Job Summary

The medication history pharmacy technician, under the supervision of the medication history pharmacist, will systematically interview the patient, family, and/or caregiver to obtain a complete list of home medications. This medication list will then be confirmed with a second source that may include a pharmacy, mail order, or nursing facility documentation. Following completion, the pharmacy technician will document findings in the medical record and communicate pertinent medication information with the medical team, nursing staff and pharmacy staff.
There is opportunity for an 8 on-6 off schedule (10 hour shifts) or for a regular 5-day schedule (8 hour shifts).
