    This position will be part of Ashtabula Regional Medical Center's new patient care tower. The patient care tower will be a 115,000 square foot multi-story building with 55 new private patient rooms, including a new Intensive Care Unit; five new operating rooms in the Surgical Suite; and 24 new rooms in the Emergency Department. The new patient rooms will have private bathrooms and showers and will be more spacious and comfortable for families while visiting their loved one. The new tower will create an even better environment for our patients and caregivers and incorporates the latest technology in healthcare. Caregiver input was a critical piece of the tower design process. Caregivers were involved to help ensure the tower was designed around their workflow, not the other way around. They will deliver services in partnership with Montgomery County's Family Treatment Court (FTC), and is designed to improve outcomes for Montgomery County families facing substance abuse and child welfare issues. Services delivered by the CPS will meet the program objective to protect children, reunite families and expedite permanency. The CPS will have knowledge of community services and the family court process; ensuring service coordination and providing services that support recovery, build resilience, and help create dramatic differences in the lives of individuals and families struggling with substance abuse and other life challenges. This staff position will be based at WestCare Ohio/East End Community Services and provide services within the Montgomery County Juvenile Court and other community locations. This compassionate, self-motivated, and non-judgement peer will help individuals identify personal goals, strengths, and interests to prevent relapse and promote long-term recovery. Essential Job Functions include those listed below. Essential Job Functions: Attends weekly Family Treatment Court proceedings; Assesses the needs, conditions, and desires of identified program participants; Works collaboratively with all program partners such as Family Treatment Court Program Partners to ensure continuous coordination of services for peers and their families; Conducts data collection and ensures timely delivery of reports and other materials as directed by WestCare, program funders, and the FTC program. Uses own unique, life-altering experience to guide and support peers and their families; Mentors and develops meaningful and trusting relationships with program peers struggling with drug addiction, mental health crisis and child welfare issues; Meets regularly with partners to support and advocate for peers and families engaged in WestCare, Family Treatment Court program. Bathing: provides assistance with personal hygiene (i.e. shaving, dental hygiene, nail care, hair care, etc.) Mobility: uses various devices that enable resident to ambulate independently (i.e. cane, walker, wheelchair, crutch, etc.) Dining: meal time reminders, sets up and cleans tables as needed, transports food from buffet to table, waits tables as needed, room service as needed. Toileting: assists in transfer and dressing, assists in applying incontinence devices. Maintains current CPR and First Aid skills. Other duties as assigned. What do I need for this role? High School Diploma or equivalent. Training in geriatrics (nursing, social work, rehabilitation or therapeutic recreation) or sufficient related prior work experience is preferred but not required.
