Maintain confidentiality of resident and facility information. 17. Report changes in resident conditions to Charge Nurse or Nurse Supervisor. 18. Follow facility procedures for admission, discharge and transfer of residents. 19. Complete documentation accurately, timely, following facility policies and procedures. 20. Listen to resident and family complaints and report to Charge Nurse. 21. Ensure residents have call lights in reach. Answer call lights promptly. 22. Assist with activities of daily living as designated in each resident's care plan. 23. Participate in the planning and implementation of the individualized plan of care for assigned residents. 24. Ensure residents are given adequate assistance with meals, nourishment and transportation to meal service area(s). 25. Check name bands on all assigned residents daily and report to Charge Nurse any bands that have been removed or need to be changed. 26. Consistently work cooperatively with residents, charge nurses, treatment nurses, restorative nurses, other nursing assistants, physicians, families, consultant personnel and ancillary service providers. 27. Observe all facility safety policies and procedures. 28. Observe all facility policies and procedures. 29. Report for work in a clean, neat uniform and consistently present an appropriate professional appearance. 30. Report for work as scheduled and consistently demonstrate dependability and punctuality. 31. Assume accountability for data contained in the employee handbook. 32. Perform other related duties as directed by his/her supervisor. 33. Perform all duties assigned in an effective, timely and professional manner. 34. Follow facility's infection control procedures. 35. Follow Resident Rights policies at all times. 36. Be responsible for the safety of residents under his/her supervision. 37. Attend training in dementia management and resident abuse prevention as assigned. 40. Participate in all compliance and ethics training programs. 41. Use only authorized abbreviations established by the facility when recording information. 42. Agree not to disclose assigned user ID and password for accessing resident/facility information. Promptly report suspected or known violations of such disclosure to the Charge Nurse or Compliance and Ethics Officer. 43. Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Charge Nurse or Compliance and Ethics Officer. 44. Assist residents with bathing, oral care, dressing/undressing, toileting, perineal care, catheter care, grooming, eating and taking/recording vital signs in accordance with facility policies. 45. Strip bedding, change linens and make resident beds as assigned. 46. Ensure that department computer workstations left unattended are properly logged off. 47. Report all accidents/incidents to your supervisor no matter how minor they may be. Report such occurrences on the shift in which they occur. 48. Report any known or suspected unauthorized attempt to access facility's information system. 49. Report suspected resident abuse immediately to the Administrator. 50. Promptly report suspected violations of the facility's compliance and ethics program to the facility's Compliance and Ethics Officer.
