Job Summary
Perform necessary coordination and delegation of clinical and managerial activities for senior housing clients as directed, in order to accomplish the objective of quality client care. As a Home Health Certified Nursing Assistant, here's a big-picture view of what you'll do:
Care for seniors and those with an illness, injury or chronic disease in their homes
Help with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake
Assist with ambulation, transfers and range of motion exercises
Check vitals, measure intake/output, monitor blood glucose and document any changes
Work with a team of home health nurses and aides to ensure a patient's care plan is followed
Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment
A few must-haves for Home Health Certified Nursing Assistants:
High school diploma (or equivalent) and active Nursing Assistant Certification in the state of Colorado
6 months of CNA or similar health care experience required
BLS/CPR certification (demonstration course required)
Compassionate nature, good communicator and ability to lift up to 50 lbs.
Pass a background check and a drug test which includes testing for THC

Job Summary
Perform necessary coordination and delegation of clinical and managerial activities for senior housing clients as directed, in order to accomplish the objective of quality client care. Responsible to organize, teach, and supervise the training curriculum as directed. Will be responsible for meeting compliance requirements for state regulations governing senior housing and the certification process. Responsible for directing and implementing total case management for all housing clients on a regular basis. Includes personal care management, medication administration, care planning, scheduling physician visits, setting up appointments, and working with clients, families, home health services, county and state offices to assure client needs are met. Responsible to oversee and conduct specified training for clinical staff to meet regulatory requirements on an ongoing basis. Responsible for coordinating with nursing leadership to identify needs for additional staff educational topics through the performance improvement process. General knowledge of instructional and communication skills, and group dynamics is required.
Qualifications
Bachelor's degree in nursing required.
Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
For Rural Health Network facilities and Good Samaritan Society (GSS) only, employees who do not possess a Bachelor's degree in the required field, leadership may consider an educational plan with proven continuous action toward achieving a Bachelor's degree in Nursing within five years of hire into position. Additionally for Rural Health Network facilities and Good Samaritan Society (GSS) locations, for current employees, leadership may have considered acceptable qualifications and work experience equivalency based on facility size, rural market, and business need, prior to new Sanford education requirements.
Thorough knowledge of the nursing practice act and standards of nursing care, and a working knowledge of administrative and management techniques.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Current Basic Life Support (BLS) certification required. Certified Nursing Assistance (CNA) instructor preferred.
