Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies procedures and training and in compliance with regulations set forth by the corporation state and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and CQI Activities including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMC Quality Enhancement Goals (QEP).
DUTIES / ACTIVITIES:
CUSTOMER SERVICE:
* Responsible for driving the FMS culture through values and customer service standards.
* Accountable for outstanding customer service to all external and internal customers.

* Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
* Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor.
* Perform vascular access evaluation pre-treatment and report unusual findings to nurse supervisor.
* Perform vascular access cannulation and report any difficulties in cannulation or unusual findings to nurse supervisor.
* Perform administration of Heparin as delegated or as allowed by state law.
* Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
* Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
* Monitor patients' response to dialysis therapy and report any unusual findings to nurse supervisor.
* Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
* Discontinue dialysis treatment according to established procedures
* Evaluate patient prior to termination of venous access - standing & sitting blood pressure.
* Obtain Hemostasis and apply appropriate dressings.
* Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor.
* Perform and record Pre and Post dialysis evaluation weight and vital signs with initial identification.
* Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
* Responsible for ensuring appropriate safety alarms are enabled including Narrow Venous Limits (NVL) 160.
* Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
* Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and if applicable progress notes.
* Report any significant information and/or change in patient condition directly to the nurse supervisor.
* Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the nurse supervisor.
* Perform and document any intervention for unusual patient status and document patients' response to intervention.
* Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures provide appropriate intervention as needed document and report any unusual findings to the nurse supervisor.
* Obtain all prescribed laboratory testing and prepare specimens for collection.
MAINTENANCE/TECHNICAL:
* Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
* Clean and disinfect dialysis machine surface chair equipment and surrounding area between treatments according to facility policy.
* Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
* Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
* Initiate Solution Delivery System (SDS) system.
* Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
* Prepare organize and efficiently utilize supplies and equipment to prevent wastage.
CLERICAL & ADMINISTRATIVE:
* Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
* Enters all treatment data into the designated clinical application accurately and in a timely manner.
* Review treatment sheets for completeness ensure nursing signatures are documented and ensure omitted entries are completed or corrected where appropriate.
* Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
* Prepare lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
* Collect label appropriately prepare and store lab samples according required laboratory specifications.
* Ensure collection of lab specimens by appropriate lab courier.
OTHER:

* Warm dialysate solution when directed by the HT RN
* Draw dialysate solution for testing as directed by the HT RN
* Creates and sends patient reminders (supply inventory for order Kt/V collection bringing meds to clinic visit for review)
* Assembles/breaks down charts/thins charts
* Maintains logs as directed and applicable
* Inventory of Home Program
* Maintains par levels of home department supplies
* Files home patient packing slips/invoices
* Inventory for Patients
* Assists patient with supply management and contacting customer service

EDUCATION

Summary of Duties of the Resident Care Assistant
The primary responsibility of the Resident Care Assistant (RCA) is to provide physical and psychosocial care to all residents in a manner that promotes dignity, choice, and individuality. The Resident Care Assistant reports to the shift supervisor as well as the Wellness Director.
Essential Functions of the Resident Care Assistant
Provides all necessary care and services to assigned residents and assists other wellness department associates as needed.
Works collaboratively with peers and other team members.
Provides assistance with activities of daily living such as with bathing, dressing, personal hygiene (includes: shaving, oral care, nail care, hair care, foot care), toileting, eating, and ambulation/mobility.
Maintains professional demeanor at all times when interacting with residents and families.
Reports any suspected or witnessed instances of verbal, mental, or physical abuse to licensed staff immediately.
Utilizes safe transfer techniques when assisting residents with mobility.
Washes residents' laundry as needed.
Understands and uses Centers for Disease Control and Prevention Standard Precautions, OSHA's Occupational Exposure to Bloodborne Pathogens standard, and follows established infection control procedures, hazardous communication, and other safety rules.
Documents provision of services on company forms.
Operates designated medical equipment, copy, scan, fax machines, and telephone.
Escorts residents and coordinates internal transportation needs.
Assists residents with personal correspondence/telephone use when requested.
Observes and reports changes in residents' physical condition and cognitive/emotional status to Wellness Director or supervisor, as needed.
Conducts room checks and resident rounds.
Monitors for environmental safety hazards.
Respond to emergencies in a prompt and calm manner; Immediately reports such emergencies to the supervisor.
Uses tactful, diplomatic communication techniques in potentially sensitive or emotionally charged situations.
Follows-up with appropriate staff, residents, or other individuals regarding reported complaints, problems, and concerns.
Non-Essential Functions of the Resident Care Assistant
Participates in and supports the resident-centered activities program.
Care for sensory enhancement devices such as eyeglasses and hearing aids.
Completes housekeeping tasks such as bed-making, linen changes, vacuuming, emptying trash, and cleaning bathroom.
Acts as ambassador and public relations representative to guests and other off-campus visitors.
Copies special paperwork or forms.
Assists in a variety of tasks related to dining (escort residents to dining room, assist with set-up and clean tables, serve meals, bus tables, etc.).
Demonstrates an appreciation of the heritage, values and wisdom of the residents, and an understanding of the aging process.
Collaborates with associates in other departments to assure necessary care and services are provided.

Knowledge, Skills, Abilities, and Experience
Ability to read, write, and speak in English.
Certification by state approved program, if required.

Be in good physical health and free of communicable diseases; Lifting, bending, and pushing medication cart about residence is required.
Must be able to lift 40 pounds.

Report all accidents / incidents you observe on the shift as per policy and procedure.
Resident Nursing Care Functions:
Provide resident care per the resident centered service plan.
Medication management for residents.
Greet residents and escort them to their room. Assist with activities of daily living.
Assist with Admission, Transfer and Discharge functions for the residents.
Participate and receive the shift exchange report upon reporting for duty.
Follow established policies concerning exposure to blood/body fluids.
Assist with activities of daily lives following resident centered services plan.
Maintain intake and output records as instructed and per resident centered services plan.
Answer resident calls promptly.
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and per resident centered services plan.
Identify activities that appeal to the resident and encourage socialization and participation in those activities.
Dietary Functions:
Assist residents in dining area taking into account any special dietary needs and personal preferences.
Handle and prepare foods and fluids in accordance with sanitary regulations.
Set up and clear dining tables. Clean dining rooms after meal time, including washing linens and dishes.
Assist with prepping food as requested, taking orders, serving dining meals and drinks.

Housekeeping & Laundry Functions:

Create and maintain as atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment throughout the neighborhood and shift.
Meet with your neighborhood personnel on a regular scheduled basis to assist in identifying and correcting problem areas and/or the improvement of services.
Report all complaints and grievances made by residents to Director or Administrator.
Attend and participate in staff development programs and activities that are required.
Safety and Sanitation:
Participate in an in-service training program prior to performing tasks that involve potential exposure to blood/body fluids.
Wash hands before and after performing any service for the resident.
Immediately notify the Supervisor of any resident leaving/ missing from the facility.
Follow established safety precautions in the performance of all duties.
Report all hazardous conditions and equipment to the Supervisor immediately.
Report all safety violations.
Use only the equipment you have been trained to use. Operate all equipment in a safe manner. Inform supervisor of your equipment and supply needs.
RESIDENT RIGHTS:
Maintain the confidentiality of all resident care information.
Ensure that you treat all residents fairly and with kindness, dignity and respect.
Ensure that all nursing care is provided in privacy.
Knock before entering the residents room.
Report all grievances and complaints made by the resident to the Nurse Leader/ Neighborhood Nurse.
Report all allegations of resident abuse and/or misappropriation of resident property to Nurse Leader/Neighborhood Nurse immediately.
Honor the residents refusal of treatment request. Ensure that such requests are in accordance with facilitys policies governing advance directives. Discuss refusal of treatment with the Nurse Leader/Neighborhood Nurse for appropriate follow-up.
WORKING CONDITIONS:
Works throughout the nursing service area (i.e., nurses stations, residents rooms, etc.).
Sits, stands, bends, lifts and moves intermittently during working hours.
Is subject to frequent interruptions.
Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all circumstances.

What You'll Need:

Summary of Duties of the Resident Care Assistant
The primary responsibility of the Resident Care Assistant (RCA) is to provide physical and psychosocial care to all residents in a manner that promotes dignity, choice, and individuality. The Resident Care Assistant reports to the shift supervisor as well as the Wellness Director.
Essential Functions of the Resident Care Assistant
Provides all necessary care and services to assigned residents and assists other wellness department associates as needed.
Works collaboratively with peers and other team members.
Provides assistance with activities of daily living such as with bathing, dressing, personal hygiene (includes: shaving, oral care, nail care, hair care, foot care), toileting, eating, and ambulation/mobility.
Maintains professional demeanor at all times when interacting with residents and families.
Reports any suspected or witnessed instances of verbal, mental, or physical abuse to licensed staff immediately.
Utilizes safe transfer techniques when assisting residents with mobility.
Washes residents' laundry as needed.
Understands and uses Centers for Disease Control and Prevention Standard Precautions, OSHA's Occupational Exposure to Bloodborne Pathogens standard, and follows established infection control procedures, hazardous communication, and other safety rules.
Documents provision of services on company forms.
Operates designated medical equipment, copy, scan, fax machines, and telephone.
Escorts residents and coordinates internal transportation needs.
Assists residents with personal correspondence/telephone use when requested.
Observes and reports changes in residents' physical condition and cognitive/emotional status to Wellness Director or supervisor, as needed.
Conducts room checks and resident rounds.
Monitors for environmental safety hazards.
Respond to emergencies in a prompt and calm manner; Immediately reports such emergencies to the supervisor.
Uses tactful, diplomatic communication techniques in potentially sensitive or emotionally charged situations.
Follows-up with appropriate staff, residents, or other individuals regarding reported complaints, problems, and concerns.
Non-Essential Functions of the Resident Care Assistant
Participates in and supports the resident-centered activities program.
Care for sensory enhancement devices such as eyeglasses and hearing aids.
Completes housekeeping tasks such as bed-making, linen changes, vacuuming, emptying trash, and cleaning bathroom.
Acts as ambassador and public relations representative to guests and other off-campus visitors.
Copies special paperwork or forms.
Assists in a variety of tasks related to dining (escort residents to dining room, assist with set-up and clean tables, serve meals, bus tables, etc.).
Demonstrates an appreciation of the heritage, values and wisdom of the residents, and an understanding of the aging process.
Collaborates with associates in other departments to assure necessary care and services are provided.
Participates in projects or committees as assigned.
Performs related duties as assigned.
Attends and participates in staff meetings and mandatory in-services.
Knowledge, Skills, Abilities, and Experience
Ability to read, write, and speak in English.
Certification by state approved program, if required.
Satisfactory completion of the company Resident Assistant skills competency checklist.
Be in good physical health and free of communicable diseases; Lifting, bending, and pushing medication cart about residence is required.
Must be able to lift 40 pounds.
Completion of drug testing and criminal background check upon hire and upon request of supervisor.
Experience in Assisted Living preferred, but not required.
