What we're looking for

Other requirements
* Successful completion of USRC training program approved by the Medical Director, including demonstrated competency; this includes successful completion of USRC training course in the theory and practice of hemodialysis within 8 weeks of hire.
* CPR certification required within 90 days of hire or as required by the state.
* Patient Care Technician certification must be obtained through a state certification program or national commercially available certification program within 18 months of hire date and maintained in good standing thereafter. Specific state requirements may be applicable and required.

Overview

Assignments are made based on the plan of care and the aide's training and competency.
Key Responsibilities
Provide personal care as assigned, including bathing, dressing, grooming, toileting, oral hygiene, and skin care.
Assist with ambulation, transferring, repositioning, and range-of-motion exercises as instructed.
Observe and report changes in the patient's physical or emotional condition to the supervising nurse.
Assist with meals, feeding, and hydration support, as appropriate to the care plan.
Provide companionship, emotional support, and socialization to patients, fostering comfort and dignity during care.
Take and record vital signs (e.g., temperature, pulse, respiration, blood pressure) if trained and assigned by the nurse.
Offer emotional support and maintain a respectful and compassionate presence in the care setting.
Maintain a clean and safe patient environment, including light housekeeping related to direct care.
Provide short-term respite to family caregivers during scheduled visits, when appropriate.
Document care provided in accordance with agency procedures and timelines.

Qualifications
Maintains active certification as a nursingassistant (CNA) in good standing, with completion of a state-approved training program that includes at least 75 hours and supervised clinical experience.
Current CPR certification.

Understanding of and alignment with the hospice philosophy and the needs of terminally ill individuals.
Ability to work in home and facility settings with reliable transportation and valid driver's license.
