Medication Administration
Follow the six 'rights" of medication assistance (right resident, right drug, right dosage, right time, right route, right record/documentation) each and every time assistance with a medication occurs. Check each of the six "rights" of medication assistance three different times: when the medication is taken out of the medication cart, immediately before assisting with the medication, and immediately after assisting with the medication
Administer medications to residents according to their Medication Observation Record (MOR) and protect resident privacy. Provide residents with any treatments required as part of their Resident Care Plan as designated. Document clearly and accurately on the Medication Observation Record (MOR) after a medication is taken/given, refused, held, discontinued, changed, disposed of, returned, following the Oaks Senior Livening policy.
Respect the residents' right to refuse medication/treatment. Protect the residents' safety with medications. Explain to the resident what medication is being given and why, prior to assisting with medication.
Observe the resident for any adverse reaction to a medication and report it to the Wellness Director and/or the Executive Director immediately. Report all medication errors honestly and immediately.
Communications
Attend all regular staff meetings and required in-service training sessions.
Effectively communicate residents' needs with caregiver staff at change of shift according to the Oaks Senior Living policies and procedures. Effectively communicate to the Wellness Director and/or the Executive Director any changes noted in a resident's condition or behavior, adverse reaction to a medication, any resident or family member concerns, complaints, refusal of a medication, any errors noted on the Medication Observation Record (MOR).

Resident Services

Adhere to each resident's Negotiated Service Plan and notify the Wellness Director, MOD, and/or the Executive Director of any changes in resident condition.
Answer and respond immediately to resident calls.
Seek assistance and advice from the Executive Director or the Wellness Director immediately if you have any doubts or questions when assisting with medications or treatments
before
you perform the assigned task.
Assist with serving meals and supervise caregiver staff in the dining room. Ensure that dining room is cleaned after each meal service.
Control the spread of infection by following standard precautions. Dispose of infectious and potentially infectious waste according to residence policy and procedures.
Establish and maintain a good relationship with residents and their families.
Respond to resident emergencies following the proper procedures.
Assist in developing and maintaining a schedule for the caregiver staff that sufficiently meets the needs of residents and ensures proper staffing.
Respond to on-the-job injuries in accordance with the Oaks Senior Living policies and procedures.
Complete 16 hours of continuing education units (CEU) biennially specific to CMA's as outlined by the Alliant GMCF Registry. In addition 16 hours of CEU's must be completed annually.
Lead by example, encourage teamwork, and promote the Oaks Senior Living philosophy.
Provide an "open door" to employees, addressing any concerns or grievances they may have.
Maintain CPR & First Aid certification.
Medication Administration
Follow the six 'rights" of medication assistance (right resident, right drug, right dosage, right time, right route, right record/documentation) each and every time assistance with a medication occurs. Check each of the six "rights" of medication assistance three different times: when the medication is taken out of the medication cart, immediately before assisting with the medication, and immediately after assisting with the medication
Administer medications to residents according to their Medication Observation Record (MOR) and protect resident privacy. Provide residents with any treatments required as part of their Resident Care Plan as designated. Document clearly and accurately on the Medication Observation Record (MOR) after a medication is taken/given, refused, held, discontinued, changed, disposed of, returned, following the Oaks Senior Livening policy.
Respect the residents' right to refuse medication/treatment. Protect the residents' safety with medications. Explain to the resident what medication is being given and why, prior to assisting with medication.
Observe the resident for any adverse reaction to a medication and report it to the Wellness Director and/or the Executive Director immediately. Report all medication errors honestly and immediately.
Communications
Attend all regular staff meetings and required in-service training sessions.
Effectively communicate residents' needs with caregiver staff at change of shift according to the Oaks Senior Living policies and procedures. Effectively communicate to the Wellness Director and/or the Executive Director any changes noted in a resident's condition or behavior, adverse reaction to a medication, any resident or family member concerns, complaints, refusal of a medication, any errors noted on the Medication Observation Record (MOR).
Ensure required paperwork for providing resident care is completed and up-to-date.

This role includes providing hands on care, physical and emotional support as outlined in each resident's Personalized Service Plan (PSP) while maintaining a safe, comfortable home like environment. The Resident Care Associate is responsible for demonstrating all elements of wellness, remaining in accordance with federal, state and local standards and regulations and Phoenix policies to promote the highest degree of quality care and services to our residents.
PRINCIPLE DUTIES AND RESPONSIBILITIES
RESIDENT CARE
Resident Care Associate | CNA
Participates in the development of the Personalized Service Plans and updates; attends service plan review meetings with families as requested
Follows the hydration schedule established for the residents is maintained during the shift
Completes resident meal attendance sheets have been completed and any resident who has not dined in the dining room has been provided room service and / or accounted for.
Knows and complies with all resident rights
Initiates, assigns, and assist with activities, as appropriate
Check all assigned residents daily for cleanliness, appropriate clothing, any continence issues and change in condition
Uses proper sanitary procedures and universal precautions
Checks on assigned resident rooms daily for general cleanliness and neatness and takes appropriate action; ensures high risk rooms are double checked
Assist with conducting laundry rooms, common areas - including bathrooms - checks for cleanliness and any exposed hazardous chemicals; and takes appropriate action (i.e. contacting housekeeping, personally cleaning the room, securing the chemicals)
Completes all ADLs for assigned residents according to the resident's Personalized Service Plan (PSP)
Responds to security system and resident call bells promptly & immediately; takes appropriate action including resetting call bells
EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION
Resident Care Associate | CNA
High School education preferred and may be required by the state
Previous experience working with elders or disabled individuals, preferred
Must be at least 18 years of age to perform the personal care aspects of the RCA position
Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred

Position Overview - Hospice Aide:

• Observe the patient and the patient's condition and requests, report these observations, and document observations and care performed
• Provide positive communication within the home with ethical and respectful care, including confidentiality

Position Qualifications

• Demonstrates good verbal and written communication, and organization skills

• Meets the training requirements of the State
• Completed a minimum of 75 hours of classroom and supervised practical training. Sixteen hours of classroom must be completed prior to a mandatory 16 hours of practical training
• At least 18 years of age
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
• Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
