Always check on residents who have not come to eat. • Maintain good hygiene and practice proper hand washing technique for infection control. • Assists with meals including serving meals and bussing tables. • Assisting with general room cleaning as needed. • Assists new residents upon admission and orients them to facility, staff, other residents, and routines. • Monitors residents' personal needs and informs Resident Care Supervisor or Assistant Administrator. • Reports any resident behavioral/physical concerns immediately to the Resident Care Supervisor. • Is constantly aware of cleanliness and appearance of Resident Care areas and assists to clean areas of immediate concern when appropriate. • Completes Resident Observation Reports in a precise and timely manner, placing a copy of such in the proper tray. • Assists to help complete the Resident weight record as required. • Answer the phones as needed. • Assists with proper self-administration of medications technique. This role follows a Monday to Friday schedule from 8:00 AM to 4:30 PM, with flexibility to provide weekend or evening support as needed. As a valued member of the care team, the Hospice Aide delivers compassionate personal care and support, helping patients maintain comfort and dignity at the end of life. Overview The Hospice Aide is a trained caregiver who provides personal care and support services to hospice patients under the supervision of a Registered Nurse Case Manager or the Manager/Director of Clinical Services. This role promotes patient comfort, dignity, and safety by assisting with daily activities and observing and reporting changes in the patient's condition. Assignments are made based on the plan of care and the aide's training and competency. Key Responsibilities Provide personal care as assigned, including bathing, dressing, grooming, toileting, oral hygiene, and skin care. Assist with ambulation, transferring, repositioning, and range-of-motion exercises as instructed. Observe and report changes in the patient's physical or emotional condition to the supervising nurse. Assist with meals, feeding, and hydration support, as appropriate to the care plan. Provide companionship, emotional support, and socialization to patients, fostering comfort and dignity during care. Take and record vital signs (e.g., temperature, pulse, respiration, blood pressure) if trained and assigned by the nurse. Offer emotional support and maintain a respectful and compassionate presence in the care setting. Maintain a clean and safe patient environment, including light housekeeping related to direct care. Provide short-term respite to family caregivers during scheduled visits, when appropriate. Document care provided in accordance with agency procedures and timelines. Follow standard precautions, infection control practices, and HIPAA privacy guidelines. Participate in team meetings and in-service training sessions, as required. Deliver care in any setting where the patient resides, including private homes and licensed care facilities. Provide weekend or after-hours support when directed by the supervising nurse or Manager/Director of Clinical Services. Performs other duties as assigned. Qualifications Maintains active certification as a nursing assistant (CNA) in good standing, with completion of a state-approved training program that includes at least 75 hours and supervised clinical experience. Current CPR certification. Ability to read and follow written care instructions and document care accurately. Understanding of and alignment with the hospice philosophy and the needs of terminally ill individuals. Ability to work in home and facility settings with reliable transportation and valid driver's license. Demonstrated empathy, sensitivity, and ability to support patients and families from diverse backgrounds. Must demonstrate a strong commitment to Arc Hospice & Palliative Care's mission and values.
