Under the direction of the Pharmacist, this position will be responsible for day-to-day comprehensive pharmacy technician services in a complex healthcare environment. This may include dose dispensing technology and sterile medication compounding, and chemotherapy medication compounding. The Pharmacy Technician III will work in advanced technical areas, such as satellite operating room and intensive care unit, as well as be efficient in tech check tech and code response practices.
The Pharmacy Technician III will function as a subject matter expert in at least two sophisticated technical areas and can strategically use interpersonal, customer service, decision-making, and collaboration skills, to support and influence operational objectives.
**Job Responsibilities:**
+ Partner with care providers and pharmacists to ensure that patient receives the needed medication accurately, safely, in accordance with all legal requirements and policies and in a professional and timely manner.
+ Under the direction of the pharmacist, receive prescription/orders from electronic medical record, process manually or using the appropriate computer system and protocols, prepare drug therapy and/or dispense medication in an accurate and timely manner in accordance with all legal, regulatory agency and Fairview requirements.
+ Maintain the IV hoods or isolators, clean room, ante room according to manufacturer's specifications and regulatory requirements as applicable.
+ Coordinate ordering, delivery of medications, supplies and equipment to patients and clinical areas as requested.
+ Participate, coordinate or lead regulatory, quality assurance and process improvement activities and other projects as assigned.
+ Facilitate the training of pharmacy technicians.
+ Other responsibilities as assigned including but not limited to: data collection, reporting, process improvement and maintenance of patient education materials.
**Required** **Education**
+ High school diploma or equivalent.
**Required Experience**
+ Six months of sterile compounding experience.
+ One year of pharmacy technician experience in a complex healthcare environment.
**Required License/Certification/Registration**
+ Eligible to become registered as a Pharmacy Technician with the MN State Board of Pharmacy and eligible to become certified with the Pharmacy TechnicianCertification Board.
+ Nationally certified or become certified within 12 months of employment. Certifying exams must have National Commission for Certifying Agencies (NCCA) accreditation, with examples including, Pharmacy TechnicianCertification Board (PTCB) exam and The Exam for Certification of Pharmacy Technicians (ExCPT).

The purpose of this position is to improve population-based and member specific medication related outcomes. As part of the Medication Therapy Management (MTM) and pharmacy quality team, you will work closely with our members, their pharmacies and care teams to optimize medication regimens. This includes working with members at risk for non-adherence, those at high risk for adverse events from their medications due to multiple co-morbid conditions, and polypharmacy. This position will receive targeted member lists, perform outreach to those members, with the intention of ensuring members are consistently adherent to their medications, including scheduling an appointment with the MTM pharmacist.
Your Responsibilities
Support pharmacists by pre-charting and reviewing pharmacy claims information to assist with member outreach
Conduct outbound calls for refill reminders, adherence checks, and follow-up.
Assist with medication reconciliation by loading prescription claims and medication administration records into a member's medication list within an electronic health record.
Document clinical responses from member outreach within an electronic health record to track interventions and medication adherence issues.
Schedule comprehensive medication management visits with pharmacists for members who have multiple medications/chronic conditions, including cross departmental referrals.
Participate in other population health-based interventions that impact Star Ratings and/or HEDIS quality measures.
Required Skills and Experiences
5+ years of related experience. All relevant experience including work, education, transferable skills, and military experience will be considered.
Current Pharmacy Registration and Certification (Certified Pharmacy Technician)
At least 5 years of experience in ambulatory care/clinic/outpatient pharmacy, discharge, or health plan quality setting
Clear knowledge of the top 100 commonly prescribed medication names and classes
Experience researching medication claims history and/or escalations
Direct patient care experience and/or customer service experience
Microsoft/Outlook experience
High school diploma (or equivalency) and legal authorization to work in the U.S.
Preferred Skills and Experiences
Experience and knowledge working with MTM program(s) in a clinic/retail setting
Experience with Medicare Star Ratings and HEDIS Quality measures
