Under the supervision of the Transportation Coordinator, the Medical Transport is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies. Location: Central Valley PACE Center in Merced, Ca. Schedule: Monday - Friday. This position requires availability between the hours of 8:00am - 5:00pm. Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse. Inspects van and maintains it in a clean, orderly and safe condition. The driver must exhibit exceptional customer service to patients, families, case managers, facilities, physicians, and nurses. Completes driver log each time the vehicle is used. Keep an accurate record of all facility transports, mileage and times. Provides facility administrator with completed logs. Inspects the company vehicle monthly using the Vehicle Maintenance Log. Reports all maintenance or vehicle needs to the facility administrator timely and follow up as appropriate. Assists with maintaining all maintenance records and other records regarding the company vehicle. Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties Completes documentation as required.
