The **Pharmacy Technician II- Adherence** will be part of the Clinical Pharmacy Care Center (CPCC) team.
The CPCC is a clinical pharmacy call center that services Medicare, Medicaid, and the Commercial member populations. The members we reach out to have been identified as having gaps in care. Our goal is to close those gaps through education and assistance.
**How you will make an impact:**
Primary duties may include, but are not limited to:
+ Verifies member information and inputs data for the pharmacists.
+ Educates members about prescriptions and medication resources or assistance programs.
+ Educates members on medication adherence-related topics to align closely with STARs Ratings and HEDIS quality measures.
+ Conducts outreach to members or providers to close care gaps and improve outcomes, while also facilitating the scheduling of preventive tests or screenings and providing access to medication resources or assistance programs for members.
**Minimum Requirements:**
+ Requires an active, professional license, if required by state law, State Pharmacy TechnicianCertification or National Certification based on applicable state(s) requirements, to practice as a Pharmacy Technician within the scope of practice in a state or territory of the United States.
+ Requires H.S. diploma or equivalent and minimum of 2 years of pharmacy experience; or any combination of education and experience, which would provide an equivalent background.
**Preferred Skills, Capabilities and Experiences:**
+ Excellent communication skills with Commercial members a plus.
+ Call center experience with high call volumes and multitasking with computer systems preferred.
+ Experience in discussing medication adherence a plus.
+ Patient care experience preferred.

Summary
The Pharmacy Tech Medication Adherence Specialist (Pharm Tech) supports meeting HEDIS/Stars Medication Adherence and Utilization measure goals within the Community Health Organizations (CHO's) participating in the Carina's Accountable Care Organization. What You'll Do
Interact with operational and clinical leadership to assist in the identification of Medication Adherence/Utilization opportunities and will provide targeted support to CHOs to improve outcomes in Carina's value-based contract initiatives.
Improve medication adherence rates by identifying barriers that prevent patients from taking medication regimens appropriately and work with care team members to help patients be successful in managing their health.
Telephonic and electronic outreach to patients based on internal and payer reports that identify those who may need additional help in managing medications and getting timely refills.
Coordinate with members of the patient care team, including providers, pharmacists, nurses, and patient care coordinators to address patient needs related to medication use.
With a strong knowledge of pharmacy workflows, insurance plans, and available resources, help patients navigate the complexities of obtaining prescription medications and refills.
Assist the department, direct supervisor and Carina in the development of education tools related to Medication Adherence/Utilization.
Support and assist the Pharmacist in the creation of education that will train members of the patient care team as well as Carina staff, for Medication Adherence/Utilization.
Maintains a database with the results of outreach calls to patients and reports on progress and statistics on the measures.
In collaboration with the Pharmacist, this role provides monthly or ad hoc feedback on opportunities to improve Medication Adherence/Utilization.

What We're Looking For
High School diploma or equivalent.
Certified Pharmacy Technician (CPhT), preferred with at least 3-5-year community/retail pharmacy technician experience.
Medical Assistants or LPNs will be considered with experience in HEDIS/Stars measures, specifically Medication adherence/utilization measures
Fluency in written and spoken English, with reading comprehension skills. Bilingual in Spanish would be helpful
Experience with HEDIS Medication Adherence and Utilization measures
Knowledge of several EHR systems preferred (ECW, Athena, Greenway Intergy, Epic).
Communicate effectively with team members and clients

Exceptional interpersonal, public speaking, and presentation skills to deliver training and education is preferred.

The Pharmacy Technician II- Adherence will be part of the Clinical Pharmacy Care Center (CPCC) team.
The CPCC is a clinical pharmacy call center that services Medicare, Medicaid, and the Commercial member populations. The members we reach out to have been identified as having gaps in care. Our goal is to close those gaps through education and assistance.
How you will make an impact:
Primary duties may include, but are not limited to:
* Verifies member information and inputs data for the pharmacists.
* Educates members about prescriptions and medication resources or assistance programs.
* Educates members on medication adherence-related topics to align closely with STARs Ratings and HEDIS quality measures.
* Conducts outreach to members or providers to close care gaps and improve outcomes, while also facilitating the scheduling of preventive tests or screenings and providing access to medication resources or assistance programs for members.
Minimum Requirements:
* Requires an active, professional license, if required by state law, State Pharmacy TechnicianCertification or National Certification based on applicable state(s) requirements, to practice as a Pharmacy Technician within the scope of practice in a state or territory of the United States.
* Requires H.S. diploma or equivalent and minimum of 2 years of pharmacy experience; or any combination of education and experience, which would provide an equivalent background.
Preferred Skills, Capabilities and Experiences:
* Excellent communication skills with Commercial members a plus.
* Call center experience with high call volumes and multitasking with computer systems preferred.
* Experience in discussing medication adherence a plus.
* Patient care experience preferred.
