Performance Responsibilities: May included, but not limited to: Give report to licensed nurse/senior personal care attendant at the end of shift. Reminding residents to take medication, opening containers, opening pre-packaged medications, reading the medication labels, observing residents while taking medications, checking the self-administered dosage against the label of the container and reassuring residents that they have obtained and are taking the dosage as prescribed. Supervising or giving showers and baths. Supervise dressing or assisting residents to dress. Assuring cleanliness of resident. Assisting with setting up food and feeding residents. Supervise or provide oral hygiene. Supervise or provide hair care and shampoo. Supervise or shave residents. Supervise or toilet residents and assist with incontinence supplies when appropriate. Walk residents to meals and/or activities. Supervise or transfer residents when necessary. Reports unusual conditions, observations and pertinent information to the nurse. Fosters and maintains effective working relationships between peers, professional staff and other personnel within the community. Supervise or assist with meal preparation, light housekeeping, clothes laundering, Shopping for food or other items, and the use of transportation. Determine when nurse needs to be contracted and call her. Demonstrate a courteous, polite, and friendly attitude with residents, families, and visitors, staff and co-workers. Respond to the needs of residents, families, and visitors by providing immediate assistance. Assist in maintaining contact with residents who are not involved in activities due to illness or injury. Record resident vital signs, weights and measurements, intake and output, food consumption, and other information as assigned. Routinely turn and position residents as necessary. Observe residents carefully and report changes in condition to Charge Nurse. Recognize a residents ability to make choices and support these within the constraints of the caregiving environment. Participate in collecting data needed for the accurate completion of the MDS and the plan of care. Provide input to the nurses for development of the plan of care.     Broad Function: Supervise or assist residents with personal care services, which include Self-Administered Medication Management, Activities of Daily Living and Instrumental Activities of Daily Living as identified in the Resident Service Plan and assigned by the Charge Nurse. Supervision includes reminding and/or observing residents while they perform activities. Assistance is either through physical assistance or supervision. Performance Responsibilities: May included, but not limited to: Give report to licensed nurse/senior personal care attendant at the end of shift. Reports any changes to licensed nurse/senior aide immediately for determination of calling on-call nurse. Reminding residents to take medication, opening containers, opening pre-packaged medications, reading the medication labels, observing residents while taking medications, checking the self-administered dosage against the label of the container and reassuring residents that they have obtained and are taking the dosage as prescribed. Supervising or giving showers and baths. Supervise dressing or assisting residents to dress. Assuring cleanliness of resident. Assisting with setting up food and feeding residents. Supervise or provide oral hygiene. Supervise or provide hair care and shampoo. Supervise or shave residents. Supervise or toilet residents and assist with incontinence supplies when appropriate. Walk residents to meals and/or activities. Supervise or transfer residents when necessary. Reports unusual conditions, observations and pertinent information to the nurse. Know the community's disaster and fire safety policies and procedures, including the location and correct use of the fire alarm system. Follow the designated plan of action in the event of a fire or other emergency. Must be able to assist in evacuation of residents. Must be able to lift, push, pull, and move equipment, supplies, etc., in excess of fifty (50) lbs.
