Position Summary:
This position is responsible for monitoring and racking all participants' health and nutrition records to ensure program is in compliance at all times.
Staff is also responsible for contacting physicians to request health and nutrition records accordingly, as well as helping families access needed health services.

Job Description
Assist families in making and keeping medical appointments.

Ensures that parents obtain the needed prescription or over the counter medication, as needed.
Teaches parents on how to follow the doctor's instructions.
Submits complete, accurate and timely medical and dental records.

Informs supervisor of all needs expressed by family members or observed.

SECTION I-JOB DESCRIPTION
Responsible for developing, facilitating and implementing outreach activities/strategies, special events, programs and presentations which support the activities of the Precinct. Also responsible for connecting people in the community by creating agreements and cultivating relationships with businesses, individuals and other relevant organizations.
ESSENTIAL FUNCTIONS:
Responsible for building and maintaining relationships with community contacts.
Develops, facilitates and implements outreach activities, special events, programs and presentations to increase awareness of the Precincts presence and activities in the community.
Serves as a contact for media inquiries and request for information.
Develops and maintains relationships with the local media, including print and electronic media outlets.
Manages constituent related activities, such as complaints, correspondence and other similar administrative and customer support functions.
Assists in the preparations of presentation or speeches.
Provides creative support for internal and external communication efforts including print, web sites, electronic communication, community engagement, media and video.

EDUCATION AND EXPERIENCE:

Turn all medications found in the resident's room/possession over to the Nurse Supervisor/Charge Nurse.
Provide residents with Reality Orientation as instructed.
Watch for and report any change in room temperature, ventilation, lighting, etc.
Food Service Functions
Prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to/from dining room, etc.).
Serve food trays. Assist with feeding as indicated (i.e., cutting foods, feeding, assist in dining room supervision, etc.).
Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.).
Record the resident's food/fluid intake. Report changes in the resident's eating habits.
Keep residents' water pitchers clean and filled with fresh water (on each shift) and within easy reach of the resident.
Serve between meal and bedtime snacks.
Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, take to bathroom, brush teeth, clean dentures, etc.).
Check rooms for food articles (i.e., food in proper container, unauthorized food items, etc.).
Assist residents with basic activities of daily living (ADL's).
Promptly answer resident call lights.
Transport and assist residents throughout the facility.
Maintain safety of residents at all times.
Qualifications:

Personnel Functions
Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.

Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents.
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.
Report all complaints and grievances made by the resident.
Notify the facility when you will be late or absent from work.
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to your supervisor.
Personal Nursing Care Functions
Participate in and receive the nursing report upon reporting for duty.
Follow established policies concerning exposure to blood/body fluids.
Assist residents with daily dental and mouth care (i.e., brushing teeth/dentures, oral hygiene, special mouth care, etc.).
Assist residents with bath functions (i.e., bedbath, tub or shower bath, etc.) as directed.
Give backrubs as instructed.
Assist residents with dressing/undressing as necessary.
Assist residents with hair care functions (i.e., combing, brushing, shampooing, etc.).
Assist residents with nail care (i.e., clipping, trimming, and cleaning the finger/toenails).

Shave male residents.
Keep hair on female residents clean shaven (i.e., facial hair, under arms, on legs, etc.) as instructed.
Keep residents dry (i.e., change gown, clothing, linen, etc., when it becomes wet or soiled).
Change bed linens. Keep linens tight to avoid wrinkles from forming under the resident.
Make beds (occupied and unoccupied).
Put extra covers on beds as requested.
Position bedfast residents in correct and comfortable position.
Assist resident with bowel and bladder functions (i.e., take to bathroom, offer bedpan/urinal, portable commode, etc.).
Maintain intake and output records as instructed.
Keep incontinent residents clean and dry.
Check and report bowel movements and character of stools as instructed.
Prepare and give enemas. Report results as instructed.
Collect specimens as instructed (i.e., urine, sputum, stools, etc.).
Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.).
Assist residents in preparing for activity and social programs (i.e., church services, parties, visitors, etc.).
Assist in transporting residents to/from appointments, activity and social programs, etc., as necessary.
Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts etc.
Assist residents to walk with or without self-help devices as instructed.
Perform restorative and rehabilitative procedures as instructed.
Change dressings, bandages, binders, etc., as instructed.
Provide eye and ear care (i.e., warm/cold compresses, eye/ear instillation, cleaning eyeglasses/hearing aides, etc.) as instructed.
Assist with the application of nonsterile (moist and dry) warm/cold compresses.
Assist in preparing the resident for a physical examination.
Weigh and measure residents as instructed.
Measure and record temperatures, pulse, and respirations (TPRs) as instructed.
Answer resident calls promptly.
Ensure that residents who are unable to call for help are checked frequently.
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
Assist with the care of the dying resident.
Provide post-mortem care as instructed.
Special Nursing Care Functions
Observe and report the presence of pressure areas and skin breakdowns to prevent decubitis ulcers (bedsores).
Report injuries of an unknown source, including skin tears.
Provide daily indwelling catheter care.
Provide daily perineal care.
Perform nonsterile (clean) caginal irrigation/douches.
Assist with the application of slings, elastic bandages, binders, etc.
Give tepid sponge baths.
Provide daily Range of Motion Exercises. Record data as instructed.
Turn bedfast residents at least every two (2) hours.
Perform diabetic urine testing (i.e., clinitest, acetest, etc.).
Provide physical and respiratory therapy as instructed.
Perform special treatment as instructed.
Observe disoriented and comatose residents. Record and report data as instructed.
Turn all medications found in the resident's room/possession over to the Nurse Supervisor/Charge Nurse.
Provide residents with Reality Orientation as instructed.
Watch for and report any change in room temperature, ventilation, lighting, etc.
Food Service Functions
Prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to/from dining room, etc.).
Serve food trays. Assist with feeding as indicated (i.e., cutting foods, feeding, assist in dining room supervision, etc.).
Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.).
Record the resident's food/fluid intake. Report changes in the resident's eating habits.
Keep residents' water pitchers clean and filled with fresh water (on each shift) and within easy reach of the resident.
Serve between meal and bedtime snacks.
Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, take to bathroom, brush teeth, clean dentures, etc.).
Check rooms for food articles (i.e., food in proper container, unauthorized food items, etc.).
Assist residents with basic activities of daily living (ADL's).
Promptly answer resident call lights.
Transport and assist residents throughout the facility.
Maintain safety of residents at all times.
Qualifications:
CNA license is required.

ProSidian seeks a CertifiedNursingAssistant to support an engagement with The Veterans Integrated Services Network 17 (VISN 17). This position supports the current expanded operations and post-COVID operations to restore the healthcare systems to pre-Pandemic operations.
This is a Contract Contingent position and the CertifiedNursingAssistant services shall be performed at the: Texas Valley Costal Bend Health Care System 2701 S 77 Sunshine Strip, Harlingen, TX 78550
Services Task and Job Requirements
In order to perform the duties set forth for this project, the CertifiedNursingAssistant must meet both the minimum education and experience requirements set forth.
The CertifiedNursingAssistant will be required to perform the following duties:
Take/record blood pressure, pulse, respiration, temperature, height, and weight.
Measure/record intake and output.
Provide routine care to pre- and post-operative patients.
Obtain routine specimens (excluding blood) for laboratory analysis and perform standard tests.
Apply/remove bandages, binders, simple dressings and hot and cold packs.
Assist patients in walking, moving, and changing positions.
Prevent obstruction, dislodgment, and unnecessary movement of drainage, suction, and parenteral fluid apparatus.
Assist physicians and/or nursing staff as appropriate during patient examinations and treatments.
Provide personal care to include bathing, shaving, and feeding.
Make equipment available to patients such as bedpans, linen, and grooming items.
Accompany patients in wheelchairs, on litters or ambulatory within the hospital.
Motivate patients to manager their illnesses and provide patient instructions as directed by unit standard operating procedures.
Perform general maintenance duties including, but not limited to, making beds, cleaning/sorting supplies, handling linen according to policies and cleaning patient units.
Perform patient treatment procedures within established guidelines.
Communicate pertinent patient data to charge nurse and/or physician as appropriate.
Recognize and respond to emergency situations according to preplanned emergency response measures.
Provide patient care as assigned to include documentation of nursing activities in the patient record as appropriate.
Assist with admission and discharge of patients as well as maintenance of patient information and filling out laboratory/x-ray/EKG slips.
Additional duties may be assigned. Training will be provided if needed.
