RESPONSIBILITIES
As a Navy Chaplain, your job spans a broad range of duties. You will support fellow servicemembers during their most joyful and most difficult moments. Your responsibilities might be to:
Conduct worship services in a variety of settings
Perform religious rites and ceremonies such as weddings, funeral services and baptisms
Counsel individuals who seek guidance
Oversee religious education programs, such as Sunday school and youth groups

RESPONSIBILITIES:

MINIMUM QUALIFICATIONS:
BASIC KNOWLEDGE: Must be certified, or eligible for certification, as a Board-Certified Chaplain by and in good standing with a national chaplaincy certifying body, such as the Association of Professional Chaplains, the National Association of Catholic Chaplains, the National Association of Jewish Chaplains, and the Spiritual Care Association.This certification requires a college degree, a master's level degree in a field related to health care chaplaincy, a minimum of four units of clinical pastoral education required, and a rigorous professional and per review process. EXPERIENCE: Between six months and one year of clinical experience preferably in a health or mental health setting with an emphasis on populations to be served in the assigned area. Computer literacy and a skill level in Microsoft Office Suite of intermediate Word, Excel, and PowerPoint. INDEPENDENT ACTION: Performs independently within department policies and practices. Refers specific complex problems to manager and director where clarification of departmental policies and procedures may be required. SUPERVISORY RESPONSIBILITY: May provide supervision to students.

Responsible for the delivery of pastoral and spiritual care to patients, their loved ones, and staff on all campuses of the Medical Center. Offers a multi-faith presence that is responsive to the cultural, religious, and spiritual diversity of patients and their loved ones.
I. Major Responsibilities:
1. Assesses patient and family spiritual needs and develops pastoral care plan in response to identified needs.
2. Responds to pastoral care emergencies and consults in a timely manner. Participates in provision of 24-hour coverage. Participates in Medical Center's response to disasters, community traumas, and related situations.
3. Documents pastoral care assessments, visits and interventions in the patient's record to contribute to holistic patient centered care.
4. Collaborates with interpreter services to provide culturally competent care of patients and their loved ones. Consults with Medical Center staff regarding provision of care that is sensitive to the diversity of patients' spiritual/religious/cultural values.
5. Provides spiritual care to patients, their loved ones, UMMMC staff and students drawing from their spiritual resources to creatively cope with illness, grief, trauma, stress, and death. Includes but not limited to administering sacraments, providing rituals, memorial services, crisis intervention and end of life care.
6. Collaborates with the interdisciplinary Palliative Care team in the provision of care for patients and their loved ones living with advanced illness and/or addressing end of life concerns. Counsels patients and their loved ones regarding ethical decision making, goals of care, and the inclusion of their spiritual/religious/cultural values in their decisions. Participates in interdisciplinary family meetings with patients and their loved ones.
7. Develops, coordinates, and facilitates public worship/spiritual practices appropriate to diverse setting of the Medical Center.
8. Promotes the integration of pastoral/spiritual care into the life and service of the institution; provides leadership on hospital medical center committees and initiatives; provides education related to religious/spiritual/cultural diversity, grief and loss, and wellness.
9. Works with Patient Access Services to maintain accurate religious affiliation information for patients and a notification system for area spiritual leaders to facilitate their support of hospitalized members of their respective faith communities.
10. Participates on Medical Center committees and initiatives. Offers education on a variety of topics related to faith, religious systems, wellness, grief and loss to medical center staff/students, and to community groups.
11. Assists in the coordination of pastoral care volunteers as assigned including Eucharistic ministers.
12. Maintains good standing in own faith group and certification with professional cognate group such as the Association of Professional Chaplains, the National Association of Catholic Chaplains, the National Association of Jewish Chaplains, or the Canadian Association for Professional Practice and education.

Required:
1. Masters' degree of Divinity or related degree from school accredited by Council for Higher Education Accreditation (CHEA).
2. Current ordination or commission to function in a ministry of pastoral care, and current endorsement by a recognized faith group for ministry as a chaplain.
3. Certification by the Association of Professional Chaplains, the National Association of Catholic Chaplains, the Neshama Association of Jewish Chaplains, the Canadian Association for Pastoral Practice and Education, or eligibility for certification within three years.
4. Valid driver's license.

**Duties/Responsibilities** + Provides care and counseling including spiritual counseling, crisis ministry, bio-ethics counseling, bereavement pastoral counseling and sacramental ministry. + Completes psycho spiritual assessment of patient and family. + Develops psycho spiritual treatment plan with patient, family and hospice interdisciplinary team.
+ Assures continuity and accessibility of familiar religious resources and traditional practices to patients and their families.
+ Consults with other disciplines and staff on psycho spiritual factors of hospice care.
+ Acts as liaison and consultant to area community clergy.
+ Provides educational programs for community clergy, community providers and hospice staff.
+ Coordinates memorial services sponsored by hospice as directed by the Coordinator of Pastoral Care
+ Provides support to hospice staff
+ Participates in the Agency Quality Assurance Program.
+ Assumes administrative on-call responsibility as needed

**Education/Experience Required**
+ Master of Divinity, Master of Theology or Masters in Pastoral Counseling
+ Minimum of four units of Clinical Pastoral Education (C.P.E.), two basic, two advanced or equivalent training through an accredited pastoral counseling program.
+ Comprehensive knowledge of hospice counseling practice
+ Excellent written, oral and presentation skills
+ Demonstrated leadership and customer service approach in a hospice or related setting

**Physical/Environmental Requirements**
Majority of work is done while sitting with intermittent standing and walking and occasional bending, lifting, pushing and pulling. Handling light materials. Requires sustained visual attention and concentration. Physical effort required to lift, carry, push and pull light to heavy equipment, objects or supplies. Physical strength to transfer patients weighing as much as 100 to 250 lbs.

$34.00 - $42.00

Summary:
As a member of the interdisciplinary team, the chaplain works under the supervision of the Clinical Manager and is responsible for providing spiritual support to patients and families consistent with the patient/family's wishes and belief system.
The chaplain is responsible for implementing and coordinating the provision of spiritual support to patients and families in accordance with the hospice plan of care.
Qualifications
Qualifications:
Graduate degree e.g.: Master's Divinity or Theology preferred or equivalent theological education.
