCHC charting protocols (CC, Social, family history, medical history, surgical history, hospitalizations, etc.)
Reviews current medications during each office visit
Tracks referrals/Diagnostic imaging to ensure timely return of results/notes
Tracks lab results to ensure return in a timely manner
Sends out labs accurately by attaching ICD-9/ ICD-10 codes and pertinent billing information. Processes specimens correctly
Correctly scans and attaches documents into ECW, attaching orders appropriately
Obtains and documents vitals accurately and completely
Responds to Telephone Encounters in a timely manner. Documentation is clear and complete
Other Duties as assigned
Minimum Qualifications:
Completion of medical assistant program OR may substitute education for 1-year minimum experience in similar setting OR may substitute if CNA certification is up to date OR phlebotomist certification is up to date.
Previous experience providing care to medically underserved patients preferred.
Required license- None
Other Skills:
Ability to effectively communicate verbally and written in English and Spanish. Demonstrates ability to relate well with others in a team based environment, works quickly and accurately under pressure, and has the ability to learn computer skills. Proficient in MS Outlook, Word, and Excel.
