DUTIES:
Assists the Physical Therapist in implementing individualized physical therapy care plans in alignment with the patient's POC.
Performs daily treatment interventions to help patients achieve their therapy goals.
Guides patients through prescribed exercise routines, including passive, assistive, and resistive exercises to improve strength, muscle tone, and range of motion.
Participates in developing tailored home exercise programs based on patient needs.
Assists patients with gait training using parallel bars, walkers, crutches, canes, braces, and related assistive devices.
Performs therapeutic activities such as massage and body mechanics instruction.
Observes and documents changes in patient condition during treatment.
Accurately records patient responses and progress in patient charts.
Refers patients to the supervising PT when reevaluation is indicated.
Communicates patient progress and treatment effectiveness with the clinical team; may suggest modifications to the treatment plan.
Participates in staff in-services and education programs.
REQUIREMENTS:
Current, active California Physical Therapist Assistant (PTA) License
Current CPR certification

Duties and Responsibilities
Effectively communicates with participants, families, staff and other agencies and departments.
Drives a specially equipped van to deliver physically limited persons to varied destinations.
Assures wheelchair lifts and tie down equipment are properly operated and participants are delivered to appointments in a timely manner.
Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery routes.
Assists passengers on and off the van in a safe and courteous manner. Will work with medical equipment and manage other emergencies that may arise.
Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members.

The driver must exhibit exceptional customer service to patients, families, case managers, facilities, physicians, and nurses.
Completes driver log each time the vehicle is used. Keep an accurate record of all facility transports, mileage and times. Provides facility administrator with completed logs.
Inspects the company vehicle monthly using the Vehicle Maintenance Log.
Reports all maintenance or vehicle needs to the facility administrator timely and follow up as appropriate.
Assists with maintaining all maintenance records and other records regarding the company vehicle.

• Participate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids.
• Wash hands before and after performing any service for the resident.
• Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility.
• Follow established safety precautions in the performance of all duties.
• Keep excess supplies and equipment off the floor. Store in designated areas.

• Clean, disinfect, and return all resident care equipment to its designated storage area after each use.

Maintain and keep updated all medical records and documentation pertaining to the patients.
Assist with Purchasing and ordering of medical supplies
Assists in education with pregnant patients about prenatal care and assists primary counselor in coordinating outside services as needed.
Attend team meetings and complete all training courses timely as required.

RESPONSIBILITIES
Hospital Corpsmen (HM) assist health care professionals in providing medical care to Navy personnel and their families. They may function as clinical or specialty technicians, medical administrative personnel and healthcare providers at medical treatment facilities. Specifically, Hospital Corpsmen may be called upon to:
Perform emergency medical treatment on SEALs, Seabees, Marines and other military personnel injured in the field, as well as on Sailors aboard ships or aircraft
Perform emergency dental treatment as well as construct dental crowns and bridges, process
dental X-rays and operate X-ray equipment
Serve as an operating room technician for general and specialized surgery
Help administer a wide range of preventive care and medications, including immunizations and intravenous fluids
Conduct physical examinations and assisting in the treatment of diseases and injuries
Maintain patient treatment records, conduct research and perform clinical tests
Assist Navy Physicians and Nurses in a variety of medical fields, including, but not limited to: radiology, physical therapy, phlebotomy, dental, surgery, family medicine, pathology, womens health and more
