Current enrollment in an accredited nursing program or any healthcare program and previous patient care experience preferred. Giving personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Planning and preparing nutritious meals, including shopping, as assigned. 7. Assisting in feeding the patient as assigned. 8. Taking and recording oral, rectal, and auxiliary temperatures, pulse, respiration, and blood pressure when ordered with appropriate completed/demonstrated skills competency. 9. Providing proper care and observation of the patient's skin to prevent the breakdown of tissue over bony prominence. 10. Assessing and reporting on the patient's condition and significant changes to the Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 11. Assist in ambulation and exercise as instructed by the hospice nurse or therapist. 12. Provide a normal range of motion and transfers/positioning as directed. 13. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 14. Offering and assisting with bedpans and urinals. Assisting as assigned with light laundry needs. 15. Performing range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency. 16. Providing respite for patient's/family/caregiver when on-site, as appropriate. 17. Keeping the patient's living area clean and orderly, as assigned. 18. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 19. Participates in quality assessment performance improvement teams and activities. 20. Respectful of patient and family/caregiver environment and patient's personal needs. At least 18 years of age. 5. Ability to read and follow written instructions and document care given. 6. Qualifications Classroom Instruction Each student is required to complete 36-40 hours of classroom work. All students are required to pass (with 80%) all of the tests administered during the classroom portion. (Each student has 3 attempts to pass) If a student does not pass the classroom phase of the course they will not be placed into the clinicals. Clinical Practicum Each student is required to complete 75 hours of supervised practicum in a long-term care facility. During the practicum, the student must perform all required skills successfully. The first 16 hours of practicum is with the class instructor. Absence during this time will result in being excused from the program. During the practicum, the student may be assigned to work with a preceptor who will assist the student in organization and time management. All skills checklists booklets (RCP's) not signed during the classroom must be demonstrated and signed off during the clinicals by the clinical instructor, or other licensed nursing professional. Completed checklist (RCP) must be submitted at the completion of the clinicals with all required skills completed or will not be eligible for the required state exam. A “clinical sign-in record” is supplied to each student. This form must be completed by the student and signed by a qualified licensed professional on a daily basis. The student must include the following information on the sheet: date and actual hours worked, signature of student, cosigned by licensed supervisor. Additional Information Students are required to pass a criminal background check along with a TB test and physical prior to starting the course. Attendance to all classroom sessions is mandatory in order to meet the program requirements as set by the Indiana State Department of Health. Unsatisfactory performance or attendance issues may result in dismissal from the course/program. Students should check with the facility for details on the dress code requirements. When a student that successfully completes the course he/she will be awarded a certificate of completion, will get a scheduled test date, and will have the scheduled test paid for by Miller's Merry Manor if hired and in good standing as an employee. If the student does not pass the 1st test it may be retaken and paid for by Miller's a second time. As a CNA/Wellness Instructor for Restorative Care, you will: Report to the Director of Rehabilitation / Therapy and work exclusively with the therapy team to provide knowledge and expertise in delivering appropriate wellness, fitness, and exercise programs. Receive full restorative training provided onsite. No restorative experience necessary, (CNA Certification Required) Assist in the restorative care of patients as directed. Exclusive restorative responsibilities (Ambulation, transfer training, bed mobility, active/passive range of motion, dressing/grooming, splint/prosthesis care) with limited or none of the “less desirable” tasks that often accompany a CNA position.
