    General Purpose The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors. * Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary. * Use only authorized abbreviations established by this facility when recording information. * Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical. * Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner. * Report all accidents and incidents you observe on the shift that they occur. * Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator. * Report any known or suspected unauthorized attempt to access facility's information system. * Perform only those nursing care procedures that you have been trained to do. * Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.). * Greet residents and escort them to their room. * Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. * Make resident comfortable. * Inventory and mark the resident's personal possessions as instructed. * Store resident's clothing. * Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. * Transport residents to new rooms or to the receiving area. * Assist with loading/unloading residents' to/from vehicles as necessary. * Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. * Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. * Report all complaints and grievances made by the resident. * Participate in and receive the nursing report as instructed. * Follow established policies concerning exposure to blood/body fluids. * Make beds as instructed. * Put extra covers on beds as requested. * Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed. Weigh and measure residents as instructed. * Ensure that residents who are unable to call for help are checked frequently. * Answer resident calls promptly. * Check residents routinely to ensure that their personal care needs are being met. * Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.). * Record the resident's food/fluid intake. Report changes in the resident's eating habits to the Nurse Supervisor/Charge Nurse. * Keep residents' water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. * Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, etc.). * Participate in and receive the nursing report upon reporting for duty. * Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. * Follow work assignments, and/or work schedules in completing and performing your assigned tasks. * Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. * Notify the facility when you will be late or absent from work. * Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents. * Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). * Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled). * Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.). * Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. * Perform restorative and rehabilitative procedures as instructed. * Assist in preparing the resident for a physical examination. * Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. * Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores). * Report injuries of an unknown source, including skin tears. * Observe disoriented and comatose residents. Record and report data as instructed * Perform special treatments as instructed. * Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility. * Use only the equipment you have been trained to use. * Report defective equipment to the Nurse Supervisor/Charge Nurse * Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. * Report all allegations of resident abuse and/or misappropriation of resident property. * Honor the resident's refusal of treatment request. Report such requests to your supervisor.     Apply Description Certified Nursing Assistant (CNA) Department: Nursing Reports to: Charge Nurse and/or Director of Nursing FLSA Status: Hourly/Non-Exempt Providence Health Group is a long-term healthcare provider that is committed to providing an environment of wellness, healing, and independence for its residents. Summary: Perform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents. Environment: Work will be performed primarily indoors at one of our long-term healthcare facilities, throughout all areas, including in resident rooms, and on carpeted and/or tiled floors. Work will be performed there routinely around other co-workers, healthcare staff, residents, and guests. Due to the nature of facility's business, worker may be exposed to occasional slippery floors, object on floors, chemicals, sharp objects, hazardous materials and waste (including human), blood borne pathogens, and communicable diseases, as well as high-stress medical and/or life threatening situations. Essential Duties & Responsibilities: Meet physical and sensory requirements stated below, and be able to work in the described environment. Identify and participate in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment. Provide personal care (i.e., grooming, bathing, dressing, oral care, etc.) of residents daily and as needed. Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis. Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics. Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned. Routinely turn and position residents, as necessary. Observe residents carefully and report changes in condition to Charge Nurse. Recognize a resident's ability to make choices and support these within the constraints of the care giving environment. Participate in collecting data needed for the accurate completion of the MDS and the plan of care. Provide input to the nurses for development of the plan of care. Provide adequate hydration to residents at regular intervals each day according to specific resident needs. Prepare residents for each meal and transport to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves. Keep an accurate record of food intake and output, as directed. Maintain established housekeeping standards within assigned duty areas. Assist nursing staff with the basics of aseptic and sterile techniques to avoid infection of residents and self-contamination of equipment and supplies. Be alert for proper ventilation, temperature, light and noise control. Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed. Promptly answer call lights and assist with other resident needs. Utilize proper body mechanics when lifting or transporting residents. Assist in the orientation and training of other staff, as requested. Other special projects and duties, as assigned. Job Requirements: Current Nursing Assistant certification from state Effective verbal and written English communication skills. Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation. Highest level of professionalism with the ability to maintain confidentiality. Ability to communicate at all levels of organization and work well within a team environment in support of company objectives. Customer service oriented with the ability to work well under pressure. Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity. Strong analytical and problem solving skills. Ability to work with minimal supervision, take initiative and make independent decisions. Ability to deal with new tasks without the benefit of written procedures. Approachable, flexible and adaptable to change. Function independently, and have flexibility, personal integrity, and the ability to work effectively with employees and vendors. Physical and Sensory Requirements: Considerable physical activity: Requires heavy physical work; heavy lifting, pushing, or pulling required of objects up to fifty (50) or more pounds. Physical work is a primary part (more than 70%) of job. Push, pull, move, and/or lift a minimum of fifty (50) pounds to a minimum height of three (3) feet and be able to push, pull, move, and/or carry such weight a minimum distance of fifty (50) feet. Standing and/or walking for more than four (4) hours per day. Bending and/or stooping for more than one (1) hour at a time.
