Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water, and within the resident's reach. Pass the water pitcher as per established policy and procedure.
Dress residents neatly and in their own clothing. Assist residents in dressing as necessary.
Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.).
Serve nourishment in accordance with established facility procedures.
Feed residents who cannot feed themselves.
Assure that resident's food is accessible and self-help devices are available as needed.
Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities.
Keep incontinent residents as clean and dry as possible at all times. Change bed linens, diapers, and clothing as often as necessary.
Do not permit residents to smoke in bed.
Assure that the nurse's call system is attached to the bed and within easy reach at all times when residents are bedfast.
Answer residents' call lights promptly.
Check for food in the residents' room. Ensure that food is kept in a sealed container.

CLINICAL
Wash hands before and after caring for each resident.
Place residents in the correct and comfortable position. The head of the bed is elevated at 30° for all residents on enteral feeding.
Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner.
Record the resident's food and nourishment intake as directed.
Perform after-meal care as required - cleaning the resident's hands, face, clothing, dentures, etc.
Assist residents to/from the bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed).
Check and document bowel movements and character of stools every shift.
Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities).
Comb and brush the resident's hair daily. Shampoo your hair on shower days.
Clip and trim fingernails.
Shave male residents daily or as needed.
Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed.
Observe and report the presence of reddened areas on bony prominences.
Keep the drainage bag off the floor when a resident is in bed or in a wheelchair.
Apply Vaseline or A & D ointment to dry skin in accordance with established policies.
Move and turn bedfast residents at least every two hours and record such information on CNA notes.
Observe disoriented and comatose residents and notify the Charge Nurse immediately.

ESSENTIAL FUNCTIONS:
1. Provides basic care as directed to include bathing, feeding, assistance with feedings, assistance with activities, and hygiene.
2. Demonstrates proficiency in managing resources, following policies and procedures, safely utilizing equipment. Actively contributes to cost controls by managing time, resources, and supplies effectively.
3. Documentation in the Patient Record will be legible at all times. Maintains accurate documentation as appropriate including graphic and I&O sheets.
4. Maintains positive and caring interpersonal relationships with patients, families, and fellow employees
5. Answers call lights and responds to patient requests promptly. Demonstrates competence in taking vital signs, measuring I&O, etc.
6. Ensures that utility room, kitchen, pantries, supply rooms, and nurses lounge are maintained in a neat and orderly fashion.
7. Attendance at CPR Classes and orientation is mandatory. Attends 75%, a minimum of six, unit staff meetings, and in-services.
8. Participates in QC/QI issues as required.
9. Keeps RN and LVN informed of changes in patient's conditions or patients' requests.
10. Completes assignments given by the team leader, cleaning, checking department equipment, etc.
OTHER RESPONSIBILITIES:
1. Able to adapt to changes in workload. Able to perform in a precise and accurate manner in handling priority tasks.
2. Care for the geriatric patient with knowledge and clinical skills of the aging process.
3. Care for the pediatric patient with knowledge and skills of the growth and development process.
4. Acts as patient advocate for patient rights by initiating and maintaining confidentiality, privacy, and safety for the patient.
5. Accepts constructive criticism and incorporates suggestions into practice.
6. Supports and promotes safety standards, identifies risk situations, and intervenes appropriately.
7. Knowledge and ability to apply ethical and legal principles to practice.
8. Current knowledge of Standard Precautions in dealing with potential exposure to Bloodborne Pathogens.
9. Organizes work and completes assignments promptly.
10. Functions within limits of state Nurse Practice Act, hospital and nursing department policies and procedures.
11. CNAs who provide care to Joint Replacement patients will receive 1 hour of ortho/joint replacement-specific education yearly.
12. Other duties as assigned from time to time.

EDUCATION, KNOWLEDGE, ABILITIES, EXPERIENCE & SKILLS:
1. RN and LVN students after successful completion of the first semester of clinical training.
2. Previous clinical experience in a patient care setting preferred, with successful completion of orientation and training to the point of competence.

Under the supervision of the Radiology Director/Manager and/or Lead Technologist the Nuclear Medicine Technologist will perform all Nuclear Medicine duties as well as other related duties as needed to facilitate workflow and promote health care. The Nuclear Medicine Technologists will operate Nuclear Medicine equipment in a safe and competent manner to produce quality diagnostic images as directed by physicians according to established standards and protocols.
ESSENTIAL FUNCTIONS:
Orders and maintains Nuclear Medicine supplies and radiopharmaceuticals. Prepares radiopharmaceuticals and administers to patients appropriately.
Reviews requisitions for completeness and pertinence. Schedules and performs Nuclear Medicine scans as ordered by the requesting physician(s). Confidentiality is maintained at all times.
Performs ongoing preventative maintenance and radiation surveys. Performs quality control procedures as outlined by the manufacturer. Documents and maintains records appropriately. Takes corrective action to remedy problems identified.
Maintains all records of receipt and disposal of radioactive materials as required by State and Federal regulations. Follows radiation safety guidelines and uses appropriate safety equipment and techniques. Follows Standard Precautions
Assists the Radiology Director/Manager in developing the department's quality improvement program and reporting methods. Wears radiation-monitoring devices, such as film badges and TLD rings in an appropriate manner as outlined in the Radiation Safety Manual. Submits badges and/or rings on a monthly basis. Provides education on proper radiation badge placement on appropriate staff and demonstrates radiation principles to staff as needed.
Participates in Radiation Safety Committees and acts as assistant to Radiation Safety Officer as requested. Maintains radiation badge documentations for Radiology and Surgical departments. Participates in other hospital committees as requested.
Maintains and demonstrates professional skills through self-development and continuing education.
Functions within guidelines and has knowledge of regulatory, licensing, and accrediting bodies; i.e., Title 17, Title 22, Conditions of Participation, DNV, etc.
Promotes positive patient relations in a caring environment by making patients comfortable and at ease during procedures
Transfers and positions patients using extreme care, patience and proper body mechanics.
Identifies patients using the hospital's double identification policy.
Responds to backup calls as needed within 30 minutes.
