Changes bed linens, handles soiled linens and incontinent supplies appropriately to maintain infection control standards.
Keeps Resident rooms, common areas and spa rooms neat and clean.
Receives report from exiting shift and gives oncoming shift report.
Identifies and reports promptly any Resident skin care issues, medical or behavioral concerns and/or noted changes in Resident condition to the nurse.
Initiates and participates in Resident Therapeutic Engagement opportunities, including Linked Senior, where appropriate.
Assists with giving guidance to newly hired NursingAssistants and Resident Care Assistants using proper procedures and protocol.

Prepares reports by delineating reasons for review, methodology, findings, violations of regulations, and recommendations in order to provide written documentation of the progress of the case, the findings of the review process, and the basis for the Department's administrative actions.
Participates in the evidentiary meetings with the Section Supervisor, in-house medical staff and/or legal counsel by discussing the provider case history, case strategy, and findings in order to recommend sanctions in accordance with the Department's guidelines, and to prepare for litigation proceedings.
Testifies at administrative hearings and/or trials in Commonwealth or Federal Court by appearing as the Department's witness in order to provide testimony regarding case development or findings, other pertinent information obtained during the review, and chain of custody of evidence.
Prepares additional written material in the course of the reviews by writing memoranda, letters, and reports as indicated in order to make case status changes, refer information to other agencies, to communicate with recipients and providers, or to carry out necessary review activities.
Responds to complaints for assigned provider types from multiple sources including, but not limited to MA Provider Compliance Hotline, OMAPTips web site, letters, e-mail and phone.
Responds to requests for information from other State or Federal Government offices or outside entities.
Maintains case tracking information on assigned cases.
Makes recommendations for changes in policies and procedures related to the work of the Section.
Travels to perform duties as necessary.
Performs other related duties and special projects as assigned by the Supervisor in order to meet the goals and objectives of the Bureau.

Collects patient data, monitor and document status of vascular accesses, and patient observations.
Reviews lab work, collecting or obtaining blood samples and blood culture specimens and prepares package for shipping.
Immediately reports abnormal findings or change in status to assigned RN for further evaluation.
Communicates all emergencies, patient life-threatening complications, and related situations to the attention of the RN for further instruction.
Maintains compliance with safety and infection preventative measures, ensuring patient safety.
Reinforces education with patients and family members on all aspects of End Stage Renal Disease.

Entering prescribed treatment into dialysis machine, setting up dialysis machines, dismantling dialysis machine after treatment is completed, performing chemical disinfection of dialysis machines and recording.
Observing dialysis machines and RO water system for correct functioning, responding to visual and audible alarms and making appropriate adjustments throughout the dialysis treatment to ensure patient safety.
Monitoring vital signs during treatment and taking patient vitals pro and post dialysis; blood pressure, temperature, weight and documenting it on the Hemodialysis Treatment Record
Perform vascular access evaluation pre-treatment and report unusual findings to RN immediately
Perform cannulation of patient's vascular access or performing catheter care, as allowed by state law.
Obtain homeostasis and apply appropriate dressings.
Adheres to all facility policies, procedures, and protocols, participates in all required staff meetings.
Collects patient data, monitor and document status of vascular accesses, and patient observations.
Reviews lab work, collecting or obtaining blood samples and blood culture specimens and prepares package for shipping.
Immediately reports abnormal findings or change in status to assigned RN for further evaluation.
Communicates all emergencies, patient life-threatening complications, and related situations to the attention of the RN for further instruction.
Maintains compliance with safety and infection preventative measures, ensuring patient safety.
Reinforces education with patients and family members on all aspects of End Stage Renal Disease.
Assists in the management of inventory supplies.
Other duties, as assigned.
CUSTOMER SERVICE:
Responsible for supporting the values and customer service standards of HDS/DCC.
Accountable for outstanding customer service to all staff, clients and vendors
Develops and maintains effective relationships through effective and timely communication.
Initiates action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.
EDUCATION/ QUALIFICATIONS:
Must possess a minimum of High School diploma or GED
Current CCHT, Bonent or State required certificate
Current CPR card
EXPERIENCE:
Must be PC proficient and able to thrive in a fast-paced setting; experience in Microsoft Office Suite: Outlook, Word and Excel a must
Good presentation & communication skills
Must be productive & resourceful with the ability to meet deadlines
OTHER QUALIFICATIONS:
1 or more years' experience as a certified Patient Care Technician preferred
Must be a self-starter who can work independently yet function in a highly collaborative, fast-paced team environment.
Ability to multi-task, work under pressure and meet deadlines effectively
Excellent written and oral communication skills
Strong data entry, organizational and interpersonal skills
High integrity, team player with patient service focus and flexibility

Working hours can vary including the days, nights, weekends, and holidays. This position requires extensive travel between assigned patient's homes and may be up to long distances depending on the patient's needs. Travel to scheduled staff meeting may be required. PCTs spend considerable time walking, bending, stretching, and standing; assisting in patient transfers. PCTs must follow proper body mechanics and procedures for lifting/ repositioning patients without injury. PCTs may face hazards from exposure to chemicals and infectious/ contagious diseases. The work environment is characteristic of a dialysis facility with air temperature control and moderate noise.
Ability to lift at least 30 pounds. Moving, lifting or transferring of patients may involve lifting of up to fifty (50) pounds.
Must be able to pass the Isahara's Color Blindness Test.

What You'll Do:
As a Med Tech Charge, you'll provide compassionate, resident-centered care including:
Deliver care following the Resident Assessment and Support Plan (RASP) and guidance from supervisory staff
Assist residents with activities of daily living: bathing, dressing, toileting, oral care, eating, ambulation, and positioning
Support residents with telephone use, laundry, and room tidying
Assist in activities and dining, helping residents with meal choices and serving drinks/meals
Administer medications and treatments per physician's orders, following all policies and procedures
Accurately document care in electronic and paper records
Report changes in resident conditions promptly to the Charge Nurse
Maintain safety, sanitation, and regulatory compliance throughout the facility
Participate in admissions, transfers, discharges, and post-mortem care as needed
