CNA Certified Nursing Assistant QUALIFICATIONS • Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant. • A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled in a nursing assistant certification class. • Must possess a sincere desire to serve the ill, aged and infirm. • Ability to read, write and speak the English language. • Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public. • Willingness to work harmoniously with other personnel. • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning. • Must be in good general health and demonstrate emotional stability. • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. CNA Certified Nursing Assistant GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Perform all duties as assigned and in accordance with facility's established policies and procedures, nursing care procedures and safety rules and regulations. • Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. • Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident's reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident's food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse's call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents' call lights promptly. • Check for food in residents' room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident's room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident's food and nourishment intake as directed. • Perform after meal care as required - cleaning resident's hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident's hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility's established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company Standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected.

Prior to resident's arrival, inform the local pharmacy of the medication needs of new residents, verify medications and proper dosages with physician. Medication is not to be dispensed until verification with physician is complete. Order additional medications as needed and directed by the Health Services Director or Journey Program Director. Track all orders for medication changes and/or refills. Report to the Health Services Director or Journey Program Director any changes in medication order for residents, residents requiring extra services on a continual basis, and any changes in resident physical or mental status. Assist the Resident Services Director in maintaining and providing accurate and inclusive records on all residents. This may include the completion of Medication Administration Record (MAR) duplicate sheets for any changes in a particular resident's medication, performing monthly audits/checks on all residents' charts, assisting with the processing of a new resident's paperwork, filing and maintaining resident files in a professional manner. As requested, maintain an inventory of all department supplies. Notify the Health Services Director or Journey Program Director timely and/or place supply orders, with a supervisor's approval, when supply levels reach a pre-designated threshold. As directed, act as the main point of contact for physicians, family members and vendors in the absence of the Health Services Director or Journey Program Director. At the direction of the Health Services Director or Journey Program Director, assist with department scheduling including covering call-offs, vacations and the elimination of overtime. As required, update the Caregiver Monthly Assignment Journal, Medication Technician Med List and 24 Hour Communication Log. Assists residents with their daily medications, treatments and resident care as assigned and within the scope of practice, policy and procedures and Title 22 Regulations. Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal and notify supervisor and/or Executive Director. Maintain accurate, complete and confidential resident medication and care records according to established procedure and state law. Keep medicine carts and areas orderly, clean and appropriately stocked and locked. Maintain upkeep and cleanliness of medicine carts and medication rooms. Assist medication aides and caregivers in the performance of their job and assume caregiver activities when needed. Participate in and complete narcotic count at beginning and end of each shift. Report any and all discrepancies to Health Services Director or Journey Program Director immediately. Prepare medication for distribution to residents as per the OMG Medication Policy. Carry a radio and/or pager as directed by supervisor to ensure prompt response to resident issues. Be familiar with the meaning and usage of common medical abbreviations, symbols and terms relative to the administration of medications as used by the residence, pharmacists and physicians. Demonstrate knowledge of infection control, i.e., adequate hand-washing, blood borne pathogens procedures, universal precautions, etc. Maintain all required licenses/certifications as required by state law. Report occupational exposure to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the property's policies and procedures governing accidents and incidents. Provide support to community's marketing efforts through positive/friendly interaction during all property tours and telephone contact with the public, and as directed by a supervisor. Maintain a safe and secure environment for all staff, residents and guests, following established safety standards. Encourage teamwork through cooperative interactions with co-workers and other departments. Support a positive and professional image through actions and dress. Performs other duties consistent with the position as assigned by the Health Services Director or Journey Program Director. Qualifications MINIMUM QUALIFICATIONS Education: High school diploma or equivalent. Successfully completed the Company's Medication Administration Course with a passing grade on the test. Completed all state-required medication training. Experience: At least one (1) year experience successfully passing medications. Experience working with elderly clients preferred. Experience with handling medications preferred. Mathematical Skills: Ability to make simple mathematical calculations. Equipment: Use of standard office equipment, telephone and personal computer. Reasoning Skills: Demonstrates an awareness and concern for resident safety at all times. Ability to make independent decisions and works without supervision. Knows and understands facility fire plan, disaster plan, and safety procedures. Oral/Written Communication Skills: Ability to follow written and oral directions. Ability to interact tactfully with residents and family members, staff and general public. Ability to read, write and speak English. Physical Effort: Good physical health verified by a health screening, including a chest x-ray or an intradermal test result not more than 6 months prior or seven (7) days after employment. Be mobile and able to perform physical requirements of the job. Personal Characteristics: Makes resident satisfaction a high priority. Maintains a positive attitude in all situations. Displays sincere compassion towards older adults. Demonstrates genuine concern for the physical and emotional needs of older people and their families.
