Completes insurance and/or disability forms, precertification and/or authorizations, point of care testing, transcribing orders, or order entry (protocol). May be assigned to assist with bump lists, wait lists and appropriate WQs. May make appointments, including follow-up appointments for patients in a high customer service environment in an efficient and timely manner across the healthcare continuum including physician office visits, imaging, and lab post-visit and during patient outreach. Performs and documents in a timely and efficient manner patient outreach and call backs for messages received in pool, MyChart messaging and confirmation calls. May administer vaccines and documents appropriately and in accordance with established policies. May collect specimens and handles per establish protocols including proper labeling and safety handling. Assists physicians/providers with monitoring and reporting lab results. File all reports, labs, radiology reports and miscellaneous correspondence in EMR and attaches results to appropriate orders. May obtain prior authorization for medications, procedures, and testing. Successfully communicates with multidisciplinary team members and patients upholding our Mission, Vision and Values and adhering to Code of Ethical conduct. Maintains working knowledge of regulatory standards and is accountable to sustain these standards in daily operations. Requires flexibility and the ability to multitask in a face paced environment and adjust to the patient volume. May be asked to work at other Ambulatory offices based on volume. Experience Required Minimum one year of recent experience working in a medical facility as a medical assistant and/or documented evidence of externship completed in a medical office. Electrocardiogram (EKG), vital signs, venipuncture, capillary, and injection experience. Epic experience preferred. Excellent organizational, written/verbal communication and teamwork skills. What You Can Expect:
Prepare and monitor equipment used for intermittent or continuous dialysis and water treatment in the acute setting, including quality control checks
Perform and document pre-, intra- and post-dialysis assessments
Initiate, monitor and terminate dialysis treatments per established policies and procedures

Position Overview

What You'll Do:
As a In Home Resident Care Assistant, you will carry out our Mission of Love by shopping with or for residents, accompanying residents for appointments, pet care, laundry, medication reminders, light housekeeping, bath/shower safety and social visits.
Our In Home Resident Care Assistant is responsible for the following:
Provides quality companionship services to seniors in their homes with specific services as outlined in the service plan.
Assist residents with activities of daily living i.e. bathing, hair care, nail care, mouth care, feeding, skin care, and toileting. Completes and documents all care as assigned accurately and in a timely manner.
Assist in meal preparation and providing cueing for meals as indicated.
Assists in keeping resident's living area neat to include light housekeeping chores and laundry as directed by the service plan.
Transports resident to activities or appointments and run errands as directed by the service plan.
What You Have:
High school diploma or equivalency preferred, but not essential
Successful completion of a criminal background check
Sincere interest in working with the elderly
Satisfactorily completes orientation training
Valid Pennsylvania Drivers License required

Assists new residents with move-in transition: provide emotional support
Attendance to all medication trainings scheduled
Complete medication cart audits as assigned and maintain the medication cart as directed
Pass all medication administration observations
Communicates all medication resident needs to the HCC and documents in the Daily Log
Re-orders medications, faxes to pharmacy, communicates with resident family/RP as needed with changes. Reports in Daily Log
Documents all medication administration on the EMAR/MAR. Reports all exceptions to the HCC and documented on EMAR and Daily Log.
Completes all other duties as assigned.
West Virginia AMAP only: Assists with administration of medications within the established scope of practice. AMAP direct supervisor regarding all medication practices is the RN. All new medications or medication changes must be reviewed with the RN before administration.

Position Overview

Files all correspondence, lab work, charts, etc., daily. Schedules appointments for referrals, accurately records in referral log, and updates log as reports are received. Copies charts for referrals and for release. Calls for lab and/or x-ray reports as needed. Assists Receptionist and performs front desk duties, as necessary. Upholds patient/professional confidentiality MINIMUM REQUIREMENTS: The candidate must have completed a High School Diploma. Experience with electronic medical records is preferred. Must have previous experience in the medical field. Computer proficiency is a must.
