What is the role?
The Office Family Advocate (Office FA) is a vital member of the Nest Health interdisciplinary care team who has training and experience as a Medical Assistant (MA). The primary duty of this role is to provide office-based support to teams operating in the field, including scheduling of referrals, entering orders, and other tasks to help facilitate completion of encounters. The Office FA will eventually support multiple teams in the field. The Office FA will also, when needed, serve as a Field Family Advocate, and provide direct patient care to adults and children in the home setting (under the supervision of an authorized clinician); accordingly, this individual has lived experience with the communities served by Nest Health. The Office FA has excellent interpersonal skills, which enables effective collaboration with patients, doctors, nurses, and other professionals and helps build trust and maintain a strong connection to Nest Family members. The Office FA has strong attention to detail and organizational skills. The Office FA coordinates services delivered by Nest Health and helps refer to external social care providers that address the social drivers of health (food insecurity, housing instability, domestic violence, financial hardship, and lack of access to transportation, for example). The Office FA participates in outreach efforts to Nest Families.
In addition to their duties as an FA, this role will also serve as the local manager of the practice office, under the oversight of the Practice Operations Manager. The Office FA is responsible for overseeing the administrative and business operations aspects of the Baton Rouge Nest Health office where Nest clinical teams touch down and collaborate as part of Nest's virtual and home-based care medical practice. This includes physical space maintenance, ensuring efficient clinical operations, aiding Practice Operations Manager in ensuring business performance, acting as the culture champion.

What will you do?
- Provide support to teams operating in the field with office-based support including but not limited to:
- Assist in patient registration, checking and confirming eligibility, establishing administrative workflows, and prior authorization approvals
- Fulfill clerical responsibilities which may include: sending/receiving/scanning medical records, obtaining diagnostic reports, hospital notes & referral information, completing forms/requisitions, managing the charting process & sending specimens to labs
- Actively participate in all compliance-related matters including, but not limited to, equipment, laboratory tests, medications, and vaccine inventory
- Provide information and assistance through advocacy, navigation support, and education to Nest families on availability and eligibility of health and social care services. Follow up with families to ensure they were able to access the resources provided/suggested by Nest
- Collaborate with the interdisciplinary team to proactively contact families and ensure they are connected to the providers, resources, support they need to meet their Lead Family health goals
- Participate in outreach efforts to Nest Families including explaining program details to potential families
- Provide direct patient care and support for a panel of patients in the home setting which may include but not be limited to the following:
- Perform tasks under the direction of the licensed provider including taking vital signs, updating patient history, collecting lab specimens, and assisting with exams
- Administer oral, topical, and injectable medications and/or vaccinations
- Interview patients and record medical history including standardized screening questionnaires
- Assist providers in procedures using aseptic techniques
- Adhere to infection control standards including, but not limited to, hand hygiene, use of PPE, and isolation of patients with known
infectious disease
- Complete accurate EMR documentation of patient information and services provided per the requirements of Nest Health
- Delegate (with assistance of HSM) tasks of the day to FAs to support providers in the field M-F and rotating Saturdays
- Act as Clinical lead on the Vaccines for Children (VFC) process and maintenance. Monitor vaccine par and coordinate ordering with the Practice Operations Manager
- Demonstrate effective teamwork, communication, and collaboration as part of an interdisciplinary care team
- Serve as the local office management under the oversight of the Practice Operations Manager
- Maintain productive physical operations and space- office supplies, maintenance, IT and equipment management, mail and invoice processing, furniture and renovations/build out, vendor
management
- Efficient clinical operations- medical supplies, team and patient family scheduling, management of clinical vendors (e.g. field-based security), mileage and payroll tracking as audits, understands
clinical workflows
- Business performance- meeting organization, performance reporting and projects, power user of core tools (Athena EMR, Salesforce, SharePoint, and others), and other support to help Nest care team meet and exceed clinical quality and performance goals
- Workplace culture- local leader for onboarding fun

- Compliance with health, safety and HIPAA guidelines and local lead for business continuity planning
What do you bring to the Nest?
- Successful completion of an accredited Medical Assistant program

Responsibilities
The Home Health Physical Therapist Assistant (PTA, LPTA) performs treatment and provides exercise instruction and patient education aimed at improving/enhancing the patient's well-being while following the established plan of care.
Responsible for following all state specific laws governing the provision of physical therapy in home care, to follow the treatment set only as defined by the supervising PT; to perform only those procedures that he/she is qualified and trained to perform and to make all recommendations for changes in the plan of care to the supervising Physical Therapist.
Instructs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
Observes, records, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition. Coordinates care with the other members of the healthcare team as appropriate.
Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy within the scope of the physical therapy assistant.
Education and Experience
License Requirements
Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
Current CPR certification is required.
Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

LA and TX: Must have one (1) year of experience as a licensed PTA.

Qualifications for the Medical Office Assistant:
High School Diploma or GED required
Requires up to l year of technical or other specialized training including advanced training in secretarial/medical office support (e.g.: typing/keyboarding/word processing, medical terminology, medical insurance/billing, etc.) and office administrative procedures including customer service etc.
Must possess organizational and problem-solving skills, exceptional communication and customer service and a strong attention to detail.
Must be able to work independently and as part of a team, have good interpersonal skills and a willingness to be flexible and adapt to changing situations.
